Rebuilding Vaccine Confidence Through Community-Centered Health Promotion: A Call for Trust, Engagement, and Sustainable Immunization.

Vaccination remains one of the most effective public health interventions, yet declining vaccine confidence has emerged as a major threat to global immunization programs. Vaccine hesitancy, shaped by misinformation, institutional mistrust, sociocultural factors, and inequitable access to reliable health information and services, has contributed to stagnating vaccination coverage and the resurgence of vaccine-preventable diseases. This Perspective, which draws on a purposive rather than systematic reading of the literature, argues that rebuilding vaccine confidence requires moving beyond predom

Distribution and correlates of long-acting injectable antipsychotic use among community mental health center patients.

Although long-acting injectable antipsychotics (LAIs) are an important pharmaceutical option in the management of schizophrenia and related disorders, little is known about patient characteristics related to LAI use in real-world outpatient settings. We analyzed electronic medical records from 41,401 patients who received psychiatric services from one of 16 regional mental health centers operated by the South Carolina Department of Mental Health in 2022. We compared the use of first- and second-generation LAIs and oral antipsychotics by sociodemographic (age, gender, race/ethnicity, zip code,

A Qualitative Study of Staff and Provider Experiences with Follow-up Colonoscopy in a Community Health Center and Gastroenterology Specialty.

Colorectal cancer screening is an effective strategy to prevent disease, reduce the risk of advanced-stage diagnosis, and improve survival. Timely follow-up of abnormal screening results, particularly abnormal fecal immunochemical tests, is essential to realizing these benefits. This qualitative study examined routine processes related to colonoscopy referral and completion for patients of a federally qualified health center (FQHC) referred to community gastroenterology (GI) practices. Using a snowball sampling approach, five FQHC individuals and eight community GI practice individuals were in

Facilitators and Barriers to the Rollout of Doxycycline Postexposure Prophylaxis for Sexually Transmitted Infections in a Boston Community Health Center.

BACKGROUND: Doxycycline postexposure prophylaxis (doxyPEP) is a promising strategy to prevent bacterial sexually transmitted infections. Limited data exist evaluating patient and provider experiences since doxyPEP has become widely available. We aimed to explore such factors among providers and patients during real-world implementation within one community health center. METHODS: Doxycycline postexposure prophylaxis was rolled out at Fenway Health on February 2, 2023. To support rollout, electronic health record tools, 3 provider training sessions, and a community town hall were developed and

Delivery of an Ebola Virus-Positive Stillborn Infant in a Rural Community Health Center, Sierra Leone, 2015.

We report the case of an Ebola virus (EBOV) RNA-negative pregnant woman who delivered an EBOV RNA-positive stillborn infant at a community health center in rural Sierra Leone, 1 month after the mother's last possible exposure. The mother was later found to be immunoglobulins M and G positive indicating previous infection. The apparent absence of Ebola symptoms and not recognizing that the woman had previous contact with an Ebola patient led health workers performing the delivery to wear only minimal personal protection, potentially exposing them to a high risk of EBOV infection. This case emph

Interpersonal psychotherapy for depressed women with sexual abuse histories: a pilot study in a community mental health center.

Depression among women with childhood sexual abuse histories has a chronic and treatment-refractory course, and is accompanied by high rates of comorbid illness and adult trauma exposures. Reducing the disproportionate burden of serious mental illness among depressed, traumatized women must be a priority in community mental health settings. Effective treatments are needed. The feasibility and effects of interpersonal psychotherapy (IPT) for women with major depression and childhood trauma histories were tested. Twenty-five women in a community mental health center were enrolled in a 16-session

Attitudes and beliefs regarding colorectal cancer screening among African American patients of community health centers in North-Central Florida and stakeholder perspectives: a qualitative study.

BACKGROUND: Colorectal cancer (CRC) is one of the leading causes of cancer mortality in the US with African Americans continuing to experience CRC health disparities due to numerous factors. While colonoscopy remains the most utilized test for CRC screening, stool-blood tests are cost-effective and offered by community health centers (CHC) serving un- and underinsured patients. Qualitative research has identified multilevel barriers to CRC screening and can potentially inform the development of tailored interventions to promote stool-based screening. METHODS: In this qualitative study, we exp

Attitudes toward and training in medications for opioid use disorders: a descriptive analysis among employees in the youth legal system and community mental health centers.

BACKGROUND: Research demonstrates gaps in medications for opioid use disorder uptake (MOUDs; methadone, buprenorphine, and naltrexone) especially among adolescents. These gaps may be partly attributable to attitudes about and training in MOUDs among youth-serving professionals. We extended prior research by conducting descriptive analyses of attitudes regarding effectiveness and acceptability of MOUDs, as well as training in MOUDs, among youth legal system (YLS) employees and community mental health center (CMHC) personnel who interface professionally with youth. METHODS: Using survey data fr

Is implementation of the 5 A's of smoking cessation at community mental health centers effective for reduction of smoking by patients with serious mental illness?

We tested whether implementing the "5 A's" (Ask, Advise, Assess, Assist, Arrange) at six mental health centers reduces smoking among persons with serious mental illness. One hundred and fifty six patients were evaluated just before initiating the 5 A's and after six and 12 months. A delayed control condition evaluated 148 patients six months before 5 A's implementation, just before and then after six months. Six months of the 5 A's produced no effect. Modest cessation and reduction benefits were noted after 12 months. Implementing the 5 A's at community mental health centers may have modest be

Detection of domestic violence by community mental health teams: a multi-center, cluster randomized controlled trial.

BACKGROUND: Domestic Violence and Abuse (DVA) is associated with a range of psychosocial and mental health problems. Having a psychiatric illness increases likelihood of being a victim of DVA. Despite the evidence of a high risk for DVA and the serious effects of violent victimization in psychiatric patients, detection rates are low and responses are inadequate. The aim of the BRAVE (Better Reduction trough Assessment of Violence and Evaluation) study is to improve detection of and response to DVA in psychiatric patients. In this article, we present the protocol of the BRAVE study which follow

Patients' medical knowledge and health counseling: what kind of information helps to make communication patient-centered?

OBJECTIVE: To examine how physicians use information about a patient's background knowledge when both anticipating what a patient knows and producing actual answers in an email counseling setting. METHODS: A fictitious patient used a (high vs. low) level of technical jargon in an email inquiry about diabetes and provided explicit information on prior knowledge (high vs. low) through self-report. Final-year medical students (semi-experts) were asked to gauge the patient's knowledge level (Experiment 1) and to produce an answer to the inquiry (Experiment 2). A total of N=150 participated in one
