Independent Anomalous Origin of the Right Vertebral Artery from the Right Common Carotid Artery.

An anomalous origin of the right vertebral artery is rare. The left vertebral artery from the aortic arch is where most of the anomalies occur. The next is an origin of the right vertebral artery from the right common carotid artery in association with the aberrant right subclavian artery. However, independent anomalous origin of the right vertebral artery from the right common carotid artery has not been well known in the previous literature. We present this anomaly, and able to understand the mechanism of the occurrence by embryological knowledge. Failure of involution of the fourth segmenta

Vertebral artery to common carotid artery bypass in Takayasu's disease with delayed cerebral ischemia.

UNLABELLED: Takayasu's Arteritis is a progressive occlusive disease of the aortic arch and its branches. It may need several bypass procedures along with or without endovascular techniques. CASE DESCRIPTIONS: A twenty-six year old woman who had a history of Takayasu's Arteritis, and had a Gore-Tex Dacron Y-graft from the ascending aorta to the right subclavian and right common carotid arteries 18 years before, is presented. She presented with recurrent hemispheric ischemia and transient ischemic attacks and severe long segment stenosis in the left common carotid artery. She was successfully t

[A case of asymptomatic common carotid artery occlusion with the persistent proatlantal intersegmental artery].

A case of asymptomatic common carotid artery occlusion with direct vertebrocarotid anastomosis (proatlantal intersegmental artery) in a 70-year-old male is reported. Despite the acquired left common carotid artery occlusion, there was no demonstrable neurologic deficits that would occur by the involvement of the area supplied by the occluded carotid system. Cerebral angiography revealed, in addition to efficient collateral circulation from the contralateral carotid system through the anterior communicating artery, a collateral channel from the left vertebral artery to the ipsilateral external

Predicting the normal dimensions of the internal and external carotid arteries from the diameter of the common carotid.

In order to calculate the percentage stenosis of an artery, the residual lumen and the normal dimensions at the site of stenosis must be known. Although the residual lumen can be measured directly from an arteriogram, the normal dimensions cannot in the presence of atherosclerotic narrowing. Most calculations of percentage stenosis are based on assumptions of what the true diameter of the vessel would have been before the onset of atheroma. Measurements of the diameter of the common, internal and external carotid arteries were made at standard sites in 61 normal arteriograms. There was a wide

Spontaneous recanalization after acute common carotid artery occlusion: a case report.

INTRODUCTION AND IMPORTANCE: Albeit rare, spontaneous recanalization after common carotid artery (CCA) occlusion is an important medical phenomenon, as it can lead to increased blood flow to the brain tissue, thereby improving cerebral perfusion. However, it also increases the risk of reperfusion injury and distal cerebral embolism. CASE PRESENTATION: The patient was a 54-year-old man who presented with sudden right-sided limb weakness and speech impairment. Emergency computed tomography angiography (CTA) showed complete occlusion of the left CCA and internal carotid artery. CTA on day 1 indi

Open Retrograde Stenting of Proximal Innominate and Common Carotid Artery Stenosis.

PURPOSE: The evidence regarding the treatment of open retrograde stenting of innominate artery (IA) or common carotid artery stenosis (CCA) is limited, and is suspected to carry a high risk of stroke and death. Therefore, the objective of this study was to evaluate the outcomes of such hybrid procedures. METHODS: A retrospective, monocentric study of all patients who underwent retrograde stenting of proximal IA and CCA stenosis via surgical cutdown of the CCA, with or without concomitant CEA, between 2016 and 2023 was performed. RESULTS: Overall, 33 patients were treated. A total of 15 patie

Saddle Pulmonary Embolus and Simultaneous Common Carotid and Subclavian Artery Emboli.

A patent foramen ovale (PFO) is present in 27-35% of the population. Right to left cardiac shunts predispose patients to arterial emboli in the presence of venous thromboembolisms. Paradoxical embolus should be suspected in patients with deep venous thrombosis (DVT) and arterial emboli. A 45-year-old man with hypercoagulability and history of DVT presented with a week-long history of chest pain, shortness of breath, and left arm numbness. Imaging showed a saddle pulmonary embolus (PE) and emboli involving the aortic arch, left common carotid, and left subclavian artery. The patient proceeded w

Common carotid artery medialization and fracture dislocation of the cervical spine.

