Improvement of Tourette syndrome symptoms after intractable temporal lobe epileptic surgery: a case report.

BACKGROUND: The comorbidities of either epilepsy or Tourette syndrome (TS) are heterogeneous. However, the co-occurrence of epilepsy and TS conditions is rarely encountered, let alone effective treatments that address both neurologic disorders at the same time. METHODS: We report a 24-year-old female patient who was diagnosed with TLE and TS. She presented for seizure control. After evaluation with stereo-electroencephalography and electrocorticography monitoring, the patient underwent a resective surgery treatment and was followed for 9 months. RESULTS: At the last follow-up, the patient re

Epidural Electrotherapy for Epilepsy.

Penetrating electronics have been used for treating epilepsy, yet their therapeutic effects are debated largely due to the lack of a large-scale, real-time, and safe recording/stimulation. Here, the proposed technology integrates ultrathin epidural electronics into an electrocorticography array, therein simultaneously sampling brain signals in a large area for diagnostic purposes and delivering electrical pulses for treatment. The system is empirically tested to record the ictal-like activities of the thalamocortical network in vitro and in vivo using the epidural electronics. Also, it is newl

Numerical processing in the human parietal cortex during experimental and natural conditions.

Human cognition is traditionally studied in experimental conditions wherein confounding complexities of the natural environment are intentionally eliminated. Thus, it remains unknown how a brain region involved in a particular experimental condition is engaged in natural conditions. Here we use electrocorticography to address this uncertainty in three participants implanted with intracranial electrodes and identify activations of neuronal populations within the intraparietal sulcus region during an experimental arithmetic condition. In a subsequent analysis, we report that the same intrapariet

[Localizing target for transcranial electrical stimulation in epilepsy patients combining scalp electroencephalogram and neural computational model].

For patients with MRI-negative drug-resistant epilepsy, noninvasive localization of targets for transcranial electrical stimulation (tES) remains a clinical challenge. This study proposes a novel target localization approach that integrates electroencephalogram source imaging, brain network analysis, and a neural computational model. We analyzed electrocorticography (ECoG) data from 12 patients, quantified the epileptogenicity of epileptic network nodes, and noninvasively located optimal stimulation targets. Three source imaging methods and two brain network reconstruction measures were compar

Intracranial electroencephalographic connectivity analysis to localize epileptogenic networks: Systematic review and meta-analysis from ILAE Epilepsy Surgery Networks Task Force.

Intracranial electroencephalographic (iEEG) connectivity analysis is a promising method to localize epileptic networks and guide surgical planning in focal drug-resistant epilepsy. Despite numerous studies exploring its utility, the added value of iEEG connectivity over standard clinical presurgical evaluation remains unclear. We assess the current evidence on the efficacy of iEEG connectivity analyses to improve seizure outcomes following epilepsy surgery through a systematic review and meta-analysis. Following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) report

Chronic Cranial Window Technique for Repeated Cortical Recordings During Anesthesia in Pigs.

Cortical recordings are essential for extracting neuronal signals to inform various applications, including brain-computer interfaces and disease diagnostics. Each application places specific requirements on the recording technique, and invasive solutions are often selected for long-term recordings. However, invasive recording methods are challenged by device failure and adverse tissue responses, which compromise long-term signal quality. To improve the reliability and quality of chronic cortical recordings while minimizing risks related to device failure and tissue reactions, we developed a c

Ictal Bruxism in Temporal Lobe Epilepsy: Intracranial EEG Connectivity Study.

Ictal bruxism is a rare motor manifestation in temporal lobe epilepsy with unclear network underpinnings. We report the first case demonstrating SEEG-guided network characterization of ictal bruxism in a 55-year-old man with drug-resistant temporal lobe epilepsy who continued to experience stereotyped seizures with rhythmic teeth grinding after a failed anterior temporal lobectomy. SEEG recorded one spontaneous and two stimulation-induced seizures sampling from the superior temporal gyrus, anterior ventral insula (aVInsula), and nine additional regions of interest. Recordings identified seizur

ECoGScope: An integrated platform for real-time Electrophysiology and fluorescence imaging.

