When is arthrography indicated in Legg-Calvé-Perthes disease?

We evaluated plain radiographs and arthrograms of twenty-one children with active Legg-Calvé-Perthes disease in order to determine the value of the arthrography. We found that when epiphyseal protrusion was shown by the arthrograms it could be seen on the plain radiographs as well in all patients. Also, an acetabulum-head index of less than 90 or an acetabulum-head quotient of less than 92 as determined on plain radiographs correlated well with a diagnosis of protrusion. Thus, an arthrogram is not necessary to make the diagnosis of protrusion in most patients. The arthrogram is more useful in

Diagnosis of popliteal cyst: double-contrast arthrography and sonography.

One hundred consecutive patients who had trauma to the knee were examined by sonography and double-contrast arthrography to evaluate the presence or absence of popliteal cyst. In 58 cases both tests were negative, while in 15 both were positive. Each of those 15 patients had pain and/or a palpable mass behind the knee. In 27 remaining patients the arthrogram demonstrated a popliteal cyst that was not detected on the sonogram. None of the 27 patients complained of pain in the popliteal fossa or had a palpable mass in the area. The sonographic findings, however, showed good correlation with thes

CT Arthrography of the Elbow: What Radiologists Should Know.

Computed tomography (CT) arthrography is a quickly available imaging modality to investigate elbow disorders. Its excellent spatial resolution enables the detection of subtle pathologic changes of intra-articular structures, which makes this technique extremely valuable in a joint with very tiny chondral layers and complex anatomy of articular capsule and ligaments. Radiation exposure has been widely decreased with the novel CT scanners, thereby increasing the indications of this examination. The main applications of CT arthrography of the elbow are the evaluation of capsule, ligaments, and os

Correlation of magnetic resonance arthrography with revision hip arthroscopy.

BACKGROUND: Arthroscopic approaches for the diagnosis and treatment of hip disorders are well established; however, there are limited data regarding revision hip arthroscopy. There have been several studies evaluating the findings of MR arthrography with primary hip arthroscopy, but to our knowledge, no study has evaluated the diagnostic value of MR arthrography before revision hip arthroscopy. QUESTIONS/PURPOSES: We obtained sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of MR arthrography to detect labral lesions, chondral lesions and loose bo

Evaluation of rotator cuff tears with magnetic resonance arthrography.

The size and morphologic features of rotator cuff tears may influence treatment selection and affect final outcomes. Magnetic resonance arthrography allows observation of these features and other intraarticular structures. To assess the utility of magnetic resonance imaging in assessing size and morphologic features, we retrospectively reviewed observations on 41 shoulders in 37 consecutive surgically treated patients (mean age, 63.2 years) who had magnetic resonance imaging followed by magnetic resonance arthrography. The maximum rotator cuff defect size in the anteroposterior direction defin

[Lumbar intraspinal juxtafacet cysts: MR imaging and CT-arthrography].

PURPOSE: To present data on the MR imaging appearance of lumbar intraspinal juxtafacet cysts (JFC) and to assess the importance of additional CT arthrography. MATERIAL AND METHODS: Twenty-eight patients (16 women, 12 men) with a mean age of 64 years (range: 43 - 82), who underwent MR imaging because of radicular pain or spinal claudication, were found to have an intraspinal cyst associated with the facet joint. In 14 patients, additional CT-arthrography was performed to determine whether a communication exists between the cyst and the facet joint and to try to rupture the cyst. RESULTS: In T

MR imaging and MR arthrography of paraglenoid labral cysts.

OBJECTIVE: We investigated the pathophysiology of paraglenoid labral cysts on the basis of MR imaging, MR arthrography, and cyst aspiration. MATERIALS AND METHODS: From 2211 MR imaging examinations, 51 (2.3%) cysts in 46 patients were identified. MR arthrography (n = 5), cystography (n = 1), arthroscopy (n = 17), percutaneous needle aspiration (n = 4), and medical records were also reviewed (n = 46). RESULTS: On MR imaging and arthrography, cysts were best viewed on T2-weighted images. Mean cyst diameter and volume were 2.2 cm and 2.8 cm3, respectively. Fifty-seven percent of cysts were loca

Magnetic resonance arthrography in children with developmental hip dysplasia.

