How European centres diagnose, treat, and prevent CIED infections: results of an European Heart Rhythm Association survey.

The purpose of our survey is to analyse the clinical approach used to prevent and treat cardiovascular implantable electronic device (CIED) infections in Europe. The survey involves high-volume implanting centres. According to the survey the incidence of CIED infections shows a slight decrease in most centres and is substantially under 2% in the majority of centres interviewed. However, there are still differences in terms of prophylactic antibiotic therapy: 8.9% of the centres administer oxacillin as preoperative treatment, 4.4% of them do not give any antibiotic therapy, all centres use some

Transvenous lead extraction, factors affecting procedural difficulty.

BACKGROUND: A complex transvenous lead extraction (TLE) procedure could be associated with lower success and higher complication rates in inexperienced hands. In this study, we aim to assess the factors that determine procedural difficulty in TLE. METHODS: We retrospectively studied 200 consecutive patients undergoing TLE in a single referral centre from June 2020 to December 2021. Lead extraction difficulty was assessed by the success of simple manual traction with or without a locking stylet, the need for advanced extraction tools and the number of tools required to extract the lead. Logist

Effectiveness and safety of transvenous extraction of single- versus dual-coil implantable cardioverter-defibrillator leads at single-center experience.

The available literature lacks data concerning direct comparison of the effectiveness and safety of single- versus dual-coil implantable cardioverter-defibrillator (ICD) leads transvenous extraction. Certainly, additional shocking coil in superior vena cava adds to the amount of metal in the vascular system. Adhesions developing around the superior vena cava coil add to the difficulty of extraction of ICD lead if lead removal is required. The aim of the study was to assess the effectiveness and safety of single- and dual-coil ICD leads transvenous extraction using mechanical systems. We perfor

Inadvertent defibrillator lead placement into the left ventricle after MitraClip implantation: A case report.

RATIONALE: Inadvertent pacemaker/defibrillator lead placement into the left ventricle is an unusual cardiac device-related complication and its diagnosis is not always easy and often misunderstood. Thromboembolic events are frequently associated with this procedural complication. Percutaneous lead extraction should be performed when diagnosis is made early after device implantation while long-life oral anticoagulation is a wise option when the diagnosis is delayed and the lead is not removed. PATIENT CONCERNS: A 65-year-old man affected by dilated cardiomyopathy, previously treated with a per

Semaglutide-associated Twiddler syndrome.

52-year-old woman with cardiac sarcoidosis and a dual-chamber implantable cardioverter defibrillator (ICD) was treated with semaglutide for refractory obesity (body mass index (BMI) of 40.1 (kg/m2)). After rapid weight loss of 25 kg, she developed painful device mobility and right ventricular lead dysfunction. Chest x-ray revealed lead entanglement consistent with Twiddler syndrome, which is a mechanical complication in which a pacemaker or ICD rotates within its pocket, causing the leads to twist or dislodge and resulting in device malfunction. Lead extraction was complicated by cardiac tampo

Procedural Characteristics and Outcomes of Transvenous Lead Extraction in Patients With Prior Sternotomy.

BACKGROUND: A history of sternotomy has been debated as either a potential protective factor or a possible risk for worse outcomes in patients undergoing transvenous lead extraction (TLE). The safety and efficacy of TLE in this population remain underexplored. OBJECTIVES: This study sought to evaluate the impact of prior sternotomy on procedural outcomes in patients undergoing TLE. METHODS: Using a prospectively maintained registry, we included all patients undergoing TLE at our center between 1996 and 2022. Patients were categorized into 2 groups based on whether they had a prior sternotomy

When Pacing Fails After Generator Replacement: A Stepwise Diagnostic Approach to a Reversible Lead-Header Interface Problem.

Background and Clinical Significance: Early loss of pacing capture after pacemaker generator replacement is an uncommon but potentially life-threatening event, especially in pacemaker-dependent patients. In this setting, device malfunction is often initially attributed to intrinsic lead damage, prompting consideration of invasive lead revision or extraction. However, not all early failures reflect true structural lead dysfunction. Careful interpretation of device interrogation findings, particularly in relation to pacing configuration, may uncover reversible causes and support a more targeted

Transvenous lead extraction in patients with persistent left superior vena cava.

PURPOSE: Predictors of difficulty and complications of transvenous lead extraction (TLE) have been investigated in several studies; however, little is known about the venous anatomical characteristics that can have an impact on procedural outcomes. Among them, the persistent left superior vena cava (PLSVC) is a common anomaly often discovered incidentally during cardiac device implantation and could raise concerns if TLE is indicated. We report technical considerations and outcomes of TLE for two patients with leads implanted via PLSVC. METHODS AND RESULTS: Two cardiac implantable electronic

Percutaneous Rescue With Sequential Second-Device Usage for Left Atrial Appendage Perforation During Amulet Implantation.

BACKGROUND: Left atrial appendage closure (LAAC) prevents stroke in nonvalvular atrial fibrillation, while device-related left atrial appendage (LAA) perforation may lead to cardiac tamponade. CASE SUMMARY: A man in his early 80s underwent percutaneous LAAC using the Amulet device. During deployment, sustained counterclockwise torque in a ball-shaped configuration resulted in perforation of the LAA, with device extrusion and progressive pericardial effusion. To avoid bleeding upon extraction, we executed a sequential second-device rescue: The first device was left in situ to tamponade the def

Research on the active sealing and promoting extraction technology of high-gas mine working face return air corner gas.

To address the issue of excessive gas concentration in the return corner under "U"-shaped ventilation in high-gas mine working faces, the 317 working face of Wangjialing Mine was selected as the research object, and a combination of theoretical analysis, numerical simulation, and engineering experiments was employed to analyze the causes of gas accumulation, construct a 3D model to simulate gas distribution and flow in the goaf, and develop a novel active sealing-extraction-enhancement device, which was then tested in the field. The research results indicate that: (1) Air leakage in the goaf,

Superior venous use of bridge balloon for percutaneous lead extraction in a patient with an interrupted inferior vena cava: a case report.

BACKGROUND: Transvenous lead extraction (TLE) is the gold-standard treatment for cardiac implantable electronic device removal but carries a small risk of major complications, including superior vena cava (SVC) injury. The prompt use of endovascular occlusion devices can provide temporary haemostasis and haemodynamic stability in the rare event of SVC laceration. The presence of congenital heart disease and venous anomalies introduces additional technical complexity and necessitates careful procedural planning. CASE SUMMARY: A 52-year-old gentleman with dextrocardia and situs inversus totalis
