Plaque modification of severely calcified coronary lesions via orbital atherectomy: Single-center observations from a complex Veterans Affairs cohort.

BACKGROUND: Orbital atherectomy (OA) is a known alternative to other atherectomy devices. However, some complex patient demographics (eg, left ventricular ejection fraction <25%) were excluded from the first-in-human trial (ORBIT I) and the pivotal FDA device approval trial (ORBIT II) which evaluated the safety and efficacy of OA in severely calcified de novo coronary lesions. This single-operator cohort study aimed to examine the impact of OA on a real-world complex Veterans Affairs patient subset. METHODS: Retrospective analysis was completed on 40 consecutive patients with severely calcifi

Orbital Atherectomy for Treating De Novo Severely Calcified Coronary Narrowing (1-Year Results from the Pivotal ORBIT II Trial).

Percutaneous coronary intervention of severely calcified lesions has historically been associated with major adverse cardiac event (MACE) rates as high as 30%. In the ORBIT II (Evaluate the Safety and Efficacy of OAS in Treating Severely Calcified Coronary Lesions) trial, treatment of de novo severely calcified lesions with the Diamondback 360° Coronary Orbital Atherectomy System (OAS) resulted in low rates of procedural and 30-day adverse ischemic events. The long-term results from this trial have not been reported. We sought to determine the 1-year outcomes after orbital atherectomy of sever

Gender differences in acute and 30-day outcomes after orbital atherectomy treatment of de novo, severely calcified coronary lesions.

OBJECTIVES: The aim of this analysis is to evaluate potential differences according to gender in terms of acute and 30-day clinical outcomes in patients enrolled in the ORBIT II trial with de novo, severely calcified coronary lesions treated with orbital atherectomy to facilitate stent delivery. BACKGROUND: Previous studies have shown an increased risk of safety events in females compared to males undergoing percutaneous coronary intervention. METHODS: ORBIT II, a prospective, nonrandomized, multicenter, single arm study conducted in the US evaluated the safety and efficacy of the coronary O

Distal shower embolization during directional coronary atherectomy and stenting for diffuse stenosis of right coronary artery: current limitations of intravascular ultrasonography for evaluating fragile plaque.

A 74 year-old male with old anterior and inferior myocardial infarctions was treated with staged percutaneous coronary intervention. A chronic total occlusion of the middle segment of the left anterior descending branch was successfully stented during the first stage, and during the second stage, preprocedural intravascular ultrasonography (IVUS) revealed that the proximal segment of the right coronary artery was diffusely stenosed by mixed plaque. Directional coronary atherectomy under IVUS guidance was performed, but coronary slow flow appeared during the procedure. After successfully bailin

In-Hospital and 1-Year Outcomes of Octogenarian and Nonagenarian Patients with Severely Calcified Coronary Lesions Treated with Rotational Atherectomy.

INTRODUCTION: Severely calcified coronary lesions are a common challenge in older percutaneous coronary intervention (PCI) populations. We aimed to investigate in-hospital and 1-year outcomes of octogenarian and nonagenarian patients with heavily calcified coronary lesions treated with rotational atherectomy (RA). METHODS: Patients who underwent RA at our center were divided into two groups: octogenarians/nonagenarians (age ≥ 80 years) (n = 194) and younger counterparts (age < 80 years) (n = 591). Patients presented with acute coronary syndrome, and those treated with bare-metal stents were e

Comparison of the Efficacy and Safety of Orbital and Rotational Atherectomy in Calcified Narrowings in Patients Who Underwent Percutaneous Coronary Intervention.

We aimed to compare the safety and efficacy of rotational atherectomy (RA) and orbital atherectomy (OA) during percutaneous coronary intervention in an all-comer population with severely calcified lesions. We included all patients who underwent percutaneous coronary intervention with OA or RA in our institution from October 2013 until October 2016. Comparison of baseline and procedural characteristics, along with acute complication rates and postprocedural cardiac enzyme elevation, was performed. There were 191 RA and 57 OA patients. Other than creatinine clearance, which was lower in patients

Rotational atherectomy and simultaneous kissing stenting of a bifurcation stenosis of left anterior descending and anomalous right coronary arteries.

