Non-invasive estimation of material properties of normal and dissected human ascending aortas in vivo: comparison with the ex vivo tensile experiment.

OBJECTIVE: Patient-specific aortic material properties play a critical role in aortic dissection development. In this study, a non-invasive method was employed to assess the in vivo anisotropic mechanical properties of normal and dissected ascending aortas and compare them with their ex vivo material properties. METHODS: Biaxial tensile testing was performed on 10 ascending aortic specimens (five patients with type-A aortic dissection and five donors without aortic diseases), with testing data fitted using anisotropic Mooney-Rivlin models. An iterative algorithm was proposed to determine in v

Reduced mechanical strength correlates with decreased elastin content in aortic intima-media tissue: association with dissection in human ascending aortas.

Aortic dissection (AD) caused by the intimal tear would progress into the separation of aortic media, leading to devastating sequelae. Quantifying the mechanical and histological changes in aortic intima-media (IM) tissue during the AD development would advance our understanding of the mechanism behind the disease initiation for its prevention and management. Twenty-eight ascending aortic specimens were harvested from 19 patients with type A AD and 9 organ donors to obtain 120 aortic IM tissue samples. Uniaxial tensile experiments were performed on the samples until they failed, and their ulti

Acute Type A Aortic Dissection Treated Using a Tubular Stent-Graft in the Ascending Aorta and a Multibranched Stent-Graft in the Aortic Arch.

PURPOSE: To describe the combined use of a tubular stent-graft for the ascending aorta and an inner-branched arch stent-graft for patients with acute type A aortic dissection. TECHNIQUE: The technique to deploy these modular, custom-made stent-grafts is demonstrated in 2 patients with acute DeBakey type I aortic dissections and significant comorbidities precluding open surgery. Both emergent procedures were made possible by the availability of suitable devices manufactured for elective repair in other patients. After preliminary carotid-subclavian bypass, a long Lunderquist guidewire was intr

[Anesthetic management in a patient with acute dissection of the ascending aorta with difficult weaning from cardiopulmonary bypass due to acute myocardial infarction].

We report a case of acute dissection of the ascending aorta with difficult weaning from cardiopulmonary bypass (CPB) due to acute myocardial infarction (MI). Difficult separation from CPB was thought to be due to global coronary artery malperfusion, caused by diffuse left coronary artery sclerosis and the obstructed origin of the right coronary artery arising from the left sinus of Valsalva, resulting in intraoperative MI. In this case, supplemental coronary artery bypass grafting was effective for the separation of the patient from CPB.

A case of stent-assisted balloon-induced intimal disruption and relamination of distal remaining aortic dissection after the ascending aorta and aortic arch replacement for acute aortic dissection.

BACKGROUND: The early survival rate of patients with acute type A aortic dissection (TAAD) has improved remarkably over the past two decades. However, a false lumen may remain after proximal aortic repair and is a potential risk factor for aortic diameter enlargement, aortic rupture, and death in the chronic phase. In the stent-assisted balloon-induced intimal disruption and relamination of aortic dissection (STABILISE) technique, a stent graft is implanted at the proximal part of the aortic dissection, followed by bare-metal aortic stent placement over the distal aortic dissection. Next, an a

Extensive primary repair of the thoracic aorta in acute type a aortic dissection by means of ascending aorta replacement combined with open placement of triple-branched stent graft: early results.

BACKGROUND: To simplify extensive primary repair of the thoracic aorta in acute type A aortic dissection, we developed the open triple-branched stent graft placement technique. The early results of this new technique are reported. METHODS AND RESULTS: Between June 2008 and November 2009, 30 patients with acute Stanford type A aortic dissection underwent extensive primary repair of the thoracic aorta by means of ascending aorta replacement combined with open placement of triple-branched stent graft. Placement of the triple-branched stent graft into the true lumen of the descending aorta, arch,

A rare case of a simultaneous post-dissection saccular aneurysm of the ascending aorta and large pulmonary artery aneurysm with secondary embolism: a case report.

BACKGROUND: Aneurysms of the pulmonary arteries and the ascending aorta are rare, and both bear a high mortality risk if left untreated. In general, these entities are primarily caused by etiologies such as hypertension, pulmonary arterial hypertension, infection or congenital disorders. Treatment requires a rapid diagnostic work-up or even immediate surgical intervention in acute cases. Nevertheless, surgery entails serious perioperative risks, in particular in patients with multiple comorbidities. CASE PRESENTATION: We discuss a 70-year-old woman presented with decompensated heart failure b

[A successful surgical treatment of acute dissecting aneurysm of the ascending aorta: repair of the proximal intimal tear necessitating bypass for the right coronary artery].

This is a case report of the successful surgical treatment of acute dissecting aneurysm of the ascending aorta. The patient was a 65-year-old female, who admitted to the hospital 3 hours after the onset of severe back pain. Enhanced CT of the chest and abdomen revealed acute dissection of the ascending aorta from its origin to the bilateral common iliac arteries. Thirteen hours initiation of the symptoms, operation was carried out using pump-oxygenator with hypothermia. Transverse incision of the ascending aorta revealed acute dissection in anterior aspect extending to two third of the circumf

Asymptomatic large aortic dissection from ascending to abdominal aorta in a young man with Marfan syndrome: A case report challenging conventional diagnostic paradigms.

