OBJECTIVES: Distributions of laryngeal spaces are important in understanding their functional significance. However, laryngeal spaces in the newborn larynx are enigmatic.
METHODS: Five normal human newborn larynges were investigated using whole organ serial section technique.
RESULTS: The chief newborn laryngeal spaces were the preepiglottic space, the paraglottic space and the cricoid area. They consisted of an areolar tissue area composed of brown adipose tissue (immature adipose tissue) and loose elastic and collagen fibers. The newborn preepiglottic space was immature and occupied a smal

Pathological findings can be subtle or limited, in some cases of fatal neck compression. Decalcification and en-bloc transverse histological processing using mega block cassettes, revealed a number of internal unicortical fractures to the thyroid lamina (anteriorly at the midline and posteriorly) and bilaterally in the cricoid cartilage, not visible on external inspection, which proved to be medico-legally important, in a case with very few other pathological findings, and where the application of a neck/head lock was suspected. In order to explore the nature and mechanism of these poorly reco

OBJECTIVE: The upper esophageal sphincter plays a significant role by forming a physical barrier at the junction of the hypopharynx and the cervical esophagus. As few studies have focused on the ventral aspect of this junction, the contribution of the anterior wall of the cervical esophagus to upper esophageal sphincter function remains unknown. The purpose of this study was to examine the muscle fibers' arrangement at the junction, especially of those forming its anterior wall.
METHODS: Thirteen specimens from 13 Japanese cadavers were analyzed. Six specimens were dissected macroscopically,

In this study the authors evaluated the outcomes of tracheal resection and anastomosis in postintubation tracheal stenosis. The authors also aimed to identify clinical and radiological factors associated with restenosis. Thirty-four consecutive patients were included in the study. Patients with subglottic stenosis who underwent procedures involving cricoid cartilage and patients with other etiologies were excluded from the study. Univariate analysis was performed to determine the risk factors for restenosis. Receiver operating characteristic (ROC) curves were generated for the diameter of the

A novel narrow-field laryngectomy procedure known as central-part laryngectomy (CPL) for less invasive laryngeal diversion in patients with intractable aspiration is introduced. We conducted retrospective case reviews of 15 patients who underwent CPL. In this procedure, an area of the glottis including the mid-part of the thyroid cartilage and cricoid cartilage is removed to separate the digestive tract from the air way. The lateral part of the thyroid cartilage, the entire hypopharyngeal mucosa and epiglottis are preserved. The superior laryngeal vessels and nerve are not invaded. All fifteen

BACKGROUND: Thyroid cartilage is an important barrier in anterior cervical approach surgery. The objective of this study is to establish normative values for thyroid cartilage at three planes and to determine their significance on preoperative positioning and intraoperative traction in surgery via the anterior cervical approach.
METHODS: Neck CT scans were collected from 600 healthy adults who did not meet any of the exclusion criteria. Transverse diameters (D1, D2, and D3) of the superior border of the thyroid cartilage (SBTC), inferior border of the thyroid cartilage (IBTC), and the trachea

OBJECTIVE: To establish the anatomical relationships between the arytenoid and cricoid cartilages and apply these findings to design an arytenoidectomy based on a sound anatomical basis.
STUDY DESIGN: In 50 patients the length of the vocal process and the distance between vocal process tip and upper border of the cricoid cartilage were endoscopically measured. Twenty-five total laryngectomy specimens were sagittally and axially sectioned to verify the position of the arytenoids and their relation to the cricoid. The anatomical findings led to the design of laser partial arytenoidectomy and co

Forestier's disease, or ankylosing hyperostosis of the cervical spine, has been described to cause dysphagia, foreign body sensation, and aspiration. We report two patients with ankylosing hyperostosis producing ulceration of the posterior plate of the cricoid cartilage, inflammatory edema, and secondary bilateral vocal cord paralysis with airway obstruction. This disease initially produces minimal dysphagia when the primary location of the osteophyte is just above and posterior to the cricoid, then, progressive airway obstruction. The pathogenesis is infection superimposed on ulceration of th

Acquired laryngeal stenosis is a well-documented complication of endotracheal intubation. Premature infants requiring ventilatory support for respiratory distress syndrome are at increased risk for developing laryngeal stenosis. Until recently, the techniques for repair of laryngeal stenosis have required a temporary tracheotomy which complicates the management and has an associated mortality. In 1980 the use of an anterior cricoid split was described to treat laryngeal stenosis in infants. The technique has been modified: the anterior cricoid split procedure as originally described required a