In this study, we present ECoGScope, a versatile neural interface platform designed to integrate multiple functions for advancing neural network research. ECoGScope combines an electrocorticography (ECoG) electrode array with a commercial microendoscope, enabling simultaneous recording of ECoG signals and fluorescence imaging. The electrode array, constructed from highly flexible and transparent polymers, ensures conformal contact with the brain surface, allowing unobstructed optical monitoring of neural activity alongside electrophysiological recordings. A key innovation is the compact connec

Decoding of Individual Fingers Attempted Movement from Epidural ECoG in a Patient with Tetraplegia.

Brain-Computer interfaces (BCIs) enable direct communication between the brain and external devices. This technology holds significant potential for restoring motor function in individuals with severe neurological impairments. Among others, restoration of fine hand motor functions allowing grasping and objects manipulation is a priority for enhancing patients' lifestyle. Decoding finger movements is crucial for the precise control of hand neuroprosthetics. In this article, we analyzed neural activity of a tetraplegic patient implanted with two WIMAGINE ECoG recording devices in front of the se

Lateralization and Time-Course of Cortical Phonological Representations during Syllable Production.

Spoken language contains information at a broad range of timescales, from phonetic distinctions on the order of milliseconds to semantic contexts which shift over seconds to minutes. It is not well understood how the brain's speech production systems combine features at these timescales into a coherent vocal output. We investigated the spatial and temporal representations in cerebral cortex of three phonological units with different durations: consonants, vowels, and syllables. Electrocorticography (ECoG) recordings were obtained from five participants while speaking single syllables. We devel

Detection of spreading depolarization events and spatiotemporal analysis for advancing stroke therapy.

While the presence of spreading depolarization (SD) and associated spreading depression have been well studied and known to be associated with post-ischemic brain damage, the spatiotemporal spread of these events from the site of injury is not well understood. With the recent development of high-density micro-electrocorticographic (ECoG) electrode arrays, monitoring the spread of the depolarizing events and associated depression is possible. The goal of this work is to define the electrocorticographic features of SD and associated depression across the multichannel array and search for pattern

Left hemisphere dominance for bilateral kinematic encoding in the human brain.

Neurophysiological studies in humans and nonhuman primates have revealed movement representations in both the contralateral and ipsilateral hemispheres. Inspired by clinical observations, we ask if this bilateral representation differs for the left and right hemispheres. Electrocorticography was recorded in human participants during an instructed-delay reaching task, with movements produced with either the contralateral or ipsilateral arm. Using a cross-validated kinematic encoding model, we found stronger bilateral encoding in the left hemisphere, an effect that was present during preparation

Epilepsy surgery program in a resource-limited setting in Vietnam: A multicentered collaborative model.

OBJECTIVE: Although epilepsy surgery is more effective than medical therapy for drug-resistant patients, it is underutilized in both high-income and low- and middle-income countries. In light of our efforts to establish an epilepsy surgery program in a resource-limited setting, this study aimed to determine the outcome of the epilepsy surgery program in Ho Chi Minh City (HCMC), Vietnam. METHODS: In 2018, we developed the HCMC epilepsy core multidisciplinary team with members from various hospitals and centers. The team typically included neurologists, neurosurgeons, neuropsychologists, psychi

Intraoperative localization and preservation of reading in ventral occipitotemporal cortex.

