Atrioventricular accessory pathways in 89 dogs: Clinical features and outcome after radiofrequency catheter ablation.

BACKGROUND: Atrioventricular accessory pathways (APs) in dogs have been reported rarely. Data regarding clinical presentation and long-term outcome after radiofrequency catheter ablation (RFCA) are limited. HYPOTHESIS/OBJECTIVES: To study clinical features, electrophysiologic characteristics, and outcome of RFCA in dogs with APs. ANIMALS: Eighty-nine dogs presented consecutively for RFCA of APs. METHODS: Case series. RESULTS: Labrador retrievers (47.2% of dogs) and male dogs (67.4% of dogs) were most commonly affected. Labrador retrievers were more likely to be male than non-Labrador breed

Antidromic Atrioventricular Reciprocating Tachycardia Using a Concealed Retrograde Conducting Left Lateral Accessory Pathway.

Atrioventricular reciprocating tachycardia is a common cause of undifferentiated supraventricular tachycardia. In patients with manifest or concealed accessory pathways, it is imperative to assess for the presence of other accessory pathways. Multiple accessory pathways are present in 4% to 10% of patients and are more common in patients with structural heart disease. In rare cases, multiple accessory pathways can act as the anterograde and retrograde limbs of the tachycardia.

Characteristics of antegrade-only accessory pathways in children and adolescents.

There is minimal data regarding antegrade-only accessory pathways in young patients. Given evolving recommendations and treatments, retrospective analysis of the clinical and electrophysiologic properties of antegrade-only pathways in patients <21 years old was performed, with subsequent comparison of electrophysiology properties to age-matched controls with bidirectional pathways. Of 522 consecutive young patients with ventricular pre-excitation referred for electrophysiology study, 33 (6.3%) had antegrade-only accessory pathways. Indications included palpitations (47%), chest pain (25%), and

Catheter ablation of an antidromic atrioventricular reentrant tachycardia using a slow, decrementally conducting accessory pathway in the great cardiac vein.

The inability to ablate left accessory pathways (APs) from endocardial approaches may suggest an epicardial location. We report on a 43-year-old woman presenting with a wide QRS tachycardia with Right Bundle Branch Block (RBBB) morphology, right inferior axis, and the "pattern break" appearance in V2 resembled the outflow tract ventricular tachycardia. An electrophysiology study confirmed an antidromic atrioventricular reentrant tachycardia using an antegrade slow, decrementally conducting AP that was successfully ablated in the great cardiac vein-anterior interventricular vein junction after

Wide QRS complex tachycardia in patients with Chagas' disease: Sustained ventricular tachycardia until proven otherwise.

Algorithms used for the differential diagnosis of wide QRS complex tachycardia are extremely important in clinical practice, but they have limitations and should be applied with caution. We describe a case of a patient with positive serology for Chagas disease and normal echocardiogram who presented an episode of wide QRS complex tachycardia. It was initially diagnosed as sustained ventricular tachycardia using the Brugada and Vereckei algorithms and it was sent to the reference service for the implant of a cardioverter defibrillator. However, the clinical history, electrocardiogram in sinus r

A Case of Lown-Ganong-Levine Syndrome: Due to an Accessory Pathway of James Fibers or Enhanced Atrioventricular Nodal Conduction (EAVNC)?

BACKGROUND Lown-Ganong-Levine syndrome, includes a short PR interval, normal QRS complex, and paroxysmal tachycardia. The pathophysiology of this syndrome includes an accessory pathway connecting the atria and the atrioventricular (AV) node (James fiber), or between the atria and the His bundle (Brechenmacher fiber). Similar features are seen in enhanced atrioventricular nodal conduction (EAVNC), with the underlying pathophysiology due to a fast pathway to the AV node, and with the diagnosis requiring specific electrophysiologic criteria. CASE REPORT A 17-year-old man presented with a history

SUCCESSFUL ABLATION OF CONCEALED ACCESSORY PATHWAY MEDIATED SUPRAVENTRICULAR TACHYCARDIA IN A 5-YEAR-OLD CHILD WITH EBSTEIN ANOMALY.

Here we report a case of a 5-year-old boy with Ebstein anomaly and supraventricular tachycardia. He was diagnosed with Ebstein anomaly at the age of 3 months during the workup for afebrile convulsions, and has been followed by a pediatric cardiologist since. Electrocardiography recorded a small Rr` pattern in V1 on a few occasions, without other abnormalities. At the age of 2.5 years, the first episode of supraventricular tachycardia was recorded and stopped with adenosine. He was started on atenolol and was asymptomatic until a year later when he presented with the second recorded supraventri

Segmental septal dyskinesia associated with an accessory pathway and preexcitation in two Golden Retriever dogs.

Ventricular preexcitation secondary to anterograde conduction through an accessory pathway was diagnosed in two Golden Retriever dogs. Both dogs demonstrated similar segmental myocardial thinning and systolic dyskinesia of the basal interventricular wall on echocardiography. These changes are widely recognised in people with ventricular preexcitation but have not been previously described in dogs. Ventricular preexcitation should be considered as a potential cause for segmental wall motion abnormalities in these two dogs.

Results of catheter ablation with zero or near zero fluoroscopy in pediatric patients with supraventricular tachyarrhythmias.