Laryngeal trauma is rare but serious and potentially deadly injury. The prompt diagnosis and management of acute laryngeal trauma is necessary because the clinical presentation is variable depending on the location, severity, and mechanism of injury. Two case histories are presented: (1) case history A: a 53-year-old male, after motor vehicle accident, fractured the mid anterior thyroid cartilage and both aspects of the cricoid cartilage; however, this patient was asymptomatic from the above fractures; and (2) case history B: a 41-year-old male who sustained trauma to the chest, neck, and left

OBJECTIVE: Previous investigation of the adult cadaver larynx has shown that the cricoid diameter never exceeds that of the glottis. This further analysis looks at the anatomic relationships between the diameter of the cricoid ring and those of the main tracheobronchial tree to discover how they relate to double-lumen tube selection.
DESIGN: Prospective.
SETTING: A city mortuary.
PARTICIPANTS: Adult cadavers undergoing autopsy.
INTERVENTIONS: The trachea was partially transected above the first ring to expose the cricoid cartilage from below and was then further opened with a progressive l

Objective:To investigate the prognostic impact of different tumor invasion patterns in the surgical treatment of T3 glottic laryngeal cancer. Methods:A retrospective analysis was conducted on the clinical data of 91 patients with T3 glottic laryngeal cancer. Results:Among the 91 patients, 58 cases (63.7%) had anterior invasion and 33 cases (36.3%) had posterior invasion. The posterior invasion was significantly correlated with invasions of the dorsal plate of cricoid cartilage (P<0.001), arytenoid cartilage (P= 0.001), and subglottic region(P = 0.001). There was no statistical difference in su

OBJECTIVE: To investigate the feasibility of dynamic computed tomography in recording and describing the spatial motion characteristics of the arytenoid cartilage.
METHODS: Dynamic computed tomography recorded the real-time motion trajectory of the arytenoid cartilage during inspiration and phonation. A stationary coordinate system was established with the cricoid cartilage as a reference and a motion coordinate system was established using the movement of the arytenoid cartilage. The Euler angles of the arytenoid cartilage movement were calculated by transformation of the two coordinate syst

INTRODUCTION: Cartilaginous tumours of the larynx are rare. They usually involve cricoid cartilage, less frequently thyroid cartilage and other cartilages. The most significant clinical manifestations are hoarseness, dyspnea, dysphagia or a neck mass. On physical examination, tumour is found as a bulge with intact mucosa or a tumour situated in a part of the larynx also with fixation. CT scanning is the mainstay of radiographic imaging. The histopathologic diagnosis is made after the surgical excision. Prognosis for survival is good. The recurrences occur very often, also with malignant transf

The neural crest domestication syndrome (NCDS) hypothesis suggests that selection pressures for tameness during animal domestication resulted in a down-regulation of neural crest cell migration and proliferation, providing a mechanistic explanation for traits commonly observed across domesticates. We test the viability of this hypothesis via a derived prediction. According to the NCDS hypothesis, neural crest-derived structures should be reduced throughout the entire organism in domesticated variants compared to their wild ancestors. Here, we test this prediction by comparing the larynges of w

OBJECTIVE: Surgical treatment of a subglottic stenosis can lead to a cricoid cartilage resection, cervical trachea resection, or both that has to be reconstructed with an artificial prosthesis. However, there is at present no standard treatment in the numerous reconstruction procedures, and many prostheses encounter inflammatory rejection and mechanical constraint problems. A porous, metastable β-type titanium alloy prosthesis was developed to improve subglottic rehabilitation.
METHODS: We have designed a porous type of airway prosthesis made of porous metastable β-type titanium alloy. We use

OBJECTIVE: Subglottic laryngotracheal stenosis with side-to-side narrowing poses a challenge for successful reconstruction. The standard technique of anterior cricoid resection was modified to address a small ventricle with lateral narrowing.
METHODS: This study was a retrospective chart review and telephone questionnaire follow-up of consecutive patients with subglottic stenosis at a single institution. Follow-up questionnaires used Likert scales (ratings 1-10) to describe preoperative and postoperative symptoms, satisfaction, and perceived effectiveness. Once the anterior cricoid is removed
