Classifying hospitals as mortality outliers: logistic versus hierarchical logistic models.

The use of hierarchical logistic regression for provider profiling has been recommended due to the clustering of patients within hospitals, but has some associated difficulties. We assess changes in hospital outlier status based on standard logistic versus hierarchical logistic modelling of mortality. The study population consisted of all patients admitted to acute, non-specialist hospitals in England between 2007 and 2011 with a primary diagnosis of acute myocardial infarction, acute cerebrovascular disease or fracture of neck of femur or a primary procedure of coronary artery bypass graft or

Predicting risk-adjusted mortality for trauma patients: logistic versus multilevel logistic models.

BACKGROUND: Theoretical advantages of random-intercept multilevel (ML) logistic regression (LR) modeling over standard LR include separating variability due to patient-level and hospital-level predictors, shrinkage of estimates for lower-volume hospitals toward the overall mean, and fewer hospitals falsely identified as outliers. STUDY DESIGN: We used Nationwide Inpatient Sample data from 2002 to 2004 to construct LR models of hospital mortality after admission with a principal ICD-9 Clinical Modification injury diagnosis (ICD-9 Clinical Modification codes 800 to 904, 910 to 929, 940 to 957,

A consistent estimator for logistic mixed effect models.

We propose a consistent and locally efficient estimator to estimate the model parameters for a logistic mixed effect model with random slopes. Our approach relaxes two typical assumptions: the random effects being normally distributed, and the covariates and random effects being independent of each other. Adhering to these assumptions is particularly difficult in health studies where in many cases we have limited resources to design experiments and gather data in long-term studies, while new findings from other fields might emerge, suggesting the violation of such assumptions. So it is crucial

Skull Retrieval for Craniosynostosis Using Sparse Logistic Regression Models.

Craniosynostosis is the premature fusion of the bones of the calvaria resulting in abnormal skull shapes that can be associated with increased intracranial pressure. While craniosynostoses of multiple different types can be easily diagnosed, quantifying the severity of the abnormality is much more subjective and not a standard part of clinical practice. For this purpose we have developed a severity-based retrieval system that uses a logistic regression approach to quantify the severity of the abnormality of each of three types of craniosynostoses. We compare several different sparse feature se

Modeling the logistics of response to anthrax bioterrorism.

BACKGROUND: A bioterrorism attack with an agent such as anthrax will require rapid deployment of medical and pharmaceutical supplies to exposed individuals. How should such a logistical system be organized? How much capacity should be built into each element of the bioterrorism response supply chain? METHODS: The authors developed a compartmental model to evaluate the costs and benefits of various strategies for preattack stockpiling and postattack distribution and dispensing of medical and pharmaceutical supplies, as well as the benefits of rapid attack detection. RESULTS: The authors show

Predictors of psychiatric disorders in liver transplantation candidates: logistic regression models.

This study has two goals. The first goal is to assess the prevalence of psychiatric disorders in orthotopic liver transplantation (OLT) candidates by means of standardized procedures because there has been little research concerning psychiatric problems of potential OLT candidates using standardized instruments. The second goal focuses on identifying predictors of these psychiatric disorders. One hundred sixty-five elective OLT candidates were assessed by our unit. Psychiatric diagnoses were based on the Structured Clinical Interview for the Diagnostic and Statistical Manual of Mental Disorder

Logistic regression models of cytokines in differentiating vitreoretinal lymphoma from uveitis.

BACKGROUND: Vitreoretinal lymphoma (VRL) can commonly masquerade as chronic idiopathic uveitis due to its nonspecific clinical presentation. Thus, its early diagnosis is difficult. In this study, new logistic regression models were used to classify VRL and uveitis. Additionally, the diagnostic performance of interleukin (IL)-10, the IL-10/IL-6, and the Interleukin Score for IntraOcular Lymphoma Diagnosis (ISOLD) are evaluated. METHODS: Sixty-nine aqueous humors (AH) (46 VRL, 23 uveitis) and 65 vitreous humors (VH) (49 VRL, 16 uveitis) were collected from a single-center retrospective cohort.

Sufficient Sample Size and Power in Multilevel Ordinal Logistic Regression Models.

For most of the time, biomedical researchers have been dealing with ordinal outcome variable in multilevel models where patients are nested in doctors. We can justifiably apply multilevel cumulative logit model, where the outcome variable represents the mild, severe, and extremely severe intensity of diseases like malaria and typhoid in the form of ordered categories. Based on our simulation conditions, Maximum Likelihood (ML) method is better than Penalized Quasilikelihood (PQL) method in three-category ordinal outcome variable. PQL method, however, performs equally well as ML method where fi

Pharmacodynamic models: parameterizing the hill equation, Michaelis-Menten, the logistic curve, and relationships among these models.

