BACKGROUND: Invasive cardiopulmonary exercise testing (iCPET) combines traditional cardiopulmonary exercise testing with invasive hemodynamic measurements to assess exercise intolerance, which can be caused by preload insufficiency (PI), characterized by low ventricular filling pressures and reduced cardiac output during exertion. We hypothesize that plasma catecholamine levels at rest and during exercise correlate with hemodynamic parameters in PI.
METHODS: We included adult patients who underwent iCPET for exercise intolerance and had plasma catecholamines measured at rest and peak exercise

BACKGROUND: Pulmonary hypertension (PH) is a frequent complication in COPD and it is associated with decreased exercise capacity and poor prognosis. We hypothesized that even in COPD patients without significant PH at rest, abnormal pulmonary hemodynamics during exercise affect exercise capacity.
METHODS: Consecutive COPD patients with clinically indicated right heart catheterization and resting mean pulmonary arterial pressure (mPAP) < 25 mmHg and age- and sex-matched controls with the same limits of pulmonary hemodynamics but no chronic lung disease who underwent clinical work-up including

Obesity is an independent risk factor of cardiovascular diseases. Epidemiological studies have shown that obesity induces the production of inflammatory factors and changes in cardiac hemodynamics, remodeling and function, leading to myocardial damage and heart diseases. The positive effect of exercise on the cardiovascular system has been widely confirmed, while the acute cardiovascular stress caused by exercise cannot be ignored. Compared with the general population, obese people were more prone to arrhythmia and have a higher risk of cardiovascular events during exercise, due to their abnor

PURPOSE: This review recalls the principles developed over a century to describe trans-capillary fluid exchanges concerning in particular the lung during exercise, a specific condition where dyspnea is a leading symptom, the question being whether this symptom simply relates to fatigue or also implies some degree of lung edema.
METHOD: Data from experimental models of lung edema are recalled aiming to: (1) describe how extravascular lung water is strictly controlled by "safety factors" in physiological conditions, (2) consider how waning of "safety factors" inevitably leads to development of

OBJECTIVES: Clinical guidelines for the non-surgical management of knee osteoarthritis (OA) recommend exercise and education. This study aimed to evaluate the extent to which accredited exercise physiologists (AEPs) deliver exercise and education for knee OA and how it aligns with clinical practice guidelines.
DESIGN: Cross-sectional survey.
METHOD: An online survey targeted to AEPs across Australia to understand the exercise and education content of their interventions for knee OA. Likert scale and multiple-choice questions were used to measure responses relating to exercise prescription. T

OBJECTIVE: Accumulating evidence suggests that, in addition to various psychosocial parameters, affective responses to exercise play an important role in subserving future exercise behavior. This study comprehensively evaluated whether acute exercise duration and recovery period influenced the relationship between moderate-intensity walking exercise and mood profile.
METHOD: We employed a randomized controlled cross-over trial. Participants completed two laboratory visits, separated by one-week. One of the visits involved a mood profile assessment with no exercise, while the other visit invol

Cardiovascular responses to exercise are exaggerated in hypertension. We previously demonstrated that this heightened cardiovascular response to exercise is mediated by an abnormal skeletal muscle exercise pressor reflex (EPR) with important contributions from its mechanically and chemically sensitive components. Exercise training attenuates exercise pressor reflex function in healthy subjects as well as in heart failure rats. However, whether exercise training has similar physiological benefits in hypertension remains to be elucidated. Thus we tested the hypothesis that the EPR overactivity m

BACKGROUND: There is a paucity of studies investigating exercise-induced muscle damage (EIMD) in females and only one in response to dance-type exercise. This study sought to firstly elucidate the physiological profile of EIMD following a dance-specific protocol, and second to compare the magnitude of damage to that experienced following a sport-specific protocol in physically active females.
METHODS: Twenty-nine female recreational dancers (19±1 years) were recruited. Participants completed either a dance-specific protocol (DPFT; N.=15) or sport-specific repeated sprint protocol (SSRS; N.=14

Patients affected by end-stage renal disease (ESRD) show quite lower physical activity and exercise capacity when compared to healthy individuals. In addition, a sedentary lifestyle is favoured by lack of a specific counseling on exercise implementation in the nephrology care setting. Increasing physical activity level should represent a goal for every dialysis patient care management. Three crucial elements of clinical care may contribute to sustain a hemodialysis exercise program: a) involvement of exercise professionals, b) real commitment of nephrologists and dialysis professionals, c) ind