INTRODUCTION AND OBJECTIVES: Ionizing radiation exposure in catheter ablation procedures carries health risks, especially in pediatric patients. Our aim was to compare the safety and efficacy of catheter ablation guided by a nonfluoroscopic intracardiac navigation system (NFINS) with those of an exclusively fluoroscopy-guided approach in pediatric patients. METHODS: We analyzed catheter ablation results in pediatric patients with high-risk accessory pathways or supraventricular tachycardia referred to our center during a 6-year period. We compared fluoroscopy-guided procedures (group A) with

Ablation of manifest septal accessory pathways: a single-center experience.

BACKGROUND: Ablation of septal accessory pathways (SAPs) is associated with an increased risk of heart block. Data on outcomes of SAP ablation in adults are limited. OBJECTIVES: To describe outcomes of SAP ablation in our center. METHODS: Patients with Wolff-Parkinson-White syndrome (WPW) undergoing an EP study at our center between January 2008 and August 2019 were identified from our institutional database. Location of the pathway was noted as anteroseptal (AS), midseptal (MS), or posteroseptal (PS). Outcomes of the ablation including success, complication rates, and recurrences were also

Outcomes of ablation in Wolff-Parkinson-White-syndrome: Data from the German Ablation Registry.

AIMS: Catheter ablation is recommended for symptomatic WPW-syndrome. Commonly perceived low recurrence rates were challenged recently. We sought to identify patient strata at increased risk. METHOD: Of 12,566 patients enrolled at 52 German Ablation Registry sites from 2007 to 2010, 789 were treated for WPW-syndrome. Patients were included for symptomatic palpitations and tachycardia documentation. Follow-up duration was one year. Overall complications were defined as serious, access-related, and ablation-related. We adjudicated WPW-recurrence for re-ablation during follow-up. Risk strata incl

Successful cryoablation of Mahaim tachycardia in a child with Ebstein's anomaly.

Mahaim fibers with decremental atrioventricular (AV) node-like conduction properties comprise less than 3 % of accessory pathways. Radiofrequency ablation of right atriofascicular pathways guided by a distinct Mahaim potential detected at the anterolateral to posterolateral tricuspid annulus or in the right ventricular free wall is a safe and highly effective treatment method. The case report presents a 16-year-old boy with Ebstein's anomaly and symptomatic wide complex tachyarrhythmia. The electrophysiologic study and the entire ablation procedure were performed using a three-dimensional mapp

Antidromic and orthodromic reciprocating tachycardias over a novel left-sided accessory pathway involving the vein of Marshall and coronary sinus musculature.

INTRODUCTION: Herein, we present a rare case of the successful ablation of an accessory pathway (AP) involving the Marshall Bundle (MB) and coronary sinus musculature (CSM) in a 40-year-old man with Wolff-Parkinson-White syndrome. METHODS AND RESULTS: An orthodromic reciprocating tachycardia (ORT) was inducible with the earliest atrial activation site located at the posterolateral mitral annulus. The local conduction and the cycle length of ORT was prolonged by peri-mitral ablation; however, it failed to block the AP. The atrial insertion of the AP was identified by remapping during ORT at th

Clinical and experimental evidence of supernormal excitability and conduction.

True supernormality of excitability and conduction has been demonstrated in normal Purkinje fibers in in vitro studies. In the clinical setting, supernormality of conduction is manifested better than expected. This phenomenon is much more common than previously thought, particularly in the presence of certain clinical conditions. If a careful scanning of the cardiac cycle is performed on all patients with intermittent bundle branch block and second degree or advanced infranodal AV block, accessory pathways and mulfunctioning pacemakers, it is anticipated that a much larger amount of supernorma

Efficacy and safety of cryoablation of parahissian accessory pathways in children: a single institution study.

BACKGROUND: Catheter cryoablation of supraventricular tachycardias involving the perinodal regions is considered to be a safer alternative compared to radiofrequency ablation. Limited information is available for efficacy, midterm outcomes, and complications regarding the ablation of parahissian accessory pathways (APs) in pediatric patients. METHODS: A retrospective review of all pediatric patients who underwent cryoablation for treatment of a parahissian AP was performed. RESULTS: Twenty-five patients (median age 13 years and weight 45.6 kg) underwent cryoablation of a parahissian AP. Medi

Differential sequential septal pacing: a simple maneuver to differentiate nodal versus extranodal ventriculoatrial conduction.

BACKGROUND: Distinguishing retrograde nodal conduction from extranodal conduction using an accessory pathway (AP) can sometimes be challenging. OBJECTIVE: To distinguish nodal from extranodal ventriculoatrial (VA) conduction regardless of AP location by proposing a simple method. This method is based on the principle that moving the pacing site progressively from the basal region toward the entrance of the His-Purkinje system should shorten VA time for nodal but not for AP conduction. METHODS: Sixty-seven patients with supraventricular tachycardia were prospectively recruited. Quadripolar ca

Wide QRS tachycardia associated with multiple accessory pathways in a patient with Wolff-Parkinson-White syndrome.

The electrocardiogram of a 14-year-old boy with recurrent palpitation showed a wide QRS regular tachycardia with a right bundle branch block and right-axis deviation of 226 beats per minute. Verapamil infusion terminated the tachycardia after a few minutes. Electrophysiological study revealed that this tachycardia was considered as a reentrant tachycardia associated with the anterograde left posterior accessory pathway (AP) and retrograde right septal AP. Radiofrequency application was performed and eliminated both APs, and there was no recurrence of wide QRS tachycardia. <Learning objective:
