Pneumococcal Vaccines.

Streptococcus pneumoniae is a Gram-Positive pathogen that is a major causative agent of pneumonia, otitis media, sepsis and meningitis across the world. The World Health Organization estimates that globally over 500,000 children are killed each year by this pathogen. Vaccines offer the best protection against S. pneumoniae infections. The current polysaccharide conjugate vaccines have been very effective in reducing rates of invasive pneumococcal disease caused by vaccine type strains. However, the effectiveness of these vaccines have been somewhat diminished by the increasing numbers of cases

Pneumococcal vaccines.

Streptococcus pneumoniae continues to take a heavy toll on childhood mortality and morbidity across the developing world. An estimated 10.6 million invasive pneumococcal diseases (IPDs) occur every year, with nearly 1 million deaths in children under 5 years of age. Introduction of vaccines in the childhood immunisation programme in developed world has brought down the incidence of the disease considerably. However, childhood immunocompromising illnesses including HIV have increased the risk of IPD several folds. There is also a growing concern on the increasing antibiotic resistance among the

Pneumococcal conjugate vaccines. A review.

The pneumococcus is currently the most common cause of vaccine-preventable death in children aged less than 5 years, and in 2002 was responsible for 716,000 deaths worldwide. Treatment with antibiotics was the main approach to contain the pneumococcus, however, even with effective antibiotics, pneumococcal meningitis has a poor prognosis and with the emergence of drug resistant pneumococcal disease the need for prevention by vaccine was evident. The first born pneumococcal vaccines were polysaccharide vaccines that unfortunately did not protect young children. The most vulnerable age group of

Evaluating immune responses to pneumococcal vaccines.

Streptococcus pneumoniae (pneumococcus) is a significant cause of bacterial infections ranging from mild infections affecting the respiratory tract such as otitis media and sinusitis to severe diseases including bacteremia, pneumonia, and invasive pneumococcal disease (IPD) (eg, meningitis, septic arthritis, and endocarditis). Pneumococcal vaccines were first developed in the 1970s as capsular pneumococcal polysaccharide vaccines, which were T-cell independent and hence lacked immunologic memory. Subsequently in the year 2000, pneumococcal conjugate vaccines (PCV) conjugated to a protein to in

Should Pneumococcal Vaccines Eliminate Nasopharyngeal Colonization?

Streptococcus pneumoniae remains an important human pathogen. For more than 100 years, there have been vaccine efforts to prevent pneumococcal infection. The pneumococcal conjugate vaccines have significantly reduced invasive disease. However, these vaccines have changed pneumococcal ecology within the human nasopharynx. We suggest that elimination of the pneumococcus from the human nasopharynx can have consequences that should be considered as the next generation of pneumococcal vaccines is developed.

Pneumococcal conjugate vaccines--a European perspective.

Streptococcus pneumoniae is a leading cause of bacterial pneumonia, meningitis, and acute otitis media in children and adults worldwide. In the age group of < 2 years the incidence of invasive pneumococcal disease ranges from approximately 14 cases per 100,000 in Germany and the Netherlands and more than 90 per 100,000 children in Spain. The vulnerability of children to S. pneumoniae can also be demonstrated by the high rate of sequelae (> 20% in Germany) and the high mortality (7.5%) in pneumococcal meningitis. Furthermore, antibiotic resistance of S. pneumoniae is increasing in Europe, parti

Pneumococcal Serotype Distribution and Coverage of Existing and Pipeline Pneumococcal Vaccines.

BACKGROUND: Next-generation pneumococcal conjugate vaccines (PCVs) target an expanding array of serotype antigens. We assessed the proportions of invasive pneumococcal disease (IPD) and pneumococcal acute respiratory infections (ARIs) caused by serotypes targeted by existing and pipeline PCVs, and the annual United States pneumococcal disease burdens potentially preventable by these products. METHODS: We estimated serotype distribution and proportions of pneumococcal ARIs (acute otitis media [AOM; children only], sinusitis, nonbacteremic pneumonia) and IPD attributable to serotypes targeted b

PNEUMOCOCCAL SEROTYPE DISTRIBUTION AND COVERAGE OF EXISTING AND PIPELINE PNEUMOCOCCAL VACCINES.

BACKGROUND: Streptococcus pneumoniae (pneumococcus) causes invasive pneumococcal disease (IPD) and non-invasive acute respiratory infections (ARIs). Three pneumococcal conjugate vaccines (PCVs) are recommended in the United States with additional products in clinical trials. We aimed to estimate 1) proportions of IPD cases and pneumococcal ARIs caused by serotypes targeted by existing and pipeline PCVs and 2) annual U.S. pneumococcal burdens potentially preventable by PCVs. METHODS: We estimated serotype distribution and proportions of non-invasive pneumococcal ARIs (AOM [children only], sinu

Pneumococcal Vaccine.

