Biomedical engineering for the conservation of teeth--the use of a Nd-YAG laser for a treatment of apical focus.

When the apical focus (root cyst) is treated, if the tooth concerned can be preserved, the root canal should first be enlarged and cleaned sufficiently, and then sealed with filling materials. However, if an expensive prothesis is placed on the crown of the tooth concerned, or if the root canal is blocked with a post core of calcification, root canal treatment becomes difficult from a practical point of view. We therefore tried irradiation using the Nd-YAG laser, which is known for its high transmissibility into teeth, to the root canal, and the apical area. In its histological images it is co

Management of Complicated Crown Fracture and Associated Impacted Mesiodens- Case Report with 3yrs Follow-up.

Mesiodens being the most common type of supernumerary tooth, usually results in malocclusion, poor aesthetics and cyst formation if it is not corrected. The management protocol involves surgical removal. The situation can be complicated in young permanent dentition where there are a lot of chances of damage to the permanent dentition during surgical removal of impacted teeth. This article reports a case of impacted mesiodens which was diagnosed during management of complicated crown fracture. The case was followed up to 3years which showed a good bone formation. The article highlights the need

Biologic restoration of a traumatized maxillary central incisor in a toddler: a case report.

Trauma to the anterior teeth is relatively common in young children and teenagers. Traumatized anterior teeth require quick functional and aesthetic repair, and poses a challenge to the dental practitioner owing to the lack of co-operation ceded and the longer time invested. Reattachment of tooth fragment should be the first choice to restoring teeth when a usable fragment is available, since it gives a psychological and aesthetic advantage over the conventional technique. With the vast improvement in adhesive technology, reattachment is definitely a predictable treatment option for very young

Biomechanical effects of attachment designs in maxillary first molar mesialization with clear aligners: a finite element study.

BACKGROUND: Clear aligners face biomechanical limitations in complex tooth movements, particularly first molar mesialization, despite attachment use. Finite element analysis (FEA) can elucidate optimal attachment designs for force delivery. OBJECTIVES: To compare displacement patterns and stress distribution during maxillary first molar mesialization using four attachment designs via 3D FEA. METHODS: A maxillary model (MRI-derived) was created with periodontal ligament (PDL), alveolar bone, and clear aligner (0.75 mm thickness). Five scenarios were simulated: no attachment (Model-I), vertica

Volumetric facial contour changes of immediately placed implants with and without immediate provisionalization.

BACKGROUND: Whether immediate provisionalization can preserve facial tissue contour remains undetermined. The goal of this 12-month randomized controlled clinical trial was to compare three-dimensional (3D) ridge changes after immediate implant placement with and without immediate provisionalization. METHODS: Forty participants with an unrestorable maxillary anterior or premolar tooth were randomized to receive either a provisional crown (test) or standard healing abutment (control) after immediate implant placement. In each participant, three digital models taken before implant surgery, fina

PURPOSE: To evaluate the survival, success, and complication rates of tapered double-lead threads single implants, placed in fresh extraction sockets and healed sites of the posterior jaws. METHODS: The enrolled patients were randomly divided into 2 groups: in the test group (TG), all implants were inserted at the time of tooth extraction; in the control group (CG), all implants were placed 3 months after extraction. The implants were followed for a period of 1 to 3 years after loading. The main outcomes were implant survival, complications, and implant-crown success. RESULTS: Ninety-two pat

Predicting Agenesis of the Mandibular Second Premolar from Adjacent Teeth.

Early diagnosis of agenesis of the mandibular second premolar (P2) enhances management of the dental arch in the growing child. The aim of this study was to explore the relationship in the development of the mandibular first molar (M1) and first premolar (P1) at early stages of P2 (second premolar). Specifically, we ask if the likelihood of P2 agenesis can be predicted from adjacent developing teeth. We selected archived dental panoramic radiographs with P2 at crown formation stages (N = 212) and calculated the likelihood of P2 at initial mineralisation stage 'Ci' given the tooth stage of adja

Retrieving chronological age from dental remains of early fossil hominins to reconstruct human growth in the past.

A chronology of dental development in Pan troglodytes is arguably the best available model with which to compare and contrast reconstructed dental chronologies of the earliest fossil hominins. Establishing a time scale for growth is a requirement for being able to make further comparative observations about timing and rate during both dento-skeletal growth and brain growth. The absolute timing of anterior tooth crown and root formation appears not to reflect the period of somatic growth. In contrast, the molar dentition best reflects changes to the total growth period. Earlier initiation of mo…

Traumatically injured primary incisors: a clinical and histological study.

World-wide epidemiological studies state that from 8 percent to 30 percent of children up to 7 years of age sustain injury to primary incisors, including crown fracture, root fracture, tooth avulsion, and dental displacement, which result in malformation of permanent incisors depending upon their state of development. In this study, traumatized primary incisors were evaluated clinically and radiographically, with the following data recorded: exact or approximate time-interval of traumatic episode(s) and extraction, mobility, color change, sensitivity to percussion/palpation, swelling, sinus tr

Transient root resorption after dental trauma: the clinician's dilemma.