Intervention to anomalous coronaries can present specific challenges: non-standard guide catheters may be required to provide adequate access and support, the course of the arteries can be tortuous with sharp angulations, and they may subtend a critical area of myocardium. We present the first report of percutaneous coronary intervention to the bifurcation of an anomalous right coronary artery arising from the left anterior descending artery. This lesion contained a high burden of calcification, which required rotational atherectomy of the left anterior descending artery before kissing stentin

Increase of chlamydial LPS antibodies in patients with acute coronary syndrome without detection of chlamydial DNA in atherectomy samples.

BACKGROUND: Chlamydia pneumoniae, a respiratory pathogen, has been connected with the pathogenesis of coronary artery disease (CAD). Previous studies indicated a correlation between antibodies to chlamydial LPS and the risk of cardiovascular disease. The aim of this study was to determine whether C. pneumoniae plays a direct role in the pathology of acute coronary syndromes (ACS). METHODS AND RESULTS: Twenty-five consecutive patients (median age 56 years) with ACS (17 acute myocardial infarction, 8 unstable angina) were included in the study and underwent directional coronary atherectomy. Predictors of acute and long-term outcome with transluminal extraction atherectomy: the New Approaches to Coronary Intervention (NACI) registry.

The New Approaches to Coronary Intervention (NACI) registry was established to define the role of new coronary devices in overcoming the limitations of balloon angioplasty. The purpose of the present study was to evaluate the acute and long-term efficacy of the transluminal extraction catheter (TEC) device utilizing data from the NACI registry and identify clinical and anatomic patient subsets who may benefit from this device. From 1990-1994, >4,300 patients from 39 clinical sites enrolled consecutive patients treated with one of the 7 new devices to the NACI registry. The study population con

Subintimal excimer laser coronary atherectomy (S-ELCA) as a bail-out for device-uncrossable chronic total occlusion: case report.

BACKGROUND: Chronic total occlusion (CTO) that cannot be crossed with balloons or microcatheters despite successful guidewire passage represent one of the most challenging scenarios in percutaneous coronary intervention (PCI). Excimer laser coronary angioplasty (ELCA) is an established adjunct for device-uncrossable lesions; however, the safety of its application within the subintimal space remains limited. CASE SUMMARY: A 67-year-old male with a complex cardiac history, including prior recurrent interventions and coronary artery bypass grafting, was referred for staged PCI of a right coronar

Stentless left main revascularisation for takayasu arteritis using directional coronary atherectomy and a drug-coated balloon: a case report.

BACKGROUND: Coronary involvement in Takayasu arteritis is characterized by chronic fibrotic thickening of the arterial intima and is associated with a high risk of restenosis after coronary intervention. In young women, coronary artery bypass grafting and coronary stenting are often undesirable, owing to long-term procedural implications and considerations regarding future pregnancy. CASE SUMMARY: A 21-year-old woman presented with severe left main coronary artery stenosis caused by Takayasu arteritis. After inflammation had been adequately controlled with glucocorticoid therapy and a monoclo

Superoxide generation in directional coronary atherectomy specimens of patients with angina pectoris: important role of NAD(P)H oxidase.

OBJECTIVE: NADH/NADPH oxidase is an important source of reactive oxygen species (ROS) in the vasculature. Recently, we demonstrated that p22(phox), an essential component of this oxidase, was expressed in human coronary arteries and that its expression was enhanced with the progression of atherosclerosis. The present study was undertaken to investigate its functional importance in the pathogenesis of coronary artery disease. For this aim, the expression of p22(phox), the distribution of oxidized low density lipoprotein (LDL), and the generation of ROS in directional coronary atherectomy (DCA)

Comparative Clinical Outcomes of Elective Coronary Atherectomy in Hospitals With Versus Without On-Site Surgical Backup: A Contemporary Nationwide Analysis.

