[Low temperature plasma ablation and drug stent implantation for laryngotracheal atresia: a case report].

This paper reports a case of acquired laryngosis stenosis treated by arc plasma ablation, drug stent implantation and balloon dilatation. The patient with difficulty in pulling out the tube after intubation 3 years ago was diagnosed with laryngotracheal stenosis and treated with tracheotomy, in order to extubation. Physical examination showed that the metal trachea cannula in front of the neck was in position, unobstructed, and can be seen slightly inspiratory three concave sign when moving. CT and laryngoscope showed subglottic stenosis or atresia. After the operation, the children successful

Extremely Late In-Stent Thrombosis 12 Years After Implantation of a Drug-Eluting Stent.

Stent thrombosis is one of the most feared complications of percutaneous coronary intervention. Most commonly it occurs within the first few days after the deployment of the stent. Once the stent is completely endothelialized, this complication becomes extremely rare. Few cases of very late stent thrombosis were reported in the literature with the longest interval being around 11 years after the initial intervention. We report here the case of a 78-year-old male patient who presented with acute onset chest pain found to have acute inferior ST-segment elevation myocardial infarction due to thro

Cilostazol for Chinese Patients with Aspirin Intolerance after Coronary Drug-Eluting Stent Implantation.

BACKGROUND: Cilostazol-based dual antiplatelet therapy (DAPT) is widely used in patients with aspirin intolerance after coronary drug-eluting stent (DES) implantation in China. However, this empirical strategy is not recommended or even mentioned in Chinese or international guidelines due to a lack of evidence from large-scale studies. We aimed to explore the efficacy and safety of cilostazol-based DAPT in this special population. METHODS: In this cohort study, patients were grouped according to the DAPT strategy that they received after coronary DES implantation. The primary efficacy endpoin

Enhancing Plasticity of the Central Nervous System: Drugs, Stem Cell Therapy, and Neuro-Implants.

Stroke represents the first cause of adult acquired disability. Spontaneous recovery, dependent on endogenous neurogenesis, allows for limited recovery in 50% of patients who remain functionally dependent despite physiotherapy. Here, we propose a review of novel drug therapies with strong potential in the clinic. We will also discuss new avenues of stem cell therapy in patients with a cerebral lesion. A promising future for the development of efficient drugs to enhance functional recovery after stroke seems evident. These drugs will have to prove their efficacy also in severely affected patien

Very Late Stent Thrombosis 11 Years after Implantation of a Drug-Eluting Stent.

Very late stent thrombosis is an infrequent yet potentially fatal complication associated with drug-eluting stents. We report the case of an 88-year-old man who sustained an ST-segment-elevation myocardial infarction 11 years after initial sirolimus-eluting stent implantation. Optical coherence tomograms of the lesion showed that the focal incomplete endothelialization of the stent struts was the likely cause; neointimal formation, neoatherosclerosis, and late stent malapposition might also have contributed. To our knowledge, this is the longest reported intervening period between stent insert

In vitro and in vivo release characteristics of Tacrolimus (FK506) from an episcleral drug-delivery implant.

PURPOSE: To investigate the in vitro and in vivo release characteristics of Tacrolimus (FK506) from an episcleral drug-delivery implant. METHODS: For in vitro experiments, Tacrolimus-loaded implants (0.5 mL; at concentrations of 0.25, 0.5, and 1.0 mg/mL) were immersed in a balanced salt solution. Samples of the surrounding liquid were aspirated at different times over a 96-h period. For in vivo experiments, the experimental group received an implant loaded with Tacrolimus (0.5 mg/mL; 0.5 mL); the control group was given a subconjunctival injection of 0.5 mL Tacrolimus (0.5 mg/mL). Incidence and predictors of the late catch-up phenomenon after drug-eluting stent implantation.

BACKGROUND: Although clinical restenosis within 1 year after percutaneous coronary intervention has been remarkably reduced with the advent of drug-eluting stents (DES), the late catch-up (LCU) phenomenon remains an issue despite medical advances. The aim of this study was to investigate the incidence and predictive factors of the LCU phenomenon in an unselected population treated with first-generation DES. METHODS: A total of 923 patients treated with DES between June 2004 and August 2008 were analyzed. The LCU phenomenon was defined as secondary revascularization 1 year after index stenting

Oral antiplatelet therapy after drug-eluting stent implantation: adherence and side-effects.

Prolonged dual antiplatelet therapy is the cornerstone of the therapy after drug-eluting stent implantation. However, it is associated with risks and side-effects, such as bleeding events, which can negatively affect overall prognosis. Moreover, premature antiplatelet therapy discontinuation is not uncommon both in clinical trials and in clinical practice. It can lead to serious short-term and long-term complications, such as stent thrombosis, acute myocardial infarction and death. Although lack of patient's motivation can be regarded as an important cause of drug withdrawal, recent studies sh

Angioscopic assessment of peri-stent contrast staining following drug-eluting stent implantation.