PURPOSE: To describe the management of the discovery of a retropharyngeal carotid artery in the context of a cervical dislocation. DESCRIPTION OF THE CASE: A 68-year-old female presented acute neck pain and incomplete tetraplegia following a fall on the stairs. Radiographs, contrast-enhanced computed tomography scan and magnetic resonance of the cervical spine revealed a C5-C6 bi-articular dislocation. A detailed preoperative assessment of the images discovered a medialization of the left common carotid artery. An external reduction and a left anterior cervical approach allowed a careful mana

[Variant of endarterectomy from common carotid artery in its total occlusion].

Currently, there are relatively few publications in the literature on reconstruction of the common carotid artery (CCA). CCA occlusion occurs in 2-4% of patients examined for cerebrovascular disease. Most described surgical techniques of blood flow restoration for CCA occlusions are: coronary artery bypass grafting, retrograde loop endarterectomy, endovascular and hybrid interventions. These techniques yield good remote results, however they are not devoid of disadvantages. This article describes a clinical case report concerning successful autotransplantation of the CCA in a female patient pr

Pseudoaneurysm of the Left Common Carotid Artery With Neurofibromatosis Type 1.

We describe a 54-year-old man with neurofibromatosis type 1 who presented with a left-sided neck mass. Computed tomography demonstrated a left common carotid artery aneurysm (51 × 33 mm). Surgery was performed because of the risk of rupture. The left common carotid artery was found to be a huge aneurysmal dilatation, and the arterial wall partially collapsed and extensively adherent to the surrounding tissues. Left common carotid artery to internal carotid artery bypass grafting was performed with a reversed saphenous vein graft. Histopathological examination revealed vascular fragility of the

Common Carotid Artery Occlusion: A Single-Center Experience in 40 Cases.

The incidence of common carotid artery occlusion (CCAO) is approximately 3% in patients who undergo angiography for symptomatic cerebrovascular disease; however, few studies have reported on management of this condition. The objective of this article was to analyze risk factors, therapeutic options, and clinical benefits of surgical treatment at a hospital in the city of São Paulo, Brazil. Data were collected from medical records of 40 patients with CCAO who were treated from June 2002 to October 2013. Results were analyzed retrospectively. Most of the patients were men (63.0%), who were signi

Common carotid artery dissection: a rare cause of acute neck swelling.

Spontaneous carotid artery dissection is a rare condition with potentially devastating consequences. Internal carotid artery and vertebral artery dissections have been implicated as the cause of 20% of strokes occurring in patients younger than 45 years. We describe a very rare case of a nontraumatic common carotid artery dissection in a 45-year-old man that was initially misdiagnosed as a sternocleidomastoid hematoma. This case highlights the need for vigilance for this often-missed diagnosis, as well as the indication for noninvasive imaging in unidentified neck swellings.

Body composition and common carotid artery remodeling in a healthy population.

CONTEXT: An independent association between obesity and preclinical carotid atherosclerosis has been demonstrated, however, the pathophysiological links were not clearly established. Body composition (BC) influences systemic hemodynamics and may participate in the remodeling of common carotid artery (CCA), independently of risk factors. OBJECTIVE: This study evaluated the association between CCA structure and BC in a large population of healthy subjects. DESIGN: This was a cross-sectional study. SETTINGS: The study was conducted at 19 European centers. SUBJECTS: The study included 627 heal

Bilateral common carotid artery occlusion--a case report and literature review.

Although unilateral common carotid artery (CCA) occlusion and bilateral internal carotid artery (ICA) occlusion have been reported in the past, bilateral CCA occlusion is rare. The management and mechanism of unilateral CCA occlusion and bilateral ICA occlusion are debatable, but those of bilateral CCA occlusion are largely unknown. Herein, we present a case of bilateral CCA occlusion that had an acceptable outcome with medical treatment. The literature regarding unilateral CCA occlusion, bilateral ICA occlusion and bilateral CCA occlusion is also discussed. Experience implies that the collate

Mydriatic pupil as the presenting sign of common carotid artery dissection.

BACKGROUND: Ipsilateral mydriasis is known to accompany signs of cerebral ischemia in unilaterally compromised carotid blood flow. Mydriasis as the presenting sign of common carotid artery (CCA) dissection has not been reported thus far. CASE DESCRIPTION: We report the case of a patient who presented with a mydriatic pupil after intraoperative injury of the ipsilateral CCA. Mydriasis preceded complete third-nerve palsy and symptoms of cerebral ischemia for 12 hours. Cerebral angiography revealed occlusion of the CCA with slow collateral flow to the internal carotid artery and fetal origin of