INTRODUCTION: Aortic dissection, a serious medical condition characterized by a tear in the aorta's inner layer leading to the establishment of a false channel within the vessel wall, in this report, we present a very rare case of asymptomatic substantial aortic dissection spanning from the ascending aorta to the abdominal aorta. CASE PRESENTATION: A 36-year-old male with fever and cough was found to have clinical features suggestive of Marfan syndrome. Examination revealed bilateral fine crackles and a diastolic heart murmur. Imaging showed bilateral infiltrates, hyperinflation, and aortic d

Aorto-Cutaneous Fistula and False Aneurysm of the Ascending Aorta Five Years after its Prosthetic Replacement for Stanford Type A Acute Aortic Dissection.

Aorto-cutaneous fistula and false aneurysm of the ascending aorta in patients who previously underwent Stanford Type A acute aortic dissection are rare and severe complications. Surgical correction remains a demanding challenge. In a case of false aneurysm rupture during redo sternotomy, selective cannulation of the right axillary and left carotid arteries allowed an efficient method of cerebral perfusion.

Circumferent dissection of the ascending aorta resulting in the occlusion of supra-aortic vessels repaired using the frozen elephant trunk technique - a case report.

BACKGROUND: Our patient presented with acute back pain and dyspnea, without neurological symptoms. The computed tomography (CT) scan showed a circumferent rupture of the ascending aortic intima which was invaginated in the arch and descending aorta. CASE PRESENTATION: A 54-year-old male patient was diagnosed with a Type A aortic dissection. He was immediately transferred to our operation room (OR) from the emergency department of a peripheral hospital. He presented with a circumferential dissection of the ascending aorta originating just distal to the coronary ostia, with the invaginated inti

Acute type A aortic dissection with a tear in the posterior wall of the ascending aorta early after off-pump coronary artery bypass grafting.

The patient was a 68-year-old man who underwent triple-vessel OPCAB uneventfully with aortic proximal anastomosis at one site using the saphenous vein with a mechanical device, not a side clamp, with mild traction of the ascending aorta by aortic taping. On postoperative day 7, computed tomography revealed extremely localized AAD with a tear on the posterior wall of the ascending aorta. Emergent ascending aortic replacement was successfully performed. Surprisingly, the tear extended laterally along the traction site of the tape. To our knowledge, this is the first report of AAD early after OPC

Aortic debranching from the ascending aorta: a novel surgical approach for extended TAAA and chronic type B dissections in high risk patients.

AIM: Extend thoraco-abdominal aortic aneurysms (TAAA) involving arch vessels and the visceral arteries remains a challenging operation when affecting high risk patients (HRP). Recently, hybrid surgery has gained popularity for HRP. The conventional surgical repair is the gold standard for low risk patients with previous mortality from 6% to 15% in thoracic aneurysms up to 30% in thoracic type B dissections. The risk of paraplegia is 3% to 15%. Without repair the outcome is poor with only 35% of patient's survival at two years after diagnosis. The total endovascular technique is not widespread

[Dissection of the ascending aorta complicated by acute myocardial infarction in a patient with Marfan's syndrome and anomalous origin of the left circumflex artery. Description of a case].

Acute myocardial infarction is a rare complication of ascending aortic dissection; it is usually due to the coronary ostium involvement. The authors describe an unusual case with anomalous left coronary origin too; they suggest that, due to this combination, the aneurysmatic area pushes the anomalous coronary artery, causing acute myocardial infarction.

Single-stage total thoracoabdominal endovascular repair for acute non-A non-B aortic dissection with retrograde extension to the ascending aorta and concomitant abdominal dissecting aneurysm: a case report.

BACKGROUND: Acute non‑A non‑B aortic dissection represents a complex and high-risk condition. Traditional treatment protocols mandate emergency open surgical repair for any dissection involving the ascending aorta (Stanford Type A or DeBakey Type I/II). With ongoing advancements in endovascular devices, less invasive options are emerging as viable alternatives for select high-risk patients with ascending aortic involvement, potentially offering advantages over total arch replacement. CASE PRESENTATION: A 73-year-old male presented with one week of chest and back pain and was diagnosed with ac

Deep Hypothermic Circulatory Arrest vs. Antegrade Cerebral Perfusion in Cerebral Protection during the Surgical Treatment of Chronic Dissection of the Ascending and Arch Aorta.

Circulatory arrest during aortic surgery presents a risk of neurological complications. The present study aimed to investigate the effectiveness of deep hypothermic circulatory arrest (DHCA) vs. antegrade cerebral perfusion (ACP) in cerebral protection during the surgical treatment of chronic dissection of the ascending and arch aorta and to assess the quality-of-life (QoL) in the long-term postoperative period with respect to the used cerebral protection method. In a prospective, randomized study, 58 patients with chronic type I aortic dissection who underwent ascending aorta and aortic arch
