Advanced maternal age-related clustering of metabolic abnormalities is associated with risks of adverse pregnancy outcomes.

AIMS: Advanced maternal age (AMA) is correlated with higher risk of adverse pregnancy outcomes while the pathophysiology remains unclear. Our study aimed to investigate whether AMA is linked to the clustering of metabolic abnormalities, which in turn is associated with an increased risk of adverse pregnancy outcomes. METHOD: A total of 857 pregnant woman were recruited in a prospective cohort at National Taiwan University Hospital, from November 2013 to April 2018. Metabolic abnormalities during pregnancy were defined as following: fasting plasma glucose ≥92 mg/dl, body mass index (BMI) ≥24 k

Advanced Maternal Age Differentially Affects Embryonic Tissues with the Most Severe Impact on the Developing Brain.

Advanced maternal age (AMA) poses the single greatest risk to a successful pregnancy. Apart from the impact of AMA on oocyte fitness, aged female mice often display defects in normal placentation. Placental defects in turn are tightly correlated with brain and cardiovascular abnormalities. It therefore follows that placenta, brain and heart development may be particularly susceptible to the impact of AMA. In the current study, we compared global transcriptomes of placentas, brains, hearts, and facial prominences from mid-gestation mouse conceptuses developed in young control (7-13 wks) and agi

Advanced maternal age causes premature placental senescence and malformation via dysregulated α-Klotho expression in trophoblasts.

Advanced maternal age (AMA) pregnancy is associated with higher risks of adverse perinatal outcomes, which may result from premature senescence of the placenta. α-Klotho is a well-known antiaging protein; however, its expression and effect on the placenta in AMA pregnancies have not yet been fully elucidated. The expression patterns of α-Klotho in mouse and human placentas from AMA pregnancies were determined by Western blotting and immunohistochemistry (IHC) staining. α-Klotho expression in JAR cells was manipulated to investigate its role in trophoblastic senescence, and transwell assays wer

Enhancing intrauterine insemination success in advanced maternal age: Impact of consecutive ejaculate and optimised cycle parameters.

OBJECTIVE: This study evaluated whether consecutive ejaculate (CE) strategies improve intrauterine insemination (IUI) live birth rates (LBR) in women over 35 with unexplained or male-factor infertility. It also examined the influence of follicle number and sperm count thresholds on outcomes. METHODS: In this retrospective cohort study (2010-2019), 596 IUI cycles were analysed in 263 nulliparous women-230 with CE and 366 with standard IUI. Among them, 98 patients underwent CE IUI and 165 received non-CE IUI. Patients with total motile sperm count (TMSC) <5×106 were often fast-tracked to IVF, b

Modeling dynamic velocity-based gestational weight gain trajectories in advanced maternal age: association with perinatal outcomes.

BACKGROUND: Static gestational weight gain (GWG) guidelines may be inadequate for women of advanced maternal age (AMA), a population with distinct physiological characteristics including diminished metabolic flexibility and elevated baseline risks. This study seeks to delineate dynamic GWG trajectories in this cohort to facilitate more precise risk stratification during pregnancy. METHODS: This retrospective cohort study analyzed routinely collected antenatal weight measurements from 3,818 AMA women. Distinct GWG trajectory patterns across the second and third trimesters were identified using

Sex-specific alterations in placental mitochondria, oxidative damage and apoptosis in mice of advanced maternal age.

IN BRIEF: Advanced maternal age (AMA) is associated with adverse pregnancy outcomes, particularly those associated with placental dysfunction. This study showed that in a mouse model of AMA, male but not female fetuses had increased placental apoptosis and lipid peroxidation, as well as increased mitochondrial content, suggesting that the placentas of male fetuses in AMA mothers adapt to be able to deliver sufficient energy to the fetus. ABSTRACT: Although advanced maternal age (AMA) increases the risk of fetal growth restriction (FGR) and stillbirth, the mechanisms leading to the placental d

Expectant Management of Severe Preeclampsia in Advanced Maternal Age With Multiple Risk Factors: A Case Report.

The management and outcome of severe preeclampsia are outlined in this case report of a 42-year-old multiparous woman presenting with multiple risk factors, including maternal obesity, advanced maternal age, gestational diabetes, and a significant interpregnancy interval of over eight years. The patient underwent expectant management for close monitoring of her pregnancy. However, due to the emergence of severe preeclampsia symptoms, the decision was made to deliver the baby at 29 weeks of gestation via classical cesarean section and bilateral tubal ligation. The neonate, weighing 1070 g, was

Relationship between advanced maternal age and decline of endometrial receptivity: a systematic review and meta-analysis.

Female fertility decreases with age. A decline in oocyte quality plays a key role in reproductive problems in older women. Whether advanced maternal age (AMA) is associated with a decline in endometrial receptivity (ER) remains controversial. A systematic review and meta-analysis were conducted to evaluate the relationship between AMA and ER. Eighteen eligible studies were included in this meta-analysis. Of the 18 studies, 17, 8, 14, and 9 studies reported the impact of AMA on clinical pregnancy rate (CPR), implantation rate (IR), miscarriage rate (MR), and live birth rate (LBR), respectively.

Advanced maternal age compromises fetal growth and induces sex-specific changes in placental phenotype in rats.

Advanced maternal age is associated with an increased risk of pregnancy complications. It programmes sex-specific cardiovascular dysfunction in rat offspring, however the intrauterine mechanisms involved remain unknown. This study in the rat assessed the impact of advanced maternal age on placental phenotype in relation to the growth of female and male fetuses. We show that relative to young (3-4 months) dams, advanced maternal age (9.5-10 months) compromises growth of both female and male fetuses but affects the placental phenotype sex-specifically. In placentas from aged versus young dams, t

Placental Dysfunction Underlies Increased Risk of Fetal Growth Restriction and Stillbirth in Advanced Maternal Age Women.

Pregnancies in women of advanced maternal age (AMA) are susceptible to fetal growth restriction (FGR) and stillbirth. We hypothesised that maternal ageing is associated with utero-placental dysfunction, predisposing to adverse fetal outcomes. Women of AMA (≥35 years) and young controls (20-30 years) with uncomplicated pregnancies were studied. Placentas from AMA women exhibited increased syncytial nuclear aggregates and decreased proliferation, and had increased amino acid transporter activity. Chorionic plate and myometrial artery relaxation was increased compared to controls. AMA was associa

Comprehensive embryo analysis of advanced maternal age-related aneuploidies and mosaicism by short comparative genomic hybridization.

The short comparative genomic hybridization (short-CGH) method was used to perform a comprehensive cytogenetic study of isolated blastomeres from advanced maternal age embryos, discarded after fluorescent in situ hybridization (FISH) preimplantation genetic screening (PGS), detecting aneuploidies (38.5% of which corresponded to chromosomes not screened by 9-chromosome FISH), structural aberrations (31.8%), and mosaicism (77.3%). The short-CGH method was subsequently applied in one PGS, achieving a twin pregnancy.