UNLABELLED: The diagnostic shortcomings encountered in the clinical evaluation of pulpal and periodontal healing subsequent to trauma comprise a major problem in dental traumatology. An acute dental trauma may imply impact to the hard dental tissues and damage to the pulp and periodontium (including surrounding alveolar bone). In the case of luxation injuries, the trauma often results in rupture of the neurovascular supply at the level of the apical foramen, whereas in a root fracture, the same occurs at the level of the fracture. Conventional diagnostic criteria for pulpal status (coronal dis

Identification of an unidentified corpse with gingival overgrowth: A case report.

Drug-induced gingival overgrowth (DIGO) is a common side effect of drugs, such as phenytoin, cyclosporine, and calcium channel blockers, which mainly affect the connective tissue of the gingiva. In some cases, severe gingival enlargement may obscure tooth crowns, which are easily recognizable by clinical internists and dentists. However, there are few reports on personal identification using DIGO and drug analysis. An unidentified Asian woman with a middle-to old-aged appearance was found dead in a river. DIGO was suspected based on the findings of severe gingival overgrowth and the results fr…

Vital Pulp Therapy with Calcium-Silicate Cements: Report of Two Cases.

This article describes successful use of calcium-enriched mixture (CEM) cement and Biodentine in apexogenesis treatment in two 8-year-old patients, one with immature permanent molar diagnosed primarily with irreversible pulpitis and the other with partially vital maxillary central incisor. After access cavity preparation, partial pulpotomy in molar and full pulpotomy in central was performed, and the remaining pulps was capped with either Biodentine or CEM cement, in each tooth. The crowns were restored with composite filling material at the following visit. The post-operative radiographic and

Development and growth of adenomatoid odontogenic tumor related to formation and eruption of teeth.

The number of published cases of adenomatoid odontogenic tumor (AOT) has steadily increased, and about half were reported in Asian populations. Although the clinicopathologic profile of AOT has been extensively reported, factual knowledge of its pathogenesis is rudimentary at best, and there is controversy as to precisely which odontogenic epithelium it arises from. AOT is a successional tooth-associated lesion which develops during the mixed dentition. The permanent successor differs from its deciduous predecessor in that it has an eruptive pathway from the dental follicle to the gingiva, the

A spontaneous mutation: amelogenesis imperfecta with cysts in rats.

Amelogenesis imperfecta (AI) is an inherited dental disease of enamel formation in humans, and there are various phenotypes due to the combination of enamel quality and quantity. We encountered four female IGS rats with spontaneous AI including odontogenic cysts in the incisor teeth. Histopathologically, in the incisors of the rats, the enamel organ was disorganized with the remaining enamel matrix residing within the enamel space. The expanding cysts derived from the enamel organ were formed in the periosteal connective tissue on the labial side. At the bottom of the tooth germs, the precurso

Incisor root resorptions due to ectopic maxillary canines imaged by computerized tomography: a comparative study in extracted teeth.

The purpose of the study was to analyze the ability of computerized tomography (CT) scanning to discriminate maxillary incisor root resorptions caused by ectopically erupting canines. Seventeen maxillary incisors were radiographed in vivo by CT scanning. Contiguous transverse CT scans with a slice thickness of 2 mm were exposed perpendicular to the long axis of the lateral incisors and through the crown of the adjacent, ectopically positioned maxillary canine. Each scan was analyzed and the resorptions on the roots of the laterals were graded according to the maximum depth of the cavity. After

Clinical management of injuries to the maxilla, mandible, and alveolus.

Facial bone and especially mandibular fractures are injuries in which first aid and management of late complications require dental professionals, and consequently all dentists should be familiar with the modern principles of their treatment. Tooth involvement is common in mandibular and maxillary fractures and can require more treatment than does the jaw fracture. Nonsurgical treatment of bone fractures involves immobilization, which for the facial bones is achieved with maxillomandibular fixation (MMF) using dental fixed arch bars. Hence, associated tooth injuries are difficult or impossible

The finite element analysis of stress in the periodontal ligament when subject to vertical orthodontic forces.

In the past, vertical intrusive movement of teeth has been considered difficult and most routine clinical vertical movement of teeth has been confined to extrusion. It has been suggested that attempts at intrusion may result in an increased incidence of root resorption and also in occasional devitalization. The displacement and resulting stress fields associated with such treatment can be successfully studied using the finite element method. In the case being considered initial movements are known to be small; therefore, the assumption in the study that the material behaves linear-elastically

Cosmetic concerns provide opportunity for comprehensive dentistry.

This case involved a woman with occlusal dysfunction who was dissatisfied with her smile because of the unsightly display of gold crowns in her posterior and unattractive mottling on her anterior teeth. Factors such as a significant history of clenching and grinding, the management of active disease and infection, and the patient's desire for an ideal or "media-generated" smile increased the difficulty of the case. As part of a comprehensive restorative plan that included disease control treatment, root canal therapy, and orthodontics to idealize tooth positioning, a Kois deprogrammer was used
