A demonstration of clinical reasoning through a case of scrotal infection.

Clinical reasoning has been defined as collecting clues, processing information, and coming to an understanding of a patient problem, then planning and implementing interventions, evaluating outcomes, and reflecting on and learning from the process. Clinicians with effective reasoning skills positively impact patient outcomes.

Clinical reasoning and decision-making of pre-professional speech-language therapists during clinical swallowing examination.

BACKGROUND: Sound clinical reasoning is crucial for synthesizing assessment and contextual information into evidence-based and patient-centred management decisions. Whilst clinical reasoning processes and development have been explored in broader health contexts, to date there has been limited evaluation of how clinical reasoning skills are employed by pre-professional speech-language therapists (SLTs) who conduct clinical swallowing examinations (CSEs). AIMS: To evaluate how pre-professional SLTs (senior undergraduate students) apply reasoning and decision-making processes during simulated C

Endocrinology Clinical Skills Assessment Program: A Structured Clinical Assessment Program to Enhance Clinical Reasoning in Subspecialty Training.

INTRODUCTION: The Endocrinology Clinical Skills Assessment Program (ECSAP) was developed at Wake Forest University School of Medicine to enhance history-taking, diagnostic reasoning, and personalized feedback in endocrinology fellows. Recognizing a gap in formative clinical skill evaluation, ECSAP introduces standardized case-based assessments across key endocrine topics. METHODS: ECSAP sessions occur twice yearly and include four 25-minute standardized clinical scenarios (e.g., adrenal incidentaloma, hypercalcemia). Each case is pre-assigned to faculty, who provide fellows with a one-line pr

Scaphoid fractures: a higher order analysis of clinical tests and application of clinical reasoning strategies.

INTRODUCTION: The purpose of this study was to identify clinical tests for scaphoid fractures and, using a higher order analysis, to determine their diagnostic accuracy. METHODS: A literature review of the databases CINAHL, Embase, Medline and PUBMED from 1980 to September 30, 2011 was conducted to obtain applicable literature on clinical tests used in identifying scaphoid fractures. Methodological quality was determined using the criteria for validity suggested by Sackett et al (2000). Using a random effects model, pooled positive likelihood ratios (PPLR) were established for any test evalua

Reconciling how clinical reasoning is learned in the age of artificial intelligence.

Two recent studies examine the role of AI in supporting medical students’ learning – a longitudinal study of self-reported outcomes from supervised real-world AI use, and a simulated experiment comparing the benefits of AI explanations against the risks of plausible misinformation. Collectively, their findings demonstrate that the effects of AI may depend on the individual and educational environment, which has implications for how we design medical curricula and define clinical competence.

Clinical Reasoning: A 28-Year-Old Man with Subacute-Onset Ataxic Gait.

The diagnostic approach to subacute progressive polyneuropathy can be challenging, particularly when accompanied by systemic features. We present a 28-year-old man with subacute-onset ataxic gait. Four months before his initial evaluation at our center, he was assessed by orthopedics for a poorly differentiated tumor of the right tibia. He also reported intermittent fever and unintentional weight loss. Neurologic examination revealed marked weakness in all four limbs, with deep tendon reflexes absent in the lower extremities. Sensory testing demonstrated a stocking-glove distribution of hypoes

Clinical reasoning research priorities for advancing diagnostic safety: researcher and patient perspectives.

OBJECTIVES: This study used both researcher and patient perspectives to identify high-priority research questions related to clinical reasoning. METHODS: This study integrates findings from two previously published research prioritization exercises aimed at advancing the science of diagnostic safety, focusing on questions relevant to clinical reasoning. In Study 1 (April 2019), 97 researchers submitted 177 unique research questions that could be useful "to reduce patient harm in the diagnosis process". In Study 2 (November 2019), 41 patients/caregivers/advocates submitted 84 unique research q

Physiotherapists' Clinical Reasoning and Therapeutic Strategies in Fibromyalgia: A Qualitative Interpretative Study.