OBJECTIVE: Resective surgery in language-dominant ventral occipitotemporal cortex (vOTC) carries the risk of causing impairment to reading. Because it is not on the lateral surface, it is not easily accessible for intraoperative mapping, and extensive stimulation mapping can be time-consuming. Here the authors assess the feasibility of using task-based electrocorticography (ECoG) recordings intraoperatively to help guide stimulation mapping of reading in vOTC. METHODS: In 11 patients undergoing extraoperative, intracranial seizure mapping, the authors recorded induced broadband gamma activati

Leveraging electrophysiologic correlates of word encoding to map seizure onset zone in focal epilepsy: Task-dependent changes in epileptiform activity, spectral features, and functional connectivity.

OBJECTIVE: Verbal memory dysfunction is common in focal, drug-resistant epilepsy (DRE). Unfortunately, surgical removal of seizure-generating brain tissue can be associated with further memory decline. Therefore, localization of both the circuits generating seizures and those underlying cognitive functions is critical in presurgical evaluations for patients who may be candidates for resective surgery. We used intracranial electroencephalographic (iEEG) recordings during a verbal memory task to investigate word encoding in focal epilepsy. We hypothesized that engagement in a memory task would e

Four core properties of the human brain valuation system demonstrated in intracranial signals.

Estimating the value of alternative options is a key process in decision-making. Human functional magnetic resonance imaging and monkey electrophysiology studies have identified brain regions, such as the ventromedial prefrontal cortex (vmPFC) and lateral orbitofrontal cortex (lOFC), composing a value system. In the present study, in an effort to bridge across species and techniques, we investigated the neural representation of value ratings in 36 people with epilepsy, using intracranial electroencephalography. We found that subjective value was positively reflected in both vmPFC and lOFC high

Mapping human temporal and parietal neuronal population activity and functional coupling during mathematical cognition.

Brain areas within the lateral parietal cortex (LPC) and ventral temporal cortex (VTC) have been shown to code for abstract quantity representations and for symbolic numerical representations, respectively. To explore the fast dynamics of activity within each region and the interaction between them, we used electrocorticography recordings from 16 neurosurgical subjects implanted with grids of electrodes over these two regions and tracked the activity within and between the regions as subjects performed three different numerical tasks. Although our results reconfirm the presence of math-selecti

Cortical Spreading Depolarizations and Clinically Measured Scalp EEG Activity After Aneurysmal Subarachnoid Hemorrhage and Traumatic Brain Injury.

BACKGROUND: Spreading depolarizations (SDs) are associated with worse outcome following subarachnoid hemorrhage (SAH) and traumatic brain injury (TBI), but gold standard detection requires electrocorticography with a subdural strip electrode. Electroencephalography (EEG) ictal-interictal continuum abnormalities are associated with poor outcomes after TBI and with both delayed cerebral ischemia (DCI) and poor outcomes after SAH. We examined rates of SD detection in patients with SAH and TBI with intraparenchymal and subdural strip electrodes and assessed which continuous EEG (cEEG) measures wer

Medically resistant pediatric insular-opercular/perisylvian epilepsy. Part 2: outcome following resective surgery.

OBJECTIVE Seizure onset in the insular cortex as a cause of refractory epilepsy is underrepresented in the pediatric population, possibly due to difficulties localizing seizure onset in deep anatomical structures and limited surgical access to the insula, a complex anatomical structure with a rich overlying vascular network. Insular seizure semiology may mimic frontal, temporal, or parietal lobe semiology, resulting in false localization, incomplete resection, and poor outcome. METHODS The authors retrospectively reviewed the records of all pediatric patients who underwent insular cortical res

"Mirror EPC": epilepsia partialis continua shifting sides after rolandic resection in dysplasia.

BACKGROUND: Epilepsia partialis continua (EPC) is a life-threatening condition often caused by focal cortical dysplasia (FCD). Resection of the motor cortex is contemplated in the hope that the trade-off between a severe motor deficit and complete seizure control justifies the procedure. METHODS: Report of 3 patients with EPC due to histologically confirmed FCD, who underwent resection of the motor cortex under acute electrocorticography. RESULTS: All had re-emergence of medically intractable EPC in the other side of the body after rolandic resection. Two patients died and the third continue
