The doctor-patient relationship in dermatology, as in all the fields of medicine, is not a neutral relationship, removed from affects. These affects take root in the sociocultural, professional, family and personal history of both persons in the relationship. They underpin the psychic reality of the patients, along with a variety of representations, preconceived ideas, and fantasies concerning dermatology, the dermatologists or the psychiatrists. Practitioners call these "countertransference feelings", with reference to the psychoanalytical concept of "countertransference". These feelings come

Twelve experienced psychoanalysts diagnosed during a depth-psychological examination of 604 patients, coming from two different outpatient institutions, distinct clinical schizoid personality traits of 65 patients (10.7%). A further analysis researched the contribution to patient and therapist bindings and influences on the diagnosis. With the aid of a step by step regressions analysis including "solid" patient and therapist data as well as countertransference reactions of the investigator, we were able to establish an accurate diagnostical model, which enables a correct assignment with the pr

The manifest reason for the hospitalization of psychiatric patients is that someone decides that they are in danger of suicide or homicide, or that they are unable sufficiently to care for or manage themselves. From another perspective, however, hospitalization can be understood as a consequence of limitations on the part of the family, the community, or the therapist in managing the affects generated in relation to these patients' behavior and communications. In other words, it is possible that patients may be hospitalized primarily because people in their lives (and often they themselves) ca

This paper is presented primarily for its historical interest. The author's first attempted publication in psychiatry or psychoanalysis, it was submitted successively to two publications in 1949, rejected by each, and filed away until now. In it, the author suggests that transference phenomena constitute projections, and that all projective manifestations-including transference reactions-have some real basis in the analyst's behavior and represent, therefore, distortions in degree only. The latter of these two suggestions implies a degree of emotional participation by the analyst which is not

It has not been the attempt of this paper to discuss the overall treatment of adults with a history of incest, but rather to elucidate and identify the various transference and countertransference implications that are emergent in psychoanalytic therapy. Despite this, some statements can be made regarding the analysis of these themes and the overall effectiveness of treatment with these patients. Analytically oriented treatment with adults with a history of incest concerns itself with three intertwined aspects. They are the reconstruction, integration, and validation of a past history of traum

This target article focuses on dynamics that arise in three typical ethically complex cases in which psychiatric consultations are requested by physicians: a dying patient refuses life-prolonging treatment, an uncooperative patient demands to be allowed to go outside and smoke, and an angry patient demands to be admitted to the hospital. The discussion canvasses what is at stake morally and clinically in each of these cases and explores clinician-patient interactions, dynamics in relationships between consulting physicians and consultant psychiatrists, patient transference, and physician count

This paper describes an example of acting out of countertransference during a brief but unexpected serious illness. My emphasis is on the regression that occurred with hospitalization and how it dovetailed with a particular patient's transference. A confluence of my residual neurosis, the patient's neurosis, her transference state, her other characteristics (that she was female, for example), plus the fact that she was the last patient contacted, all conspired with the regression from the trauma of being hospitalized to produce the countertransference reaction. This presentation is also intend

A team of caregivers travelled to the favelas of a Brazilian province. They were subjected to emotionally charged situations, working with people faced with extreme poverty and poor health. They highlight a cultural countertransference requiring an objective approach to the psychological and political repercussions of these situations.

Songs that interrupt the psychoanalytic psychotherapist's countertransference reverie are invariably relevant and potentially useful. Like any other countertransference "presence," songs accompanying the narrative may contribute to understanding both patient and analytic process. Taking their intrusive, fecund, pesky presence one step further, song lyrics may be used as metaphor-saturated interventions, helping reach the well-defended patient. Five brief vignettes illustrate the process. Because we must learn to tolerate ordinary unhappiness, Blues music, a carrier of preconscious mythic theme

The psychoanalytic literature has been remarkably silent on the subject of erotic countertransference feelings. The recent emphasis on transference-countertransference enactments in the analytic setting has resulted in increased openness about development of such feelings. Several key themes appear to be involved in the analyst's sexual excitement, including loss of the "as-if" nature of transference and countertransference, a measure of hostility and contempt, the perception of a deficit state in the patient, a defense against loss and mourning, and oedipal and preoedipal enactments involving

Early advances in psychoanalytic knowledge, profound though they were, were incomplete structures to be built upon, modified, and partially discarded. In addition to errors due to insufficient knowledge, Freud's difficulties with Dora stemmed from countertransference. Dora's transference included an identification with a governess/maid. Important oedipal role played by a nursemaid in Freud's life made him vulnerable to being left by Dora. The maid, Monika, "the prime originator" of Freud's neurosis, seduced him, chastised him, and taught him of hell. In his self-analysis she was associated wit

BACKGROUND: People with eating disorders (EDs) such as anorexia nervosa (AN) and bulimia nervosa (BN) can trigger intense affective reactions and impulses to act in their counterparts and in those treating them (countertransference, CT). The aim was to summarize the existing evidence on CT in the treatment of EDs by conducting a scoping review oriented on the model of J. Hayes. This model describes CT according to five dimensions: therapist characteristics, triggers, manifestations, effects and management of CT.
METHOD: The PubMed and EBSCOhost databases were systematically searched for publi

Dr. Gregory Rizzolo's recent JAPA article refers to my book, Lacanian Psychotherapy: Theory and Practical Applications, to support his critique of Lacanian psychoanalysis. I respond here to his characterization of my position on countertransference, as well as Lacan's. Despite our differences on the the texts in question, I suggest that on substance, Dr. Rizzolo and I agree more than we disagree.

The article deals with the psychological background of administration of medication in psychiatry, the meaning of administration of drugs in relation to the doctor-patient relationship. The place of pharmacotherapy among other medical procedures was discussed. The differences in the approach to pharmacotherapy in different therapists was noticed, with the pointing out to two kinds of approaches (symptom oriented and problem oriented). Other than rational motivations for the use of pharmacotherapy was discussed.

In the process of psychoanalysis or psychoanalytical therapy countertransference plays a significant role: On the one hand still unconscious countertransference reactions of the therapist hinder the progress of the patient, especially when they lead to an acting-out of the countertransference resistance. On the other hand every analyzed countertransference reaction helps directly in understanding the patient and furthers of the process of therapy itself. This also applies to analytical child-therapy, which is demonstrated by means of some case histories.

Traditionally, in psychodynamic forms of treatment, the patient's relationship with the therapist is central to eliciting and exploring the patient's characteristic ways of thinking, feeling and behaving. Research has increasingly demonstrated the significance of the therapeutic relationship to the treatment outcome. Since little has been written about the therapist-patient relationship in the treatment of pathological gambling, we shall describe what we have found to be some of the common and problematic transference and countertransference reactions and interactions. Both positive and negati

Polysemous qualities of the religious patient's use of language and metaphor are among the subtle factors involved in the creation of an ambience conducive to projective identification, leading to the evolution of countertransference crises in the psychotherapist. The psychotherapist's activity during countertransference crises consists in converting the patient's unsorted images, memory traces, and affective states that have never before been symbolified into resilient representational material. This requires the psychotherapist's imaginative process, but it also involves the accurate precons
