The SoftHand Pro platform: a flexible prosthesis with a user-centered approach.

BACKGROUND: Among commercially-available upper-limb prostheses, the two most often used solutions are simple hook-style grippers and poly-articulated hands, which present a higher number of articulations and show a closer resemblance to biological limbs. In their majority, the former type of prostheses is body-powered, while the second type is controlled by myoelectric signals. Body-powered grippers are easy to control and allow a simple form of force feedback, frequently appreciated by users. However, they present limited versatility. Poly-articulated hands afford a wide range of grasp and ma

Experimental Investigation of Vibration Analysis on Implant Stability for a Novel Implant Design.

Osseointegrated prostheses are widely used following transfemoral amputation. However, this technique requires sufficient implant stability before and during the rehabilitation period to mitigate the risk of implant breakage and loosening. Hence, reliable assessment methods for the osseointegration process are essential to ensure initial and long-term implant stability. This paper researches the feasibility of a vibration analysis technique for the osseointegration (OI) process by investigating the change in the dynamic response of the residual femur with a novel implant design during a simula

Evaluation of an articulated passive ankle-foot prosthesis.

BACKGROUND: Current ankle prostheses for people with unilateral transtibial amputation (TTA) or transfemoral amputation (TFA) are unable to mimic able-bodied performance during daily activities. A new mechanical ankle-foot prosthesis was developed to further optimise the gait of people with a lower-limb amputation. This study aimed to evaluate the Talaris Demonstrator (TD) during daily activities by means of performance-related, physiological and subjective outcome measures. MATERIALS AND METHODS: Forty-two participants completed a protocol assessing performance and functional mobility with t

Color-matching techniques for creating esthetic hand and finger prostheses.

BACKGROUND: Color matching is a critically important part of, and by far, also the most challenging aspect of producing esthetic silicone prosthesis. There is a knowledge gap in the literature and a lack of opportunities for training in the expertise, particularly regarding color-matching techniques. OBJECTIVE: This article details a color-matching technique that is able to achieve life-like coloration in esthetic prosthesis. TECHNIQUE: Each prosthesis is molded in an outer and inner layers of silicone tinted to different shades and opacities incorporating an intermediate touch-up layer to r

A soft neuroprosthetic hand providing simultaneous myoelectric control and tactile feedback.

Neuroprosthetic hands are typically heavy (over 400 g) and expensive (more than US$10,000), and lack the compliance and tactile feedback of human hands. Here, we report the design, fabrication and performance of a soft, low-cost and lightweight (292 g) neuroprosthetic hand that provides simultaneous myoelectric control and tactile feedback. The neuroprosthesis has six active degrees of freedom under pneumatic actuation, can be controlled through the input from four electromyography sensors that measure surface signals from residual forearm muscles, and integrates five elastomeric capacitive se

Sensory feedback for limb prostheses in amputees.

Commercial prosthetic devices currently do not provide natural sensory information on the interaction with objects or movements. The subsequent disadvantages include unphysiological walking with a prosthetic leg and difficulty in controlling the force exerted with a prosthetic hand, thus creating health issues. Restoring natural sensory feedback from the prosthesis to amputees is an unmet clinical need. An optimal device should be able to elicit natural sensations of touch or proprioception, by delivering the complex signals to the nervous system that would be produced by skin, muscles and joi

Non-amputated limb muscle coordination of unilateral transfemoral amputees.

Unilateral transfemoral amputees rely heavily on non-amputated limb muscles to regulate the prosthetic gait. In this study, we compared the non-amputated limb muscle coordination of eight unilateral transfemoral amputees to eight able-bodied controls. Inverse dynamics approach was conducted via a musculoskeletal model to obtain lower limb joint moments and muscle forces. In addition to the muscle forces at the instants of peak joint moments and the maximum muscle forces, the peak joint moments of the lower limbs were also investigated. The results showed that there were significant differences

Evaluation of a Simultaneous Myoelectric Control Strategy for a Multi-DoF Transradial Prosthesis.

While natural movements result from fluid coordination of multiple joints, commercial upper-limb prostheses are still limited to sequential control of multiple degrees of freedom (DoFs), or constrained to move along predefined patterns. To control multiple DoFs simultaneously, a probability-weighted regression (PWR) method has been proposed and has previously shown good performance with intramuscular electromyographic (EMG) sensors. This study aims to evaluate the PWR method for the simultaneous and proportional control of multiple DoFs using surface EMG sensors and compare the performance wit

Towards Sonomyography-Based Real-Time Control of Powered Prosthesis Grasp Synergies.