BACKGROUND: Peri-stent contrast staining (PSS) has been recognized as a predictor of late stent thrombosis following drug-eluting stent (DES) implantation. However, the intravascular conditions at PSS sites remain unclear. METHODS AND RESULTS: We evaluated 10 patients (median age 72 years, 80% male) with stable angina pectoris by coronary angioscopy. The patients had a total of 11 DES implantations (5 sirolimus-eluting stents; 4 paclitaxel-eluting stents; 2 everolimus-eluting stents) that showed PSS. Neointimal coverage (NIC), presence of thrombus, and yellow plaques underneath the stent were

Prognostic impact of lipid contents on the target lesion in patients with drug eluting stent implantation.

We sought to determine the morphologic predictors of major adverse cardiac events (MACEs) after successful percutaneous coronary intervention (PCI) with drug-eluting stents (DES), using integrated backscatter intravascular ultrasound (IB-IVUS). Conventional IVUS and IB-IVUS were performed in 260 consecutive patients who underwent PCI with DES. Three-dimensional analyses were performed to determine plaque volume and the volume of each plaque component (lipid, fibrous, and calcification). Patients were divided into two groups according to the median lipid volume (LV) in the target lesion. MACEs

Predictors of early and late target lesion revascularization after drug-eluting stent implantation.

INTRODUCTION: Recent studies have suggested the existence of a late catch-up phenomenon after drug-eluting stent (DES) implantation. The aim of this study is to identify predictors of early (≤1 year) and late (>1 year) target lesion revascularization (TLR) in DESs. METHODS: The COACT (CathOlic medical center percutAneous Coronary inTervention) registry was designed to evaluate the clinical outcomes after DES implantation. Data from 9,127 consecutive patients were reviewed, all of whom underwent percutaneous coronary intervention (PCI) with DES between January 2004 and December 2009, including

Beneficial impact of prolonged dual antiplatelet therapy after drug-eluting stent implantation.

BACKGROUND: Twelve-month dual antiplatelet therapy (DAT) with aspirin and clopidogrel after drug-eluting stent (DES) implantation is routinely recommended. It is unclear if prolonged (>12-month) DAT is also favorable. We compared the outcome of patients discontinuing DAT 12 months after off-label DES implantation versus those with DAT for >12 months. METHODS: Baseline, treatment, and outcome data of patients undergoing off-label DES implantation and free from events 11.5 months after index procedure were retrospectively retrieved. Those discontinuing DAT between 11.5 and 12.5 months (12-month

Origin of restenosis after drug-eluting stent implantation in hyperglycemia is inflammatory cells and thrombus.

AIMS: The cellular and molecular mechanisms and safety after drug-eluting stent (DES) implantation in diabetic patients are still poorly understood; therefore, in this study, we evaluated the pathologic responses of the sirolimus-eluting stent (SES) or paclitaxel-eluting stent (PES) in a type I diabetes mellitus (DM) rat model. METHODS: The type I DM rat model was manipulated by intra-peritoneal streptozotocin injection. Two weeks later, DES was implanted in the aorta of rats with hyperglycemia or not as a control. Four weeks after DES implantation, the stented aorta was isolated and histomor

Transdermal power transfer for recharging implanted drug delivery devices via the refill port.

This paper describes a system for transferring power across a transdermal needle into a smart refill port for recharging implantable drug delivery systems. The device uses a modified 26 gauge (0.46 mm outer diameter) Huber needle with multiple conductive elements designed to couple with mechanical springs in the septum of the refill port of a drug delivery device to form an electrical connection that can sustain the current required to recharge a battery during a reservoir refill session. The needle is fabricated from stainless steel coated with Parylene, and the refill port septum is made fro

A review of implantable intravitreal drug delivery technologies for the treatment of posterior segment eye diseases.

Intravitreal implantable device technology utilizes engineered materials or devices that could revolutionize the treatment of posterior segment eye diseases by affording localized drug delivery, responding to and interacting with target sites to induce physiological responses while minimizing side-effects. Conventional ophthalmic drug delivery systems such as topical eye-drops, systemic drug administration or direct intravitreal injections do not provide adequate therapeutic drug concentrations that are essential for efficient recovery in posterior segment eye disease, due to limitations posed

Understanding the impact of accelerated release testing conditions on physicochemical properties and drug release mechanisms from etonogestrel implants based on ethylene vinyl acetate.

Accelerated release testing is a useful tool for both development and validation of controlled release polymeric implants, as it can shorten development cycles. The objective of this study was to investigate the impact of ethanol and elevated temperature on drug release mechanisms of etonogestrel implants. Based on studies conducted using EVA films, ethanol caused swelling, which increased permeability primarily by increasing solubility while having no impact on polymer crystal structure. Higher temperature caused increased drug release rate but also changed crystal structure. Etonogestrel res

Biodegradable drug-eluting nanofiber-enveloped implants for sustained release of high bactericidal concentrations of vancomycin and ceftazidime: in vitro and in vivo studies.

We developed biodegradable drug-eluting nanofiber-enveloped implants that provided sustained release of vancomycin and ceftazidime. To prepare the biodegradable nanofibrous membranes, poly(D,L)-lactide-co-glycolide and the antibiotics were first dissolved in 1,1,1,3,3,3-hexafluoro-2-propanol. They were electrospun into biodegradable drug-eluting membranes, which were then enveloped on the surface of stainless plates. An elution method and a high-performance liquid chromatography assay were employed to characterize the in vivo and in vitro release rates of the antibiotics from the nanofiber-env
