A template approach for detecting fractures in adults sustaining low-energy ankle trauma.

Low-energy trauma such as sprains account for 75% of injuries to the ankle; therefore, ankle pain is one of the most common problems encountered by emergency physicians. A close inspection of the radiographs is often required for diagnosis and this depends on the knowledge of high-risk areas since proper treatment relies on the accuracy of the initial diagnosis. In this article, we will present a template that has helped to increase the accuracy for detecting subtle avulsion fractures, chip fractures, and osteochondral fractures of the hind foot in adults sustaining low-energy ankle injuries.

Ankle sprain complications: MRI evaluation.

Sprains are disruptions of the ligamentous anatomy about a joint. The ankle sprain is one of the most common injuries seen in podiatric and orthopedic practice. It usually is incurred from an inversion force on the ankle, but eversion forces also can traumatize the ankle. Many times, this injury is taken for granted because of the frequency of its presentation. The patient usually is given appropriate initial care, but the patient can experience continued or residual pain. Podiatrists have found this problem is common and have come to recognize that secondary or accessory injuries occur that s

Comparison of conventional treatment and supervised rehabilitation for treatment of acute lateral ankle sprains: a systematic review of the literature.

STUDY DESIGN: Systematic literature review. OBJECTIVE: To compare the effectiveness of conventional treatment complemented by supervised rehabilitation training (supervised exercises) with conventional treatment alone for the rehabilitation of acute lateral ankle sprains. BACKGROUND: Conventional treatment is advocated as a preferable treatment strategy. Whether supervised exercises should complement conventional treatment is unclear. METHODS AND MEASURES: We searched 5 computerized databases from 1966 to March 2004, checked the reference lists of all studies that fulfilled our eligibility

A theoretical model for treatment of soft tissue injuries: treatment of an ankle sprain in a college tennis player.

OBJECTIVE: To present theories of manual treatment approaches to soft tissue injuries using an example case report. CLINICAL FEATURES: A college tennis player with an ankle sprain for 6 weeks, not responding to standard treatment, was unable to play tennis or compete in tournaments. INTERVENTION AND OUTCOME: Soft tissue treatment was applied to the ankle for 2 visits. The patient experienced complete resolution of the problem and returned to play without relapse during a 9-month follow-up period. Theories of myofascial distortion treatment are discussed. CONCLUSION: This treatment approach

Lateral ankle sprain: isokinetic test reliability and comparison between invertors and evertors.

OBJECTIVE: Evaluate reliability of isokinetic tests for inversion and eversion in individuals with recurrent lateral ankle sprain and compare inversion to eversion muscle performance in the same ankles. DESIGN: Reliability was evaluated by comparing the results of two ankle inversion/eversion isokinetic tests. Inversion and eversion were compared with the results presented by the tests. BACKGROUND: Previous studies showed reliable isokinetic tests for ankles with no sprain history. It is unknown if such tests can also be performed in lateral sprained ankles. It is unclear whether there is di

Posterior olecranon resection and ulnar collateral ligament strain.

Valgus extension overload is well described in the throwing athlete. The surgical treatment involves posterior olecranon osteophyte resection. It is not known how much of the posterior olecranon can be resected before increased stress is placed on the ulnar collateral ligament. Ten cadaveric arms underwent posterior olecranon resection in 4-mm increments. After each bone resection, three different valgus loads were applied to the elbow at 70 degrees and 90 degrees of flexion. Ulnar collateral ligament strain significantly increased with each load. Significantly more strain occurred on the ulna

Low-back pain in athletes.

While most occurrences of low-back pain in athletes are self-limited sprains or strains, persistent, chronic, or recurrent symptoms are frequently associated with degenerative lumbar disc disease or spondylolytic stress lesions. The prevalence of radiographic evidence of disc degeneration is higher in athletes than it is in nonathletes; however, it remains unclear whether this correlates with a higher rate of back pain. Although there is little peer-reviewed clinical information on the subject, it is possible that chronic pain from degenerative disc disease that is recalcitrant after intensive

Lumbar spine region pathology and hamstring and calf injuries in athletes: is there a connection?

This paper discusses the theory that subtle lumbosacral canal impingement of the L5 nerve root may be a relatively common occurrence in older footballers and may in fact be a common underlying basis for the age related predisposition towards hamstring and calf strains.

Surgical versus conservative treatment for acute injuries of the lateral ligament complex of the ankle in adults.

BACKGROUND: Inversion injuries, primarily sprains, of the ankle are one of the most commonly treated injuries. The three main treatment modalities for acute lateral ankle ligament injuries are immobilisation with plaster cast or splint, 'functional treatment' comprising early mobilisation and the use of an external support (e.g. ankle brace or taping), and surgical repair or reconstruction. OBJECTIVES: We aimed to compare surgical versus conservative treatment for acute injuries of the lateral ligament complex of the ankle in adults. SEARCH STRATEGY: We searched the Cochrane Musculoskeletal

Acupuncture analgesia in a new rat model of ankle sprain pain.