Sonomyography (ultrasound imaging) offers a way of classifying complex muscle activity and configuration, with higher SNR and lower hardware requirements than sEMG, using various supervised learning algorithms. The physiological image obtained from an ultrasound probe can be used to train a classification algorithm which can run on real time ultrasound images. The predicted values can then be mapped onto assistive or teleoperated robots. This paper describes the classification of ultrasound information and its subsequent mapping onto a soft robotic gripper as a step toward direct synergy contr

Predictive prosthetic socket design: part 2-generating person-specific candidate designs using multi-objective genetic algorithms.

In post-amputation rehabilitation, a common goal is to return to ambulation using a prosthetic limb, suspended by a customised socket. Prosthetic socket design aims to optimise load transfer between the residual limb and mechanical limb, by customisation to the user. This is a time-consuming process, and with the increase in people requiring these prosthetics, it is vital that these personalised devices can be produced rapidly while maintaining excellent fit, to maximise function and comfort. Prosthetic sockets are designed by capturing the residual limb's shape and applying a series of geomet

Biomechanical Characterisation of Bone-anchored Implant Systems for Amputation Limb Prostheses: A Systematic Review.

Bone-anchored limb prostheses allow for the direct transfer of external loads from the prosthesis to the skeleton, eliminating the need for a socket and the associated problems of poor fit, discomfort, and limited range of movement. A percutaneous implant system for direct skeletal attachment of an external limb must provide a long-term, mechanically stable interface to the bone, along with an infection barrier to the external environment. In addition, the mechanical integrity of the implant system and bone must be preserved despite constant stresses induced by the limb prosthesis. Three diffe

The role of exercise testing in predicting successful ambulation with a lower extremity prosthesis: a systematic literature review and clinical practice guideline.

BACKGROUND: Growing discontent with the k-level system for functional classification of patients with limb loss and movement of healthcare toward evidence-based practice has resulted in the need for alternative forms of functional classification and development of clinical practice guidelines to improve access to quality prosthetic interventions. The purpose of this project was to develop and present a clinical practice recommendation for exercise testing in prosthetic patient care based on the results and synthesis of a systematic literature review. METHODS: Database searches of PubMed, Goog

Preliminary Analysis Of Positive Knee Energy Injection In A Transfemoral Amputee Walking With A Powered Prosthesis.

Individuals with lower limb amputations show higher metabolic costs during ambulation than able-bodied individuals. This high metabolic cost may be explained by the inability of available passive prostheses to replicate the biomechanics of intact legs during walking. Powered prostheses promise to address this issue by imitating the power generation of intact legs. Several studies have investigated the effect of power generation and energy injection at the ankle joint in transtibial amputees. In contrast, no study has directly addressed knee power generation in transfemoral amputees. This study

Rehabilitation of the elderly amputee.

A rehabilitation service based in a department of geriatric medicine and dealing exclusively with amputees aged 65 years or over, is described. Provision of prostheses has been expedited, and the multi-disciplinary approach of the department has been beneficial in rehabilitation especially of the amputee with multiple pathology. The experience of the first 146 patients with an average age of 74.1 years is reviewed. Discharge home was achieved in 77.6% of the patients provided with a prosthesis and in a small multi-centre study of the post discharge status of 20 elderly amputees it was found th

A new in-built device for one-point stepless prosthetic alignment.

The authors have developed a new in-built device for alignment of below-knee prostheses. The device allows one-point, stepless adjustment of angle and slide, independently of each other and also after completed prosthetic manufacture. Principally, the device consists of a half-sphere for angle adjustment and a plate for both slide adjustment and socket application. If the socket has to be exchanged, the whole device can be re-used, apart from the plate, which forms part of the socket bottom. The device is light, compact, cosmetically easy to cover and cheap. The design incorporates a combinati

Semi-flexible sockets for amputation below the knee.

A semi-flexible socket fitted to a series of 47 consecutive below-knee amputees is evaluated, and the problems in assessing prosthetic components and appropriate methodology are discussed. The results suggest that this type of socket deserves wider use and further evaluation in active amputees, where comfort, perspiration control or the condition of the skin is interfering with the patient's function.