Pneumococci are the most common bacterial cause of childhood pneumonia, especially in children aged younger than 5 years. In adults, pneumococci account for 10% to 30% of adult community-acquired pneumonia. The conjugate pneumococcal vaccines have been shown to have a remarkable effect, reducing the incidence of all types of pneumococcal diseases. Overtime the vaccines have been updated to include protection against more serotypes. Schedules for administration have also changed based on age and pre-existing conditions. Development and implementation of pneumococcal vaccines and racial disparit

Pneumococcal vaccine.

Pneumococcal infection is responsible for a wide range of diseases, including upper respiratory tract infections such as otitis media and sinusitis, pneumonia and other lower respiratory infections, and the disseminated infections of bacteremia and meningitis. Invasive pneumococcal disease and pneumonia, which represent the threat of serious morbidity and mortality, occur most frequently in certain age groups, selected ethnic populations, and individuals with chronic medical diseases or immunosuppression. There are two pneumococcal vaccines currently available, Pneumovax 23 and Pnu-Immune 23.

Streptococcus pneumoniae or pneumococcus is a major pathogen causing meningitis, pneumonia, other invasive infections, and the common infections acute otitis media and sinusitis. The major virulence factor is the polysaccharide capsule, present as one of approximately 90 serotypes. Anticapsular antibodies protect against infection. In 1977 and 1997, vaccines composed of purified capsular polysaccharide from 14- and 23-capsular types, respectively, were licensed for use in those children 2 years of age or older who are at increased risk for invasive pneumococcal infection. These vaccines have l

Pneumococcal vaccines for prevention of adult pneumonia.

Streptococcus pneumoniae, pneumococcus, is one of most important bacterial pathogens in adult community-acquired pneumonia. Although it can cause a variety of illness including invasive diseases (IPD), pneumonia has a greater impact than IPD from the perspective of health economics. 23 valent pneumococcal polysaccharide vaccine (PPSV23) and 13 valent pneumococcal conjugate vaccine (PCV13) have been recommended for people ≥65 years old until recently based on evidence in preventing IPD and pneumonia. Because the introduction and dissemination of PCVs for infants and its effects on herd immunity

Pneumococcal Vaccines - How Many Serotypes are Enough?

Streptococcus pneumoniae causes meningitis, pneumonia, septicemia, arthritis, sinusitis and otitis media specially in children and over 65 y age groups. It contributes significantly to under-five mortality and morbidity worldwide as well as in India. Use of pneumococcal vaccine seems to be the most effective measure to decrease the disease burden and reduction of under-five mortality. Many countries have already included Pneumococcal Conjugate Vaccines (PCV) in their National Immunization Programmes (NIP). Government of India has announced recently to include PCV13 in NIP in a phased manner. S

Recent Topics of Pneumococcal Vaccination: Indication of Pneumococcal Vaccine for Individuals at a Risk of Pneumococcal Disease in Adults.

Pneumococcal disease is one of the most common and severe vaccine-preventable diseases (VPDs). Despite the advances in antimicrobial treatment, pneumococcal disease still remains a global burden and exhibits a high mortality rate among people of all ages worldwide. The immunization program of the pneumococcal conjugate vaccine (PCV) in children has decreased pneumococcal disease incidence in several countries. However, there are several problems regarding the pneumococcal vaccine, such as indications for immunocompetent persons with underlying medical conditions with a risk of pneumococcal dis

Review: current and new generation pneumococcal vaccines.

Pneumococcal polysaccharide vaccines (PPVs) and conjugate vaccines (PCVs), of which PPV23 and PCV13 are the current front runners, have had a significant, beneficial impact on public health. With regard to PPV23, there has been some debate, however, about its protective efficacy against all-cause pneumonia, as opposed to invasive pneumococcal disease, in high-risk cases. PCVs, on the other hand, have been included in many national immunisation programmes for prevention of severe pneumococcal disease in infants and young children, as well as for adults in various high-risk categories. Although

Pneumococcal vaccines: an update on current strategies.

Streptococcus pneumoniae is a major cause of morbidity and mortality in infants, children and the elderly. Despite the availability of excellent antimicrobial therapy and adequate health care systems, respiratory diseases and invasive infections caused by pneumococci still comprise a major health problem. The emerging resistance to penicillin and other commonly used antibiotics underscores the importance of the development of novel vaccine strategies to combat pneumococcal disease. Although the 23-valent polysaccharide (PS) vaccine is immunogenic and protective in most adults and children over
