Driver sleepiness and risk of motor vehicle crash injuries: a population-based case control study in Fiji (TRIP 12).

INTRODUCTION: Published studies investigating the role of driver sleepiness in road crashes in low and middle-income countries have largely focused on heavy vehicles. We investigated the contribution of driver sleepiness to four-wheel motor vehicle crashes in Fiji, a middle-income Pacific Island country. METHOD: The population-based case control study included 131 motor vehicles involved in crashes where at least one person died or was hospitalised (cases) and 752 motor vehicles identified in roadside surveys (controls). An interviewer-administered questionnaire completed by drivers or proxie

An examination of knowledge, awareness and practices towards breast cancer risk factors and early detection among females in the Fiji Islands.

BACKGROUND: This study aims to present a comprehensive examination of breast cancer awareness among women in Fiji. It was prompted by the alarmingly high rates of breast cancer in Fiji and the Pacific Islands. The goal was to enhance our understanding of women's knowledge, attitudes, and practices (KAP) regarding breast cancer in Fiji. METHODS: To achieve this, we conducted a cross-sectional KAP survey developed using the Breast Cancer Awareness Measures (BCAM) and surveyed over 250 women across Fiji who were aged 20 and above. RESULTS: The survey results revealed an overall low level of awa

Resilience measurement for environmental shocks and stressors: scale development and psychometric assessment for coastal urban informal settlements in Fiji and Indonesia.

BACKGROUND: Climate change and weather-related hazards, such as droughts and floods, pose substantial threats to the human health and well-being, especially for those in low-income households and informal settlements. Resilience, defined as the ability to cope, adapt, and recover, is critical for communities to manage these evolving threats. While there has been increased interest in ensuring that global public health and development programs contribute to resilience, the lack of valid and user-friendly resilience measurement tools limits the evidence base on the effectiveness of interventions

Black carbon and elemental characterization of PM2.5 in dense traffic areas in two cities in Fiji, a Small Island Developing State.

PM2.5 characterizations are essential in understanding its impact on the health of the exposed population. Sampled PM2.5 by Mani et al. (2020) was characterized to determine atmospheric metal concentration and inhalation health risk in Suva and Lautoka Cities, the only two cities in Fiji and one of the largest in the South Pacific Islands. Twenty-two elements (Al, As, Ba, Ca, Cd, Co, Cr, Cu, Fe, K, Mg, Mn, Mo, Na, Ni, P, Pb, S, Si, Sr, V, Zn) were analyzed using ICP-OES. Black Carbon (BC) sampling was also done at three different sites in Suva City, namely, Fiji National University Samabula In

Development and Evaluation of a Point-of-Care Test in a Low-Resource Setting with High Rates of Chlamydia trachomatis Urogenital Infections in Fiji.

Rapid and precise detection of Chlamydia trachomatis, the leading global cause of sexually transmitted infections (STI), at the point of care (POC) is required for treatment decisions to prevent transmission and sequelae, including pelvic inflammatory disease, ectopic pregnancy, tubal factor infertility, and preterm birth. We developed a rapid POC test (POCT), termed LH-POCT, which uses loop-mediated amplification (LAMP) of nucleic acids. We performed a head-to-head comparison with the Cepheid Xpert CT/NG assay using clinician-collected, deidentified paired vaginal samples from a parent study

Island-Wide Surveillance of Gastrointestinal Protozoan Infection on Fiji by Expanding Lymphatic Filariasis Transmission Assessment Surveys as an Access Platform.

As part of lymphatic filariasis (LF) transmission assessment surveys (TAS) on Fiji, an island-wide assessment of gastrointestinal protozoan infection was performed by inspection of a concomitant stool sample collection to investigate the distribution of parasitic protozoa. All grade 1 and 2 students of 69 schools on the two main islands were targeted in two phases (one in the Western Division and the other in the Central and Northern Divisions, except Taveuni sub-Division of Northern), where fecal samples of 1,800 students were available for coproscopy using formalin-ether-acetate concentratio

Use of geographically weighted logistic regression to quantify spatial variation in the environmental and sociodemographic drivers of leptospirosis in Fiji: a modelling study.

BACKGROUND: Leptospirosis is a globally important zoonotic disease, with complex exposure pathways that depend on interactions between human beings, animals, and the environment. Major drivers of outbreaks include flooding, urbanisation, poverty, and agricultural intensification. The intensity of these drivers and their relative importance vary between geographical areas; however, non-spatial regression methods are incapable of capturing the spatial variations. This study aimed to explore the use of geographically weighted logistic regression (GWLR) to provide insights into the ecoepidemiology

Prevention of bacterial complications of scabies using mass drug administration: A population-based, before-after trial in Fiji, 2018-2020.

