The impact of delayed source control and antimicrobial therapy in 196 patients with cholecystitis-associated septic shock: a cohort analysis.

BACKGROUND: Cholecystitis-associated septic shock carries a significant mortality. Our aim was to determine whether timing of source control affects survival in cholecystitis patients with septic shock. METHODS: We conducted a nested cohort study of all patients with cholecystitis-associated septic shock from an international, multicentre database (1996–2015). Multivariable logistic regression was performed to determine associations between clinical factors and in-hospital mortality. The results were used to inform a classification and regression tree (CART) analysis that modelled the associa

A differentiated approach to repeat small-bowel anastomoses in patients with postoperative peritonitis: a prospective cohort study.

BACKGROUND: Postoperative peritonitis still remains the cause of a high mortality rate in emergency abdominal surgery. Here we aimed to evaluate the efficacy of different surgical strategies for small-bowel perforations that resulted in postoperative peritonitis. METHODS: Surgical management results for 140 patients with postoperative peritonitis due to small-bowel perforations, necrosis and anastomotic leakage were comparatively analyzed. Using the APACHE-II and MPI scoring systems, different surgeon attitudes were examined in three patient groups (primary anastomosis, delayed anastomosis, a

Parsimonious Logistic Regression Models for 90-Day Mortality Prediction in the Intensive Care Unit.

Introduction Early identification of critically ill patients is essential in intensive care units to guide triage and allocate resources. To support these decisions, prognostic scoring systems are commonly used to estimate illness severity and predict outcomes. The Acute Physiology and Chronic Health Evaluation II (APACHE II) and the Sequential Organ Failure Assessment (SOFA) are among the most widely used. While newer APACHE versions (III and IV) offer enhanced predictive power, they require extensive variables, are computationally expensive, and often rely on proprietary systems - making the

Comparison between the body mass index and the Controlling Nutritional status to determine the severity in patients with abdominal sepsis.

BACKGROUND: The systemic response of the organism, in defense against the aggressor agent, generates acute catabolic response, which leads to deterioration of the nutritional status. OBJECTIVE: Compare the usefulness of the body mass index (BMI) and the CONUT scale to determine the severity in abdominal sepsis. METHODS: Retrospective, descriptive, cross-sectional study in patients with diagnosis of abdominal sepsis, from April 2016 to February 2017. RESULTS: We included 153 cases (61 female and 92 male); mean age of 47.44 years, the main organ causing abdominal sepsis was the appendix 43%.

Role of Scoring Systems in Prognosticating Outcomes of Patients With Acute Pancreatitis: A Prospective Cohort Study.

INTRODUCTION: Acute pancreatitis (AP) is a common cause of emergency hospital admissions, putting a substantial burden on the healthcare system. The clinical course of AP is usually mild and often resolves without a sequel. Severe AP (SAP) is associated with an intense inflammatory response leading to localized or systemic complications and significant morbidity and mortality (American Gastroenterological Association). Early diagnosis and precise assessment of disease severity are imperative during initial evaluation in patients with AP, as it has a bearing on deciding the course of management

New Insights on Continuous Renal Replacement Therapy for Acute Respiratory Distress Syndrome: A Systematic Review and Meta-Analysis.

BACKGROUND: In recent times, the applications of continuous renal replacement therapy (CRRT) beyond kidney-related conditions have been progressively increasing, and its implementation in randomized controlled trials (RCTs) specifically for acute respiratory distress syndrome (ARDS) has been documented. This meta-analysis compiles all existing RCTs to assess whether CRRT benefits ARDS. METHODS: We searched 12 databases in English and Chinese and two clinical trial centers up to November 28, 2023. The main outcome indicator is the mortality rate. Secondary outcome indicators include incidence

Prognostic accuracy of oxygen debt for mortality in patients undergoing venoarterial extracorporeal membrane oxygenation therapy: a retrospective cohort study.

BACKGROUND: Cardiogenic shock is associated with high mortality. Prognostic scales, such as Sequential Organ Failure Assessment (SOFA), Acute Physiology and Chronic Health Evaluation II (APACHE II), and Survival After Venoarterial ECMO (SAVE), have been used to estimate mortality risk or survival probability. However, their performance remains limited in the context of Venoarterial Extracorporeal Membrane Oxygenation (VA-ECMO) therapy. This study aimed to validate oxygen debt (DEOx) as a predictor of 28-day mortality in critically ill patients receiving VA-ECMO and to compare its prognostic ac

[Early immunoglobulin therapy in high risk patients for infection after heart surgery].

The efficacy of early supplemental intravenous immunoglobulin G (ivIgG) treatment was investigated in 41 patients at risk for sepsis following cardiac surgery (APACHE II score > or = 19 on the 1st postoperative day). The ivIgG preparation (Psomaglobin N) was chosen because of its reported high antibody titers and effectiveness in animal models against gram-positive microorganisms, preponderant as infective agents after heart surgery. The control group consisted of a historical cohort of 42 cardiac surgical patients of comparable pretreatment disease and sepsis severity. Following ivIgG therapy