Conventional and gadolinium enhanced magnetic resonance arthrograms were done on 14 hips in 10 children ages 7 to 24 months. The contralateral normal hips in those with unilateral disease were studied with unenhanced magnetic resonance imaging for comparison. By conventional arthrography, there were no well visualized structures. Visualized structures seen as filling defects were the labrum, ligamentum teres, and transverse acetabular ligament. By magnetic resonance arthrography, well visualized structures were the labrum, ligamentum teres, transverse acetabular ligament, and pulvinar. By unen

CT arthrography and arthroscopy in chronic glenohumeral joint instability.

The authors evaluate the value of CT arthrography in glenohumeral joint instability. A study was conducted on a group of 16 patients with recurrent dislocation of the shoulder. All patients underwent CT arthrography and arthroscopy. The diagnostic accuracy of CT arthrography was rated for different types of lesions on the basis of arthroscopic confirmation of its findings. Our results, which include a statistical analysis, showed a diagnostic accuracy of 97.3% in Bankart lesions and 100% in Hill-Sachs lesions and loose bodies. Dilation of the subscapular bursa and injury of the glenoid labrum-

Double-contrast computed tomographic arthrography of the shoulder joint.

Dislocation of the shoulder joint can cause post-traumatic injury to certain anatomical structures, which may lead to recurrent dislocation. The operative visualization of some of these abnormalities may be difficult and their identification preoperatively allows selection of the appropriate surgical approach and avoids the necessity for lengthy surgery. Fifty-one patients with a history of recurrent dislocation, single dislocation or suspected recurrent subluxation as a cause of pain were examined over a 3-year period by double-contrast computed tomographic (CT) arthrography. Thirty-five pati

Flouroscopic arthrography versus MR arthrography of the lesser metatarsophalangeal joints for the detection of tears of the plantar plate and joint capsule: a prospective comparative study.

BACKGROUND: Derangements of the plantar plate and joint capsule are an underrecognized cause of lesser metatarsalgia. Fluoroscopic arthrography and magnetic resonance (MR) arthrography are both used for diagnosis. Currently there are no studies comparing the effectiveness of these two modalities. METHODS: Patients suspected of having plantar plate or capsular tears underwent both fluoroscopic arthrography and MR arthrography; the imaging findings were then compared and correlated with intraoperative findings, when available, to evaluate the effectiveness of the different imaging modalities. Comparison Between Computed Tomography Arthrography (CTA) Without Traction, CTA With Traction, and Magnetic Resonance Arthrography in Hip Impingement: What Is the Best Method?

PURPOSE: Femoro-acetabular impingement (FAI) is a possible mechanism for the development of early osteoarthritis. FAI occurs when there is a conflict between the rim of the acetabulum and the femoral neck, usually due to irregularities of the proximal femur or acetabulum. Our aims were to compare the diagnostic efficacy of computed tomography arthrography (CTA) without traction, CTA with traction, and magnetic resonance arthrography (MRA) for the detection and grading of chondral and labral lesions in patients with a clinically positive impingement test and to determine the correlation between

[MR and CT arthrography in cartilage imaging : Indications and implementation].

BACKGROUND: The imaging of chondral pathologies is an essential part in the work-up of acute and chronic joint diseases. Besides conventional MR imaging, CT and MR arthrography are well-established methods in evaluating articular cartilage. The application of these techniques requires knowledge of indications and safe injection procedures by the performing radiologist. PURPOSE: Our goal is to describe the techniques of cross-sectional arthrographies of different joints, give an overview of general and joint-specific considerations for practical application as well as provide typical indicatio

Multidetector CT arthrography of the wrist joint: how to do it.

With its exquisite spatial resolution, multidetector computed tomographic (CT) arthrography of the wrist is a valuable tool for the diagnosis and evaluation of a wide spectrum of articular disorders. Traumatic tears of the interosseous ligaments can be classified as complete or incomplete and as partial- or full-thickness tears at multidetector CT arthrography and can also be differentiated from asymptomatic degenerative lesions. In addition, tears of the triangular fibrocartilage complex can be differentiated according to their location. A tailored contrast material injection technique and mu

Evaluation of chronic wrist pain by arthrography, arthroscopy, and arthrotomy.

Arthrography and arthroscopy of the wrist were compared as diagnostic tools in the evaluation of chronic wrist pain. Arthroscopy proved to be a more sensitive procedure than arthrography in 20 consecutive patients with confirmed wrist injuries. There was 86% correlation (the exception--one false negative arthroscopic procedure) between arthroscopy and arthrography in eight patients with triangular fibrocartilage tears. Compared with arthroscopy, there were five false negative and five false positive arthrograms when evaluating interosseous scapholunate and lunotriquetral ligament tears. Arthro
