Helicobacter pylori has no effect on plasma ghrelin levels.

OBJECTIVE: Helicobacter pylori is the major etiologic agent for chronic active gastritis, and it also plays a crucial role in gastric and duodenal ulcer disease, as well as in gastric carcinoma. H. pylori infection has been shown to decrease plasma somatostatin (SST) and increase plasma gastrin concentrations. Ghrelin is a recently discovered peptide produced mostly in the stomach of rodents and humans and is secreted into the bloodstream. There is no data in the literature about the relationship between H. pylori and ghrelin. DESIGN: Thirty-nine age- and BMI-matched H. pylori infection posit…

[What role today for Helicobacter pylori in peptic ulcer?].

Helicobacter pylori (H. pylori) and nonsteroidal anti-inflammatory drugs/aspirin (NSAIDs) remain today the main etiologies of duodenal (DU) and gastric (GU) ulcers. In some countries or areas in which prevalence of H. pylori infection has decreased, and probably also in which the consumption of NSAIDs is high, the proportion of ulcers not associated with H. pylori is high. Nevertheless, the proportion of Helicobacter pylori-negative, NSAID-negative ulcers remains low, less than 6% in most studies. Furthermore, this proportion is probably overestimated because the search for the infection and N…

[Association between Helicobacter pylori and gastro-esophageal reflux disease].

The relationship between Helicobacter pylori (H. pylori) infection and gastro-esophageal reflux disease (GERD) is complex. Since some studies have suggested that H. pylori eradication may result in an increased incidence of GERD in duodenal ulcer patients, there have been debates about the protective function of H. pylori infection on GERD. H. pylori-associated antral gastritis can induce increased gastric acid output via increasing gastrin secretion. Changes in gastric acid secretion depend on the distribution (e.g. antral, corpus or pangastritis) or severity of gastritis, not on H. pylori in…

Expression of Helicobacter pylori vacuolating toxin in Escherichia coli.

VacA is a secreted toxin that plays a role in Helicobacter pylori colonization of the stomach and that contributes to the pathogenesis of peptic ulcer disease. Studies of VacA structure and function have been hindered by the lack of an efficient system for expression and genetic manipulation of this toxin. In this study, we developed methodology for expression of a functionally active VacA toxin in Escherichia coli. We then used a high-throughput screen to analyze a library of mutant toxins with pentapeptide insertions and identified six mutants that lacked the capacity to induce vacuolation o…

Is Helicobacter pylori infection associated with chronic idiopathic urticaria?

BACKGROUND: Chronic idiopathic urticaria (CIU) is one of the most frequent skin diseases, however its causes remain unknown in the vast majority of cases. There is increasing evidence for systemic effects of gastric Helicobacter pylori infection, which may result in extra gastrointestinal disorders. Although CIU can result from several causes, a possible relationship between chronic urticaria and Helicobacter pylori has been recently suggested. OBJECTIVE: The aim of this study is to determine the prevalence of Helicobacter pylori infection in a series of patients with CIU, and measure the eff…

[First line therapy for Helicobacter pylori eradication in France].

The available results of triple therapy for the eradication of Helicobacter pylori (H. pylori), as recommended in European countries--i.e. combination of proton pump inhibitor (PPI) and two antibiotics among amoxicillin, clarithromycin, metronidazole for 7 days--lead to rates of failure of about 30%. Several clinical studies have been recently conducted to distinguish factors influencing effectiveness of therapy and to evaluate results of new regimens. Comparative trials have demonstrated the equivalence of omeprazole 20 mg, lansoprazole 30 mg, pantoprazole 40 mg, rabeprazole 20 mg and esomepr…

[Role of Helicobacter pylori in drug-induced gastrointestinal bleeding].

TWO CO-FACTORS: Helicobacter pylori (Hp) infection and ingestion of NSAIDs are two factors which play a major role in the occurrence of gastric and duodenal ulcers, simple or complicated by perforation and GI bleeding. Hp increases the risk of ulcer in patients receiving an NSAID and the effects of Hp and NSAIDs are additive. Following GI bleeding in a patient receiving a conventional NSAID, the eradication of Hp does not make it possible to protect the patient from another episode of GI bleeding and proves to be a much less effective therapy than continuous therapy with proton pump inhibitors…

[Antimicrobial resistance of Helicobacter pylori: clinical and molecular aspects].

