Comparison of procedure-related complications and success rates between duodenoscopes with disposable end caps and conventional reusable duodenoscopes: a randomized trial.

BACKGROUND AND AIMS: Duodenoscopes with disposable distal end caps (DDEC) have been introduced to reduce the risk of transmitting multi-drug-resistant organisms compared to conventional reusable duodenoscopes (CRD). However, there have been reports of increased procedure-related complications with DDEC. This study aims to compare the complication rate and procedure success rate between DDEC and CRD. METHODS: Patients undergoing endoscopic retrograde cholangiopancreatography (ERCP) were randomized to either the DDEC or CRD group. The primary outcomes were procedure-related complications, while

Efficacy of spray flushing in the reprocessing of flexible endoscopes: A randomized controlled trial.

BACKGROUND: Commonly used cleaning brushes in the reprocessing of flexible endoscopes often cause damage within the working channels. AIM: To develop a spray flushing system to achieving effective cleaning of the working channels while minimizing damage. METHODS: This prospective study included 60 used endoscopes and 60 Teflon tubes randomly divided into a control group (n = 30) and an experimental group (n = 30). The material of Teflon tubes was the same as that of the endoscope working channel. Endoscopes in the control group were manually cleaned using traditional cleaning brushes, while

Pilot investigation into osteotome hard surface coating and cutting-edge degradation.

BACKGROUND: Osteotomes are bone cutting tools commonly reused in orthopedic surgical procedures. Despite undergoing rigorous cleaning, visual inspection, and sterilization procedures between every use, the condition of the cutting blade edge is commonly not qualitatively assessed. Subjective feedback from surgeons suggests a large variation in osteotome cutting-edge sharpness is found during use. This study seeks to investigate the retention of osteotome cutting-edge sharpness by comparing the wear resistance of as-supplied, electroless nickel, and titanium nitride coated osteotomes following

Differences in alarm events between disposable and reusable electrocardiography lead wires.

BACKGROUND: Disposable electrocardiographic lead wires (ECG-LWs) may not be as durable as reusable ones. OBJECTIVE: To examine differences in alarm events between disposable and reusable ECG-LWs. METHOD: Two cardiac telemetry units were randomized to reusable ECG-LWs, and 2 units alternated between disposable and reusable ECG-LWs for 4 months. A remote monitoring team, blinded to ECG-LW type, assessed frequency and type of alarm events by using total counts and rates per 100 patient days. Event rates were compared by using generalized linear mixed-effect models for differences and noninferio

Lymphocyte contamination of laryngoscope blades--a possible vector for transmission of variant Creutzfeldt-Jakob disease.

Variant Creutzfeldt-Jakob disease (vCJD) is associated with extensive prion infection of lymphoreticular tissues during the prolonged asymptomatic incubation period. Instruments exposed to infected tissues of preclinically infected individuals during medical or surgical procedures represent a potential risk of iatrogenic transmission of vCJD prions. We assessed the frequency of contamination with lymphoid tissue of single-use laryngoscope blades used for tracheal intubation for general anaesthesia. Using a cyto-centrifugation technique, lymphocytes were detected from 30% of laryngoscope blades

Review of Sterilization Techniques for Medical and Personal Protective Equipment Contaminated With SARS-CoV-2.

The outbreak of the novel coronavirus disease, COVID-19 turned into a global pandemic in March 2020. During these unprecedented times, there is an increased demand in medical and personal protective equipment (PPE). Since the supplies may take a long time to meet the global demand, reusing PPEs will help health care workers in their response to the COVID-19 pandemic. To ensure the safety and well-being of the medical first responders, PPE needs to be sterilized before reuse. In this review, we examine various sterilization techniques that can be used to sterilize PPEs and point out its limitat

Validation of "Personal Protective Equipment Conservation Strategies Tool" to Predict Consumption of N95s, Facemasks, and Gowns During Pandemic-Related Shortages.

We sought to prospectively validate a model to predict the consumption of personal protective equipment in a pediatric emergency department during the COVID-19 pandemic. We developed the Personal Protective Equipment Conservation Strategies Tool, a Monte Carlo simulation model with input parameters defined by members of our emergency department personal protective equipment task force. Inputs include different conservation strategies that reflect dynamic reuse policies. Over the course of 4 consecutive weeks in April and May 2020, we used the model to predict the consumption of N95 respirators

Perspectives on disposable and reusable surgical materials in laparoscopic surgery: a global survey amongst surgeons.

BACKGROUND: Surgical practices generate substantial waste due to disposable materials. Reusable alternatives have the potential to substantially reduce emissions, yet their implementation has been slow. This study investigates reasons for this by examining surgeons' perspectives on using disposable and reusable materials during laparoscopic cholecystectomy (LC). MATERIALS AND METHODS: A survey, containing both quantitative and qualitative questions, targeted surgeons' perceptions of sustainable healthcare in general and reusable versus disposable instruments (trocars, clip applicators, drapes

Hollow nanoneedle array and its utilization for repeated administration of biomolecules to the same cells.

We present a novel hollow nanoneedle array (NNA) device capable of simultaneously delivering diverse cargo into a group of cells in a culture over prolonged periods. The silica needles are fed by a common reservoir whose content can be replenished and modified in real time while maintaining contact with the same cells. The NNA, albeit its submicrometer features, is fabricated in a silicon-on-insulator wafer using conventional, large scale, silicon technology. 3T3-NIH fibroblast cells and HEK293 human embryonic kidney cells are shown to grow and proliferate successfully on the NNAs.
