The neonatal nurse practitioner.

Neonatal nurse practitioners have been introduced into tertiary level neonatal intensive care units in the United States, Canada, and the United Kingdom in response to the increased survival rates of extremely low birth weight infants, the short-age of physicians, and the nursing profession's emphasis on development of advanced nursing practice roles. This brief review summarizes the recent research related to the facilitating and constraining factors that influence the role; the safety, effectiveness, and cost of the role; the impact of this role on other members of the health care team; and

Nursing diagnosis and obstetric, gynecologic, and neonatal nursing: breastfeeding as an example.

Nursing diagnosis is steadily gaining acceptance as the tool to use to communicate nursing's contribution to health care. Historically, however, obstetric, gynecologic, and neonatal (OGN) nurses have had difficulty applying the primarily problem-based diagnoses to a primarily well population. Two nursing diagnoses addressing breastfeeding have been accepted by the North American Nursing Diagnosis Association. The development and use of these diagnoses are discussed in this article. In addition, OGN nurses are challenged to develop other diagnoses that work within the OGN specialties.

Nursing diagnosis neonatal hyperbilirubinemia: A survival analysis.

PURPOSE: To analyze the short-term survival of the nursing diagnosis of neonatal hyperbilirubinemia in hospitalized neonates within the first 24 h of life and over a maximum of 7 days. METHODS: A prospective open cohort study with a longitudinal design was developed with 120 newborns during the first 24 h of life. The survival rate after a nursing diagnosis of neonatal hyperbilirubinemia was analyzed for 7 days. FINDINGS: The number of new cases of neonatal hyperbilirubinemia during the follow-up was 82 (RR: 90.1%, daily incidence rate: 34.17%). The greatest manifestation of the diagnosis oc

Neonatal integrated home care: nursing without walls.

A Neonatal Integrated Home Care Program was developed to cross-train NICU staff nurses to provide follow-up care for high-risk neonates in the home. Implementation required collaboration of the NICU and the Center for Home Care and Hospice. Initial target populations were premature infants in transition to oral feeds and oxygen-dependent neonates. Staff, parent, and insurer enthusiasm for this innovative program resulted in expansion to serve infants and families with many other care needs. Continuity of care provides an invaluable resource for families at home with their high-risk infants. Ou

[Proposal for difficult nursing in neonatal Hirschsprung's disease].

Often the Hirschsprung's disease presents severe neonatal manifestations: enterocolitis, intestinal occlusion. The authors experienced a conservative treatment in 3 patients with Hirschsprung's disease using elemental diets. This behaviour avoided the colostomy, otherwise necessary with a normal nursing.

Legal and ethical issues in neonatal nursing: a case study.

Neonatal nurses regularly face complex legal and ethical dilemmas. This article discusses the hypothetical case of Jack, a two-day-old infant diagnosed with trisomy 13 (syndrome), a life-limiting condition. Jack's prognosis is poor, and he is not expected to live past two weeks of age. The legal and ethical perspectives of withholding life-sustaining treatment in infants and children will be explored through the application of ethical frameworks, as well as statute and case law relevant to children and adolescent nursing. The article also discusses the neonatal nurse's role, with reference to