Helicobacter pylori is a relevant pathogen for gastroduodenal diseases in human beings. Although its eradication often improves gastroduodenal diseases, H pylori is acquiring an elevated rate of resistance to various antimicrobials, such as metronidazole, clarithromycin, tetracycline and amoxicillin. Multi-drug resistance is a major problem to select the appropriate treatment of infectious diseases. To improve our understanding on the complexity of the problem, in this article we review the resistance mechanisms and give an update on H pylori antimicrobial resistance.

[Helicobacter pylori infection--clinical aspects and indications for treatment].

Helicobacter pylori (H. p.) causes active chronic antrum gastritis in all infected patients. In a relatively small percentage complications of H. p.-gastritis including duodenal ulcer, gastric ulcer, giant fold gastritis, lymphocytic gastritis, autoimmune gastritis, gastric carcinoma and gastric MALT lymphoma may develop. Strongly recommended indications for eradication therapy include gastroduodenal ulcer disease, giant fold gastritis, lymphocytic gastritis, autoimmune gastritis, gastric MALT lymphoma, atrophic gastritis, corpus-predominant gastritis, post gastric cancer resection and patient…

Helicobacter pylori in duodenal ulcer disease and its eradication.

Antral biopsy specimens were processed for Helicobacter pylori by Gram staining, rapid urease test (RUT) and culture from 25 patients with symptoms of duodenal ulcer, amongst whom the positivity rate was 84%. Follow up of 16 patients after appropriate therapy showed complete regression of the disease in 87.5% of cases whereas in 12.5% of cases a decrease in the extent of duodenal ulceration was noted.

Helicobacter pylori infection in skin diseases: a critical appraisal.

More than 50% of the human population have long-term Helicobacter pylori infection, causing, in some cases, severe diseases such as peptic ulcers and stomach cancer. In the last few years several extra-gastrointestinal disorders have been associated with H. pylori infection. This review summarized the current medical literature, identified through hand searching and MEDLINE research, including our own studies, with regard to H. pylori and skin diseases. From the literature it can be seen that the role of H. pylori in skin diseases is still a controversial subject. Randomized controlled trials …

Analysis of cell damage in Helicobacter pylori-associated gastritis.

Coumarins, a diverse class of naturally occurring and synthetic compounds, have garnered significant attention in drug development due to their ability to modulate enzymatic activity. These molecules exhibit dual functionality, acting as both enzyme inhibitors and enzyme inducers, influencing various biochemical pathways. Their structural flexibility allows for diverse interactions with key enzymes, making them valuable in pharmacology, particularly in the treatment of cancer, cardiovascular diseases, and neurodegenerative disorders. This comprehensive review explores the intricate mechanisms

[Epidemiology and clinical role of childhood Helicobacter pylori infection].

Colonization of Helicobacter pylori depends on virulence factors of the organism, host factors and environmental factors. The prevalence of H. pylori differs between developing and Western developed countries, suggesting that the socioeconomic status influences the acquisition of the infection. The prevalence of Japanese children has been almost equal to that of Western countries. Although a main mode of transmission remains unclear, childhood is critical for acquisition of the infection. H. pylori causes gastritis (especially nodular gastritis) and duodenal ulcers in children but rarely gastr…

Beneficial effect of Helicobacter pylori eradication in dermatologic diseases.

BACKGROUND: A relationship between skin diseases, particularly rosacea and chronic urticaria, and H. pylori infection has been suggested. METHODS: We preformed a prospective evaluation of the effect of H. pylori eradication in patients with a variety of chronic skin diseases. Patients were followed monthly for at least one year after cure of the infection. The effect of therapy was scored using a three point scale: complete remission (> 90% improvement), partial remission (50-90% improvement) or no improvement (< 50 improvement). The relationship between response and anti-H. pylori Ig G and E

Intragastric nitric oxide/nitrite in Helicobacter pylori-infected subjects.

BACKGROUND: Nitrite (NO2-) in swallowed saliva is reduced to nitric oxide (NO) and other nitrogen oxides by the intragastric acidity. This mechanism is probably important for the intragastric clearance of ingested micro-organisms and nitrosating compounds. The study examines the balance between intragastric NO and NO2- in relation to endogenous acid production and infection with Helicobacter pylori. METHODS: Six healthy H. pylori-negative and six H. pylori-positive volunteers with no known gastroduodenal pathology were examined after an overnight fast. Gastric NO was measured using a chemilum…