STUDY DESIGN: Treadmill testing on a progressive incline of 11 wheelchair athletes.
OBJECTIVE: To determine if a novel treadmill exercise protocol which uses increments in inclination, rather than the standard increments in velocity, can be used to effectively determine maximum oxygen uptake VO2max for elite wheelchair athletes.
SETTING: Nottwil, Switzerland.
METHODS: Eleven elite wheelchair basketball players (29.3+/-6.3 years, 72.7+/-16.9 kg and 177+/-9.6 cm) performed an exercise protocol with increasing inclination on the treadmill. Eight players had a spinal cord injury (SCI), two had

Intense exercise during childhood and adolescence may result in primary amenorrhea and low peak bone mineral density (BMD). After puberty, exercise may result in secondary amenorrhea and bone loss. Higher BMD in amenorrheic athletes than amenorrheic sedentary persons suggests that exercise may partly offsets the effects of amenorrhea. To examine this possibility, we measured BMD (g/cm2) by dual x-ray absorptiometry in 32 ballet dancer and 23 healthy controls of comparable age with regular menstrual cycles, 34 pre-pubertal female gymnasts bone age 8.9 +/- 0.2 years and 37 girls matched by bone

We investigated arterial PCO2 (PaCO2) and pH (pHa) responses in ponies during 6-min periods of high-intensity treadmill exercise. Seven normal, seven carotid body-denervated (2 wk-4 yr) (CBD), and five chronic (1-2 yr) lung (hilar nerve)-denervated (HND) ponies were studied during three levels of constant load exercise (7 mph-11%, 7 mph-16%, and 7 mph-22% grade). Mean pHa for each group of ponies became alkaline in the first 60 s (between 7.45 and 7.52) (P less than 0.05) at all work loads. At 6 min pHa was at or above rest at 7 mph-11%, moderately acidic at 7 mph-16% (7.32-7.35), and markedly

Peripheral artery disease (PAD) is a significant medical concern that is highly prevalent, costly, and deadly. Additionally, patients with PAD have significant impairments in functional independence and health-related quality of life due to leg symptoms and ambulatory dysfunction. Exercise therapy is a primary treatment for patients with PAD, as ambulatory outcome measures improve following a program of exercise rehabilitation. This review describes the outcomes that improve with exercise, the potential mechanisms for improved leg symptoms, key exercise program considerations for training pati

Sarcopenia is a progressive age-related musculoskeletal disorder characterized by loss of skeletal muscle mass, strength, and physical function. Closely associated with aging and physical inactivity, it significantly compromises mobility, independence, and quality of life in older adults. Exercise is widely recognized as an effective non-pharmacological intervention for managing sarcopenia. However, the underlying molecular mechanisms remain incompletely elucidated. Among the emerging mediators, exerkines, physical exercise-induced signaling molecules, are secreted by multiple tissues, includi

Exercise-induced laryngeal obstruction is a multifactorial disorder involving reversible laryngeal narrowing during exercise, influenced by developmental, physiologic, psychological, and sport-specific factors. A high index of suspicion in youth with exertional dyspnea is needed, as clinical history most strongly supports the diagnosis along with pulmonary evaluation and laryngoscopy to rule out alternative or comorbid pathology. Management is multidisciplinary, with growing evidence for behavioral and physiologic interventions-including laryngeal control therapy and respiratory retraining-alo

The role of non-pharmacological management in MG is currently underdeveloped and underutilized. Exercise training has been shown to be both feasible and beneficial in improving function and strength in individuals with MG. Observational studies indicate that physical activity is associated with reduced perceived fatigue. Barriers to exercise persist in MG, and should be addressed by healthcare professionals. Maintaining or increasing muscle strength and endurance as well as cardiorespiratory fitness should be prioritized regardless of current level of function. In the general population, exerc

BACKGROUND: Patellar tendinopathy is a prevalent condition that commonly affects the tendon's origin, causing pain at the front of the knee. The main treatment for patellar tendinopathy involves different types of exercise (e.g. strengthening and stretching). The most common method of strengthening exercise is eccentric (lengthening) muscle loading. Strengthening exercises can be land-based or water-based, weight-bearing or non-weight-bearing, or both. Other treatments include surgery and glucocorticoid injections.
OBJECTIVES: To evaluate the benefits and harms of exercise for the treatment o