The lack of suitable experimental animal models for persistent pain showing clear acupuncture analgesia, has been the major stumbling block in the investigation of the physiological mechanisms of acupuncture analgesia. The present study developed a new rat model of ankle sprain pain and the effect of electroacupuncture (EA) on this model was examined. A common source of persistent pain in humans is the lateral ankle sprain. To model this condition, the rat's right ankle was bent repeatedly, overextending lateral ligaments, for 4 min under halothane anesthesia. The rat subsequently showed swell

Ergonomic exposures in apple harvesting: preliminary observations.

BACKGROUND: Recent research on occupational injury to apple harvest workers in New York and Pennsylvania indicates that muscle strain is one of their leading problems. A descriptive study of ergonomic risks for strains of the shoulder and lower back among apple harvesters was carried out to identify opportunities for intervention. METHODS: Based upon pilot observation in four orchards, a Posture-Activities-Tools-Handling (PATH) data collection template was designed and used to analyze the work of fifty-one workers in eight additional apple orchards. RESULTS: Physical loading on the shoulder

Pseudoaneurysm as a complication of ankle arthroscopy.

We describe a case of pseudoaneurysm of the anterior tibial artery as a complication after arthroscopic ankle synovectomy, in which standard anterolateral and anteromedial portals were used. Pseudoaneurysm has been previously reported as a complication in ankle arthroscopy with the use of the anterocentral portal. Previously described anatomic variations of the tibial artery and its close relationship with the anterior ankle capsule may complicate arthroscopic surgery, especially when aggressive synovectomy is performed. Anterior tibial artery aneurysm is a rare complication of ankle arthrosco

The posteromedial impingement lesion of the ankle. A series of six cases.

We report on a series of six cases of posteromedial impingement lesion of the ankle operated on during a 3-year period with excellent or good results and a return of the patients to preinjury levels of activity. This lesion occurs, on occasion, after a severe ankle-inversion injury in which the deep posterior fibers of the medial deltoid ligament become crushed between the medial wall of the talus and the medial malleolus. Initially, posteromedial symptoms do not predominate, compared with the symptoms of the lateral ligament disruption, and they usually resolve without specific treatment. Occ

Salter-Harris I fractures of the distal radius misdiagnosed as wrist sprain.

Over a period of 5 years, 65 children under 12 years of age were diagnosed as having "wrist sprain" at the American University of Beirut Medical Center, Emergency Department. Thirty-eight children were reevaluated; 33 patients (86.8%) were diagnosed as having a Salter-Harris I fracture of the distal radius that necessitated further treatment to relieve symptoms and allow early wrist function. The diagnosis of "wrist sprain" is often inadequate in establishing a proper treatment regimen. Rather, by taking a careful history, performing an exacting examination, and utilizing appropriate diagnosti

Recurrent posterior thigh symptoms detrimental to performance in rugby union: predisposing factors.

Recurrent hamstring injury is a very common problem in rugby union, but has been largely ignored in the literature. It is concluded that a multifactorial aetiology may be present, and that these symptoms may be part of a continuum of symptoms that may lead to more serious injuries. Effective management needs to focus not just locally, but include proximal issues such as lumbo-pelvic stability and correcting lumbar spine dysfunction. The factors addressed in a prevention programme are likely to contribute to performance gains.

MR imaging of the thrower's shoulder. Internal impingement, latissimus dorsi/subscapularis strains, and related injuries.

In conclusion, internal impingement apparently occurs in nearly all patients and is demonstrable on MR imaging. Pathologic changes associated with internal impingement seem to develop with repetitive placement of the arm into a position of extreme external rotation and abduction. Findings may include lesions of the posterior superior labrum, undersurface irritation, or tearing of the supraspinatus-infraspinatus junction near the attachment site and cystic changes of the posterior superior glenoid and posterior lateral greater tuberosity. There is no evidence for a particular sequence of pathol

Effects of reduced ankle dorsiflexion following lateral ligament sprain on temporal and spatial gait parameters.

Partial rupture of the lateral ligament complex of the ankle is the commonest soft tissue injury affecting the lower limb. The effect of the limitation of motion at the ankle, particularly of dorsiflexion, on gait is unclear. In this study, 34 subjects were measured during their recovery from a partial rupture of the lateral ligament for both range of dorsiflexion and the temporal and spatial parameters of walking. Consistent relationships were identified between the range and the gait variables which were in concordance with the characteristics expected from an antalgic gait pattern.