The safety of complex percutaneous coronary intervention (PCI) at hospitals without on-site cardiac surgical backup (Non-SOS) has been broadly established, but data specifically addressing elective coronary atherectomy in this setting remain limited. To evaluate whether on-site cardiac surgical backup status influences clinical outcomes of elective coronary atherectomy procedures using contemporary nationwide data. A retrospective cohort study was conducted using the National Inpatient Sample (NIS) from 2016 to 2023. Elective hospitalizations involving coronary atherectomy were identified usin

Intravascular Lithotripsy Versus Rotational Atherectomy for Calcified Coronary Lesions: A Systematic Review and an Updated Meta-Analysis of Clinical Outcomes.

BACKGROUND: Severe coronary artery calcification (CAC) remains a significant challenge in interventional cardiology, especially in elderly and comorbid patients, such as diabetes or chronic kidney disease. CAC often leads to complications such as stent thrombosis and restenosis, therefore leading to bad clinical outcomes and increased major adverse cardiovascular events (MACE) rates. Traditional approaches, including rotational atherectomy (RA), are commonly used to treat calcified plaques; however, are limited by procedural complexity, length of procedures, and risk of vascular injury. In con

Initial Experience With GlideAssist to Facilitate Advancement of Orbital Atherectomy Prior to Plaque Modification of Severely Calcified Coronary Artery Lesions.

OBJECTIVES: We report our initial experience with GlideAssist (Cardiovascular Systems, Inc) to facilitate advancement of the orbital atherectomy crown prior to plaque modification of severely calcified coronary artery lesions. BACKGROUND: Severe coronary artery calcification increases the complexity of percutaneous coronary intervention (PCI) and is also associated with worse clinical outcomes compared with PCI of non-calcified vessels. Orbital atherectomy is an effective tool to modify calcified plaque prior to stenting. However, advancement of the orbital atherectomy crown may be technicall

Serial Assessment of Vascular Healing Process After Directional Coronary Atherectomy Followed by Drug-Coated Balloon: An Optical Coherence Tomography Study.

The mechanisms of luminal narrowing after percutaneous coronary intervention (PCI) with directional coronary atherectomy (DCA) and drug-coated balloon (DCB) remains unclear. This study aimed to investigate the pattern and mechanisms of luminal narrowing after DCA/DCB using optical coherence tomography (OCT). Patients who underwent DCA/DCB for de novo lesions and serial OCT imaging at 3, 6, and 18 months were evaluated to determine the pattern of luminal narrowing. Among 40 patients who had follow-up (F/U) OCT at 3 months post-PCI, 33 and 23 patients had F/U at 6 and 18 months, respectively. Th

Dual-Prep registry: Atherectomy devices and intravascUlAr lithotripsy for the PREParation of heavily calcified coronary lesions registry, 1-year results.

Combination therapy with atherectomy and intravascular lithotripsy (IVL) has emerged as a promising strategy for the treatment of severely calcified occlusive coronary lesions, which potentially enhances procedural efficacy without increasing complication risk. The Dual-Prep Registry is a multicenter, prospective registry designed to evaluate the safety and efficacy of IVL after atherectomy in severely calcified lesions. Combined use was selectively applied when the risk of complications was anticipated to be high with a larger atherectomy burr size, or when it was deemed non-beneficial due to

Intravascular Ultrasound-Guided Planned Rotational Atherectomy for Grade Ⅲ Coronary Calcified Stenose Versus Cutting Balloon: A Single-Center Prospective Study.

Planned rotational atherectomy (RA) has been widely used for severely calcified lesions, but moderate to severe coronary calcified lesions (CCL) are recommended for bailout RA. In this study, we sought to clarify whether intravascular ultrasound (IVUS)-guided planned RA is superior to cutting balloons (CB) for moderate to severe CCL. Patients with grade Ⅲ CCL were randomly assigned to percutaneous coronary intervention with RA or CB. The primary endpoint was strategy success and the coprimary endpoint was stent expansion. Secondary endpoints included procedural expense and duration, contrast a
