An 80-year-old woman with a history of congestive heart failure, atrial fibrillation, and hypertension was transferred to our institution with hematemesis. Her drug regimen included 2 mg warfarin potassium/day to prevent thromboembolic events. Transthoracic echocardiography (TTE) performed at 78 years of age revealed a mass attached to the noncoronary cusp and a cardiac tumor was suspected. The patient declined surgery and was meticulously followed up with periodic TTE. Upper gastroendoscopy revealed a gastric ulcer with an exposed blood vessel; anticoagulant therapy was ceased. On day 15 of a

A 70-year-old woman without any previous history of heart disease was referred to our hospital for repeated chest discomfort. She had experienced temporary hemiparesis because of a cerebral infarction of unknown etiology. Clinical evaluations were all within normal limits except for echocardiography. There was a mobile tumor in her ascending aorta, about 15 mm in diameter, adhering to a part of the right coronary cusp on its aortic side. Urgent surgery was performed. The tumor was attached to the edge of the right coronary cusp and excised with a part of the right coronary cusp, which was then

Endoscopic cardiac surgery offers favorable early outcomes because of its minimally invasive nature. However, its application in combined aortic and mitral valve procedures remains limited owing to differences in optimal visualization angles and the technical complexity of the procedure. A septuagenarian woman (body surface area 1.2 m2) with severe mitral stenosis, atrial fibrillation, and aortic valve papillary fibroelastoma, who was wheelchair-bound, underwent endoscopic double-valve surgery. Preoperative computed tomography revealed that a single camera port did not provide optimal visualiz

Papillary fibroelastoma (PFE) is a benign primary cardiac tumor arising from the endocardial surface, most commonly affecting the valvular endocardium. Although often incidental, PFEs may lead to serious complications, including systemic embolization, valvular obstruction, or regurgitation. Aortic valve fibroelastoma resulting in severe aortic insufficiency (AI) may be rare. A 77-year-old woman with a history of hypertension and chronic kidney disease presented with progressive dyspnea, orthopnea, and bilateral lower extremity edema. Examination revealed a diastolic murmur at the left lower st

A 66-year-old man with prostate cancer underwent contrast-enhanced computed tomography for cancer staging; the imaging incidentally revealed a 10 mm left ventricular mass. Transthoracic echocardiography demonstrated a highly mobile, pedunculated mass attached to a left ventricular false tendon. Cardiac magnetic resonance imaging was insufficient to characterize the tissue because of the extreme mobility of the lesion. Because of the left-sided location, pronounced mobility, and potential embolic risk, the tumour was excised en bloc with the attached false tendon via a transmitral approach thro

BACKGROUND: Papillary fibroelastoma (PFE) is a benign cardiac tumor typically found on valves and is often asymptomatic. However, embolic complications are known.
CASE SUMMARY: A patient with hypertrophic cardiomyopathy (HCM) was found to have an echocardiographic mass on the mitral valve. Despite no histologic confirmation, the imaging characteristics were consistent with PFE. The patient was managed conservatively and has remained asymptomatic under clinical and echocardiographic surveillance.
DISCUSSION: Although PFEs have been reported on the aortic valve and left ventricular outflow tra

BACKGROUND: It is very common for patients with newly diagnosed lung masses to have heart disease. However, papillary fibroelastomas (PFEs) of the tricuspid valve (TV) combined with lung cancer are rarely reported. It is thus unclear whether a two-stage surgery or concomitant surgery is optimal.
CASE PRESENTATION: We report the case of a 73-year-old Chinese male who was diagnosed with PFEs on the TV by transthoracic echocardiography (TTE) examination while being evaluated to undergo video-assisted thoracic surgery (VATS) for a right lower lung nodule. We resected both the PFEs and the lung no

We report a case of a 48-year-old man who complained episodes of angina at rest. Instrumental investigation demonstrated a mass involving the ventricular side of the aortic valve suggestive for papillary fibroelastoma; an anomalous origin of the right coronary artery from the left sinus of Valsalva and no significant coronary artery disease were shown as well. The patient underwent open heart surgery, the tumor was excised and the aortic valve was spared. Pathological examination confirmed the diagnostic hypothesis of papillary fibroelastoma. The resolution of anginal symptoms after surgery su

