Professional competencies in geriatric nursing for geriatric nurses: a latent profile analysis.

BACKGROUND: As the global population continues to age, social realities such as advanced age, disability and living alone are coming to the fore, and the demand for medical care and health services for the elderly is increasing dramatically, especially in geriatrics. Given the important role geriatric nurses play in the diagnosis and treatment of diseases and rehabilitation of elderly patients, and due to the uniqueness and complexity of geriatric work, this requires geriatric nurses not only to have the competencies that are available in general nursing, but also to ensure that they have suff

Phenotype and prognostic factors in geriatric and non-geriatric patients with transthyretin cardiomyopathy.

AIMS: Transthyretin cardiac amyloidosis (ATTR-CM) may be an underestimated cause of heart failure among geriatric patients and represent a unique phenotype and prognostic profile. METHODS AND RESULTS: This retrospective, observational, cohort study characterizes cardiac and extracardiac disorders at diagnosis and assesses prognosis among ATTR-CM patients based on age (geriatric vs. non-geriatric) and amyloidosis subtype (wild type, ATTRwt and hereditary, ATTRv). In total, 943 patients with ATTR-CM were included, of which 306 had ATTRv and 637 had ATTRwt. Among these, 331 (35.1%) were non-geri

Pediatric and Geriatric Considerations.

Pediatric and geriatric traumatic brain injury (TBI) patients require age-specific considerations. This must occur at every juncture: preoperatively assessing frailty, intraoperatively with specific surgical considerations, post-operatively, during family discussions, and throughout the rehabilitation process. Both pediatric patients and patients over the age of 65 have an increased risk of infection as well as an increased risk of syndrome of the trephined. Long-term outcomes after pediatric and geriatric TBI (particularly in repetitive injury), the use of biomarkers for prognosis, optimal re

[Multimorbidity in Geriatric Oncology].

The incidence of many cancers increases with age. Because of the frequent presence of comorbidity, the prognosis is usually significantly worse in these cases. Furthermore, there is little evidence on how cancer treatments can be adapted in the presence of comorbidity. This article discusses key aspects of comorbidity in older cancer patients and provides practical management recommendations.

Hyponatremia in geriatric patients.

Hyponatremia is the most frequent electrolyte imbalance in geriatric medicine. Causes of hyponatremia were retrospectively analyzed in all in-patients treated in 2016 (N = 2267, 1564 women, 703 men, mean age ± standard deviation 81.9 ± 7.6 years). Any form of hyponatremia on admission, during the stay or on discharge was noted in 308 patients (13.6%, 231 women, 77 men; mean age ± standard deviation 83.1 ± 7.3 years, p = 0.009 vs. age of all patients). Women had a higher probability of developing hyponatremia compared to men (p = 0.019), 131 patients were hypovolemic, and dyspnea as an indicato

Considerations in geriatric dermatopathology.

This review provides an overview of considerations in geriatric dermatopathology. The nuances of specimen collection and reporting in this population, including the importance of obtaining adequate tissue, providing detailed clinical information and ethical considerations surrounding specimen collection will be discussed. The histopathologic changes associated with aging and the morphologic features of common lesions related to photoaging are outlined followed by a discussion of common interpretation pitfalls, specifically entities at risk of overinterpretation and those related to hyper- and

Anemia of geriatric patients.

Anemia is a common finding in the elderly. Approximately 10 percent of the elderly suffers from anemia. Anemia per se is an independent factor of mortality in older patients regardless its cause. Frailty is also frequent in geriatric patients. That means that there is a decreased reserve capacity to react to different stress factors including anemia. The frequent presence of heart failure and also impaired cerebrovascular circulation makes more difficult to tolerate anemia in older age. Anemia is a symptom, finding and treating the underlying cause is also important. Treatment always depends o

Geriatric Palliative Care.

In the geriatric age group, few studies demonstrate the efficacy of aggressive treatment. Often, a more palliative approach is wanted; such an approach can lead to better quality of life and even a longer life. The author discusses the limits of medical interventions in the elderly, the paucity of data, and the benefits of palliation in certain medical conditions, including dementia, Parkinson, depression, arthritis, congestive heart failure, and pneumonia. The role of frailty is addressed; specific goals of palliative care are delineated, such as reduction of polypharmacy, fall prevention, pa

The evolution of geriatric neurology.

The founders of neurology in the late 19th century and early 20th century laid the foundation for modern geriatric neurology by detailed observations, in their patients, of the clinical expressions of aging of the nervous system. Further advances in clinical manifestations of aging and more detailed examination of disease states in the elderly accelerated in the last quarter of the 20th century. Basic, translational, and clinical research support of geriatric neurology studies expanded and advanced during this period and into the 21st century. The size and sophistication of this movement was s

Lithopedion in a Geriatric Patient.

Lithopedion (lithos = rock and paidion = child) is a rare condition that only occurs in 1.5 to 1.8% of extrauterine pregnancies and in 0.00045% of all pregnancies. It consists of an ectopic pregnancy in which the fetus dies but cannot be reabsorbed by the mother's body, which then coats it in a calcium-rich substance. We present the case of a 77-year-old woman with an incidental diagnosis of a lithopedion, which had been retained in her left pelvis for presumably 40 years.

[Urosepsis in Geriatric Patients].

Due to the demographic shift, increasing numbers of geriatric patients are admitted to acute care hospitals of all levels of care. This means that special challenges must be met in the medical care and management of these patients.Immunosenescence and multimorbidity make elderly patients vulnerable to infectious diseases. Urinary tract infections range from "simple" cystitis to pyelonephritis and urosepsis and, at 25%, are the second most common form of infection in geriatric patients. It is often difficult to make a diagnosis because typical symptoms do not always occur. Urosepsis, a hyperact

Vasculitis in the geriatric population.

Vasculitis represents an uncommon but important group of disease entities that may affect older patients. The most common vasculitic disease in humans in giant cell arteritis, a disease process seen almost exclusively in patients older than 50 years in age. Vasculitic disease in geriatric patients presents unique challenges with regard to diagnosis and treatment. A thorough understanding of the vasculitic disease entities that may affect older patients as well as their diagnosis and management is essential in minimizing disease and treatment-related morbidity and mortality.

The elderly represent the fastest-growing segment of the population. As the country ages, the number of elderly patients who present in extremis will undoubtedly increase. Emergency physicians must be cognizant of the fact that age alone is a poor predictor of outcome in the critically ill elderly patient. Premorbid status, including previous level of functioning and pre-existing disease, is more important than chronologic age in predicting outcome, guiding assessment, and deciding on therapy. Knowledge and consideration of the geriatric ABCs includes a fundamental understanding that loss of p