BACKGROUND: Scabies is an important predisposing factor of impetigo which can lead to serious bacterial complications. Ivermectin-based mass drug administration can substantially reduce scabies and impetigo prevalence in endemic settings, but the impact on serious bacterial complications is not known. METHODS: We conducted a before-after trial in the Northern Division of Fiji (population: 131,914) of mass drug administration for scabies control. Prospective surveillance was conducted from 2018 to 2020. Mass drug administration took place in 2019, involving two doses of oral ivermectin or topi

The structural drivers of violence against children in Fiji, Papua New Guinea, Solomon Islands, Tonga and Vanuatu: An exploratory review.

BACKGROUND: Pacific Island countries have the highest rates of violent discipline, child labour and bullying in the Asia Pacific region, yet there is limited research on the drivers of violence against children in these contexts. OBJECTIVE: To address this gap, we explored the drivers of violence against children in the four most populous Pacific Island countries, Fiji, Papua New Guinea, Solomon Islands and Vanuatu, as well as Tonga where Save the Children, who commissioned this review, planned to commence child protection programming. METHODS: We undertook an exploratory literature review i

COVID-19 pandemic and the management of type-2 diabetes mellitus among women in Fiji: concerns and context specific interventions.

INTRODUCTION: The COVID-19 preventive strategies had a profound impact on type-2 diabetes management, creating challenges for individuals who depended on routine in-person consultations with healthcare workers. Given that the pandemic did not affect diabetic women equally across countries worldwide, this research was conducted in Fiji to explore how women living with type-2 diabetes managed their condition during the COVID-19 pandemic. MATERIALS AND METHODS: This study used a qualitative approach from a descriptive phenomenological lens. A total of 45 women with type-2 diabetes were purposive

Predictive risk mapping of an environmentally-driven infectious disease using spatial Bayesian networks: A case study of leptospirosis in Fiji.

INTRODUCTION: Leptospirosis is a zoonotic disease responsible for over 1 million severe cases and 60,000 deaths annually. The wide range of animal hosts and complex environmental drivers of transmission make targeted interventions challenging, particularly when restricted to regression-based analyses which have limited ability to deal with complexity. In Fiji, important environmental and socio-demographic factors include living in rural areas, poverty, and livestock exposure. This study aims to examine drivers of transmission under different scenarios of environmental and livestock exposures.

The Epidemiology of Scabies and Impetigo in Relation to Demographic and Residential Characteristics: Baseline Findings from the Skin Health Intervention Fiji Trial.

Scabies and associated impetigo are under-recognized causes of morbidity in many developing countries. To strengthen the evidence base for scabies control we undertook a trial of mass treatment for scabies. We report on the occurrence and predictors of scabies and impetigo in participants at baseline. Participants were recruited in six island communities and were examined for the presence of scabies and impetigo. In addition to descriptive analyses, logistic regression models were fit to assess the association between demographic variables and outcome of interest. The study enrolled 2051 parti

Typhoid fever: hurdles to adequate hand washing for disease prevention among the population of a peri-urban informal settlement in Fiji.

PROBLEM/CONTEXT: The Pacific island nation of Fiji Islands has high rates of endemic typhoid fever which is difficult to diagnose and often underreported. However, the majority of cases are preventable through use of safe water; adequate sanitation; vaccination; and, most sustainable of all, simple hygienic behaviour, such as hand washing with soap (HWWS). Despite many attempts by public health authorities, little progress has been made in the area of environmental adaptation and behaviour change. ACTION: To explore perceptions of typhoid fever risk among urban squatters and behavioural deter

Pilot study of nurse-led rheumatic heart disease echocardiography screening in Fiji--a novel approach in a resource-poor setting.

We designed a pilot study of a training module for nurses to perform rheumatic heart disease echocardiography screening in a resource-poor setting. The aim was to determine whether nurses given brief, focused, basic training in echocardiography could follow an algorithm to potentially identify cases of rheumatic heart disease requiring clinical referral, by undertaking basic two-dimensional and colour Doppler scans. Training consisted of a week-long workshop, followed by 2 weeks of supervised field experience. The nurses' skills were tested on a blinded cohort of 50 children, and the results w

Prevention of young infant infections using oral azithromycin in labour in Fiji (Bulabula MaPei): study protocol of a randomised control trial.

INTRODUCTION: Infections are a leading cause of neonatal mortality globally and can be transmitted from mother-to-child vertically or horizontally. Fiji has higher rates of serious neonatal infections and infant skin and soft tissue infections (SSTIs) than high-income countries. Research from the Gambia found that a single dose of oral azithromycin in labour decreased bacterial carriage and infections in mothers and infants, particularly infant skin infections. The Bulabula MaPei clinical trial evaluates the safety and efficacy of a single dose of azithromycin in labour in reducing the inciden