BACKGROUND: High-dose mediastinal radiotherapy can result in progressive valvular degeneration and secondary malignancy.
CASE SUMMARY: A 50-year-old woman with prior chest radiotherapy and stem cell transplantation for Hodgkin lymphoma presented with progressive exertional dyspnea. Transthoracic and transesophageal echocardiography revealed severe mitral regurgitation with mitral annular calcification and an incidental finding of a mobile 12 × 8 mm left atrial mass. The patient subsequently underwent mitral valve replacement and resection of the left atrial mass with histologic confirmation o

BACKGROUND: Tumors of the heart are rare compared to other cardiac diseases. Their clinical symptoms vary from absent to nonspecific. This great variation and general paucity of symptoms related to cardiac tumors often result in delayed diagnosis and treatment.
METHODS: We retrospectively evaluated all patients who underwent cardiac surgery for a space-occupying lesion in the observation period between 2000 and 2010 at our hospital. Clinicopathological features, imaging characteristics, and disease outcomes were analyzed, and the results were compared with the available English literature.

Cardiac masses are highly heterogeneous and vary widely in their clinical presentation, imaging features, and survival outcomes. Our understanding is limited by their rarity and the fact that few are confirmed based on surgical pathology. We set out to provide a comprehensive analysis of all cardiac masses resected at our institution from 1999 to 2015, including imaging methods and histopathologic findings. We found papillary fibroelastomas (PFEs) to be the most commonly resected benign cardiac masses, followed by myxomas. Patients with PFEs most frequently presented with cerebrovascular accid

PURPOSE OF REVIEW: Embolic stroke of undetermined source is a challenging clinical entity. While less common than atrial fibrillation and endocarditis, many noninfective heart valve lesions have been associated with stroke and may be considered as culprits for cerebral infarcts when other more common causes are excluded. This review discusses the epidemiology, pathophysiology, and management of noninfective valvular diseases that are commonly associated with stroke.
RECENT FINDINGS: Calcific debris from degenerating aortic and mitral valves may embolize to the cerebral vasculature causing sma

We describe a case of a young male without stroke risk factors who presented with a sudden onset of left-sided weakness, left hand numbness, and left eye blurriness. CT scan of the head without contrast and diffusion-weighted MRI of the brain with contrast revealed an ischemic stroke in the right middle cerebral artery distribution. Transesophageal echocardiography (TEE) revealed a mobile pedunculated mass on the posterior surface of the mitral valve. This mass was resected and pathology showed a cardiac papillary fibroelastoma (CPFE), which was determined to be the cause of the patient's card

This review article - which is devoted to a wide range of physicians, e.g., pathologists, clinicians, radiologists, and general practitioners - is an up-to-date clinico-pathological description of cardiac tumours. Although rare, cardiac tumours are key components in oncology practice since both their early diagnosis and appropriate management denote urgent needs. Primary cardiac tumours (PCTs) are categorized in recent WHO classifications as well as in other scientific contributions. Their incidence is around 0.02 % while their prevalence is between 0.001 % and 0.03 %. Among PCTs, benign lesio

Papillary fibroelastoma (PFE) is the second most common benign primary cardiac tumor, usually affecting left-sided valves. Pulmonary valve (PV) involvement is extremely rare, representing less than 8% of all PFEs. Although often asymptomatic, PFEs can present with embolic events. We report a 60-year-old man admitted with acute dyspnea, orthopnea, and lower-limb edema. Transthoracic echocardiography revealed severe left ventricular systolic dysfunction (ejection fraction: 15%), severe functional mitral regurgitation, and a mobile echogenic mass (1.0 × 0.8 cm) attached to the arterial surface of

BACKGROUND: Two-dimensional echocardiography is the main noninvasive imaging tool to identify cardiac masses but is unable to provide detailed tissue characterization.
AIM: The aim of the study was to assess the ability of low mechanical index (MI) contrast echocardiography to detect presence and amount of tissue vascularization as validated by histopathology study of cardiac masses.
METHODS AND RESULTS: Twelve consecutive patients (5 females and 7 males, age range 51-82 years) underwent conventional and contrast two-dimensional echocardiography with low MI. By contrast echocardiography, mas
