The role for prophylaxis inferior vena cava filters in patients undergoing bariatric surgery: replacing anecdote with evidence.

The number of patients choosing surgical alternatives for weight reduction continues to increase. Despite common thromboembolic preventive methods, which include perioperative subcutaneous heparin injections, early mobilization, and sequential compression devices, postoperative deep vein thrombosis/pulmonary embolism remains a devastating complication after bariatric surgery. The role prophylactic inferior vena cava (IVC) filters may play in bariatric surgery remains controversial, and this article aims to address the risks and benefits of prophylactic IVC filters in high-risk bariatric patien

Current indications for preoperative inferior vena cava filter insertion in patients undergoing surgery for morbid obesity.

BACKGROUND: Pulmonary embolus is a potentially lethal complication in patients undergoing surgery for morbid obesity. In a select group of patients at high risk for venous thromboembolic events (VTE), we have chosen to prophylactically insert inferior vena cava filters via a jugular percutaneous approach. We propose guidelines for preoperative insertion of inferior vena cava filters in patients with clinically significant obesity. METHODS: All patients who underwent preoperative insertion of inferior vena cava (IVC) filters as prophylaxis for pulmonary emboli were reviewed. Data regarding bod

Successful retrieval of a vena cava filter in a patient with duodenal perforation by open surgery: a case report and literature review.

BACKGROUND: Vena cava filter placement represents a safe and effective intervention to prevent the potential occurrence of pulmonary embolism caused by the dislodgement of deep vein thrombosis. However, prolonged indwelling can lead to some severe and life-threatening complications such as duodenal perforation. CASE PRESENTATION: In 2023, a 60-year-old woman was admitted to our hospital with a three-month history of intermittent lower abdominal and back pain. She underwent a vena cava filter implantation in 2016, to prevent pulmonary embolism. However, she didn’t receive a systematic follow-up

Inferior vena cava filters for prevention of venous thromboembolism in obese patients undergoing bariatric surgery: a systematic review.

OBJECTIVE: The use of inferior vena cava (IVC) filters for prevention of venous thromboembolism (VTE) in bariatric surgery is a contentious issue. We aim to review the evidence for the use of IVC filters in bariatric surgical patients, describe trends in practice, and discuss challenges in developing evidence-based guidelines. BACKGROUND: The incidence of VTE in modern bariatric procedures with traditional methods of thromboprophylaxis, such as sequential calf compression devices and perioperative low molecular weight heparin, is approximately 2%. METHODS: A systematic review of the literatu

Implantation of 2 trabecular microbypass stents in a patient with primary open-angle glaucoma refractory to previous glaucoma-filtering surgeries.

UNLABELLED: A 77-year-old woman with primary open-angle glaucoma was referred for surgical evaluation after failing to achieve target intraocular pressure (IOP) despite previous combined phacotrabeculectomy and Ahmed valve implantations. The IOP prior to surgery was 28 mm Hg on topical dorzolamide-timolol, latanoprost, and brimonidine. Trabecular bypass surgery with implantation of 2 iStents was performed. The IOP decreased by 11 mm Hg to 17 mm Hg after surgery and has remained stable for 2 years. This is the first published case reporting this magnitude of IOP reduction sustained for more tha

Immediate surgery for acute internal carotid artery dissection and thrombosis during filter deployment prior to stenting: a case report.

Carotid artery stenting (CAS) is a validated option in the treatment of selected extracranial carotid artery stenosis. Carotid artery dissection during CAS is a rare but potentially devastating complication. We report a case of acute dissection and thrombosis of the left internal carotid artery during filter tip wire engaging maneuvers, complicated by intraoperative complete blindness of the left eye. Immediate conversion to carotid endarterectomy was performed under general anesthesia with electroencephalographic monitoring. The patient was discharged home symptomless and remains asymptomatic