The Hill equation is often used in dose-response or exposure-response modeling. Aliases for the Hill model include the Emax model, and the Michaelis-Menten model. There is confusion about the appropriate parameterization, how to interpret the parameters, what the meaning is of the various parameterizations found in the literature, and which parameterization best approximates the statistical inferences produced when fitting the Hill equation to data. In this paper, we present several equivalent versions of the Hill model; show that they are equivalent in terms of yielding the same prediction fo

Unlocking new insights into the somatic marker hypothesis with multilevel logistic models.

The Somatic Marker Hypothesis, an influential neurobiological account of decision-making, states that emotional somatic markers (e.g., skin conductance responses) influence decision-making processes. Despite its prominence, the hypothesis remains controversial partly because of inconsistent results stemming from inappropriate statistical methods. Tasks designed to assess decision-making often use repeated measures designs, such as the Iowa Gambling Task (IGT), which requires participants to maximize profits by selecting 100 cards among four decks offering varying win-loss contingencies. Are machine learning models superior to logistic regression models to predict 30-day mortality post-hip fracture surgery?

Hip fractures represent a significant global health burden, with high mortality rates. Accurate prediction of 30-d postoperative mortality is instrumental to optimize patient care. This study aimed to validate previously developed logistic regression and machine learning (ML) models for predicting 30-d mortality after hip fracture surgery measured from the date of the index operation (counting deaths occurring in-hospital or within 30 d after surgery) and compare their performance against one established calculator. We included 47 276 patients aged ≥65 yr who underwent hip fracture surgery, ex

Construction and comparison of preeclampsia prediction models using logistic regression and machine learning.

OBJECTIVE: Preeclampsia is a major cause of maternal and perinatal morbidity and mortality. Early identification of women at high risk is essential for timely prevention and improved outcomes. METHODS: In this study, clinical data as well as ultrasound examination results from 404 pregnant women, among whom 45 suffered and 359 did not suffer from preeclampsia, were retrospectively analyzed. Independent predictors were found via multivariable logistic regression. With these predictors, four classification models involving logistic regression, support vector machine, random forests, and extreme

Stability and interpretability of penalized logistic regression models for breast cancer risk prediction.

Penalized logistic regression is widely used in biomedical classification to address multicollinearity and improve predictive performance, yet the stability and reproducibility of selected predictors are often overlooked. This study evaluates feature stability and interpretability in ridge, lasso, and elastic-net logistic regression for breast cancer diagnosis using the Wisconsin Diagnostic Breast Cancer dataset. Models were trained with cross-validated tuning and evaluated on an independent test set using discrimination, classification, and calibration metrics. Feature stability was quantifie

Malaria attributable fractions with changing transmission intensity: Bayesian latent class vs logistic models.

BACKGROUND: Asymptomatic carriage of malaria parasites is common in high transmission intensity areas and confounds clinical case definitions for research studies. This is important for investigations that aim to identify immune correlates of protection from clinical malaria. The proportion of fevers attributable to malaria parasites is widely used to define different thresholds of parasite density associated with febrile episodes. The varying intensity of malaria transmission was investigated to check whether it had a significant impact on the parasite density thresholds. The same dataset was

A general framework for the use of logistic regression models in meta-analysis.

Where individual participant data are available for every randomised trial in a meta-analysis of dichotomous event outcomes, "one-stage" random-effects logistic regression models have been proposed as a way to analyse these data. Such models can also be used even when individual participant data are not available and we have only summary contingency table data. One benefit of this one-stage regression model over conventional meta-analysis methods is that it maximises the correct binomial likelihood for the data and so does not require the common assumption that effect estimates are normally di

How to develop, validate, and update clinical prediction models using multinomial logistic regression.

OBJECTIVES: Multicategory prediction models (MPMs) can be used in health care when the primary outcome of interest has more than two categories. The application of MPMs is scarce, possibly due to added methodological complexities compared to binary outcome models. We provide a guide of how to develop, validate, and update clinical prediction models based on multinomial logistic regression. STUDY DESIGN AND SETTING: We present guidance and recommendations based on recent methodological literature, illustrated by a previously developed and validated MPM for treatment outcomes in rheumatoid arth

Biological behavior of CIN lesions is predictable by multiple parameter logistic regression models.

OBJECTIVES: Progression and regression of premalignant cervical lesions cannot be predicted using conventional cytomorphological or histomorphological parameters. However, markers such as human papillomavirus (HPV) or makers indicating proliferation, genetic instability and chromosomal aberration may be of predictive value assessing short-term biological behavior of cervical intraepithelial neoplasia. In this paper, we have studied the usage of logistic regression models with Ki-67 labeling index (LI), chromosome index for chromosome 1 (CI#1) and aneusomy for chromosome 1 in cervical smears to

Identification of high risk groups for breast cancer by means of logistic models.

In order to identify high risk groups for breast cancer, unconditional multiple logistic regression models based on 5 widely recognized and easily identifiable risk factors (age at menarche, at menopause and at first birth, family history of breast cancer and body mass index) were applied to a large dataset including 2085 cases and 1936 controls aged 50 or over derived from two unmatched hospital-based case-control studies conducted in Italy. Although various models provided an excellent fitting, both on the whole dataset and using a training-testing approach to a priori identified separate su