Background/Objectives: Fibromyalgia (FM) is a chronic pain condition characterized by widespread pain, fatigue, sleep disturbances, and psychosocial challenges. Although physiotherapists play a key role in FM management, little is known about how biopsychosocial principles are applied in clinical practice. This study explored physiotherapists' clinical reasoning, therapeutic strategies, and approaches to promoting adherence and functional recovery in FM. Methods: A qualitative study was conducted within a constructivist-interpretative paradigm. Thirteen physiotherapists with experience in FM m

Clinical Reasoning: A 28-Year-Old Man With Seizures and Thalamic Lesions.

This case involves a 28-year-old Black man who presented with new-onset seizures and bilateral medial temporal lobe hyperintensities on brain MRI, initially raising concern for viral encephalitis. CSF analysis revealed mild lymphocytic pleocytosis, but extensive infectious testing was negative. One year later, he developed acute right-sided weakness, and an MRI showed an enhancing FLAIR/T2 hyperintense mass-like lesion in the left thalamus. Stereotactic biopsy demonstrated reactive changes and Periodic acid-Schiff diastase (PASD)-positive macrophages, prompting consideration of atypical infect

Clinical Reasoning: A 60-Year-Old Diabetic Woman With Fluctuating Visual Disturbances.

Visual disturbances are a common presenting complaint in neurology, and the characteristics of these complaints combined with the examination findings are important in localization and then the generation of a complete differential diagnosis. Patients with diabetes, particularly in the setting of hyperglycemia, can have multiple reasons for vision changes localized throughout the visual pathways, including ophthalmic manifestations such as diabetic retinopathy, early development of nuclear sclerotic cataracts, and macular edema, as well as increased risk of ischemic optic neuropathies and retr

Clinical Reasoning: A Woman With Subacute Progression of Distal Upper Extremity Weakness.

A 65-year-old woman presented with 3 months of progressive hand weakness, initially with distal-predominant symptoms. EMG was notable for diminished amplitudes in bilateral radial nerves without evidence of conduction block and with normal sensory nerve action potentials. Anti-acetylcholine receptor and antistriated muscle antibodies were positive, but subsequent EMG did not reveal abnormalities on repetitive nerve stimulation. Muscle biopsy was performed, revealing extensive inflammatory infiltrates with significant associated fibrosis. This case discusses the approach to localization within

The Limits of Inductive Reasoning for Clinical Evidence Appraisal: A Simulation Study.

Background To establish the possible likelihood of a body of evidence, inductively judged to be of 'low bias risk'/'high-quality' according to a limited set of appraisal criteria, of actually being error-free. Methods A total of 45 simulation trials were generated and randomly assigned to 0-5 errors out of a total of 65 error domains. The trials were then appraised for errors with a simulated appraisal tool consisting of five pre-specified error domains. Trial appraisal yielded either true positive, true negative, false negative or false positive results. From these values, the negative likeli

Clinical Reasoning: A 5-year-Old Boy With Subacute and Refractory Myoclonus.

We present a case of a 5-year-old, who developed refractory, multifocal myoclonus, which progressed over several weeks, leading to a profound functional decline. The diagnostic workup was complicated by a compelling genetic finding that suggested a possible primary epileptic encephalopathy. However, meticulous history-taking uncovered key details inconsistent with a purely genetic disorder, raising suspicion for an alternative, acquired condition. This case highlights the critical importance of a detailed clinical history and neurologic examination to guide the diagnostic process, particularly

Diagnosing Trigeminal Neuralgia Based on Clinical Diagnostic Reasoning in an Elderly Patient.

When pain adversely affects a patient's activities its diagnosis needs to be fast and accurate to allow effective treatment to be commenced as soon as possible. Difficulties may be found in achieving this, however, in elderly patients with age-associated cognitive decline, as they may not be capable of properly understanding or recalling their symptoms. The present case concerns a 77-year-old woman who presented with the chief complaint of pain in the right mandible persisting throughout the day, and severe enough to necessitate her lying down in bed all day long. The use of open-ended questio
