We herein report two cases of obstructive pyelonephritis with hyperammonemia causing impaired consciousness. One patient was a 79-year-old female. She was admitted for impaired consciousness and hematuria. Computed tomography (CT) revealed left hydronephrosis and hydroureter indicating obstructive pyelonephritis and her ammonia in the blood was a high level of 208 μg/dl. Her consciousness did not change immediately but improved 10 days after ureteral catheterization. The other patient was an 88-year-old female. She was also admitted for impaired consciousness and hematuria. CT revealed right h

UNLABELLED: Non-alcoholic or metabolic-associated fatty liver disease (NAFLD/MAFLD) is a non-infectious pandemic of the 21st century, affecting about a third of the adult population worldwide. According to epidemiological studies in the Russian Federation, the prevalence of NAFLD based on the Fatty Liver Index (FLI) is 39.2%. With MAFLD, the activity of ornithine (urea) cycle enzymes may decrease, potentially leading to hyperammonigistia (excessive accumulation of ammonia in tissues) and hyperammonemia. Both conditions have multiple negative effects on different cell and tissue types. The aim

INTRODUCTION: Valproic acid (VPA)-induced hyperammonemia poses several clinical challenges in psychiatric medicine. The reported incidence of this adverse effect varies widely across the literature. Furthermore, practitioners treat hyperammonemia in asymptomatic patients although studies suggest this practice is unnecessary. The purpose of this study is to evaluate if patients with VPA-induced hyperammonemia are appropriately identified for treatment based on their symptom presentation as well as determine the most efficacious treatment approach for VPA-induced hyperammonemia.
METHODS: This s

Ammonia is a potent neurotoxin that is detoxified mainly by the urea cycle in the liver. Hyperammonemia is a common complication of a wide variety of both inherited and acquired liver diseases. If not treated early and thoroughly, it results in encephalopathy and death. Here, we found that hepatic autophagy is critically involved in systemic ammonia homeostasis by providing key urea-cycle intermediates and ATP. Hepatic autophagy is triggered in vivo by hyperammonemia through an α-ketoglutarate-dependent inhibition of the mammalian target of rapamycin complex 1, and deficiency of autophagy impa

The clinical manifestations of hyperammonemia are usually easily identifiable to the clinician when associated with liver disease and lead to prompt diagnosis and treatment. However, hyperammonemia-induced encephalopathy is rare in adults in the absence of overt liver disease, thus diagnosis is often delayed or missed leading to potentially life threatening complications. Without proper treatment, such patients can decompensate rapidly with poor outcomes including seizures, coma, and death. Early assessment of plasma ammonia levels in patients with normal hepatic function and characteristic sy

OBJECTIVE: To determine ultrasonographic abnormalities in dogs with hyperammonemia.
DESIGN: Retrospective study.
ANIMALS: 90 client-owned dogs with hyperammonemia.
PROCEDURE: Ultrasonography of the abdominal vessels and organs was performed in a systematic way. Dogs in which the ultrasonographic diagnosis was a congenital portosystemic shunt were included only if they underwent laparotomy or necropsy. Dogs in which the abdominal vasculature appeared normal and dogs in which the ultrasonographic diagnosis was acquired portosystemic shunts and portal hypertension were included only if liver b

Idiopathic hyperammonemia (IHA) is a rare but serious complication of stem cell transplantation. We report a patient with immunoblastic lymphoadenopathy-like T-cell lymphoma who developed IHA 10 days after allogeneic bone marrow transplantation from an HLA-matched unrelated donor. Despite intensive supportive care, the patient died due to this metabolic disorder two days later. Being mindful of the possibility of IHA in patients who develop confusion and respiratory alkalosis soon after stem cell transplantation would allow earlier treatment and might improve the chance of survival.

1. The influence of hyperammonemia (produced by the continuous intraperitoneal infusion of ammonium acetate for 6 days) on stress-induced gastric ulcer formation was investigated in conscious rats. 2. Continuous ammonium acetate infusion significantly reduced stress-induced gastric ulceration concomitant with an increase in gastric blood flow, as determined using radioactive microspheres. The serum levels of L-arginine as well as nitrite and nitrate (oxidative byproducts of nitric oxide) were increased by ammonium acetate infusion. 3. Prior administration of N omega-nitro-L-arginine methyl est

The neuropathological consequences of acute exposure to the neurotoxicants ammonia and octanoic acid were investigated with the isolated, perfused canine brain preparation. After 1 h of combined hyperammonemia and octanoic acidemia, ultrastructural changes were apparent in all brain regions examined. The cell bodies of neurons were the primary sites of these alterations. Neuronal mitochondria were distended, and the lamellae of the mitochondrial cristae were separated. In some cases the lamellae had completely dispersed, leaving only matrix remnants. Mitochondria of adjacent astrocytes appeare

This is a report of a family with hyperammonemia in the newborn, very probably due to ornithine transcarbamylase deficiency. Typically the male infant died in the newborn period within 74 hours. In the female newborn it was possible to provide a normal development with protein restricted diet. In mother's line there are unexplained death's of infants. The examination of amino acids and organic acids excluded other disturbances. The determination of orotic acid in the urines and oral loading with protein indicated to OTC-deficiency. The RDS-symptomatic and pulmonary bleeding in the newborns wit

OBJECTIVE: Valproic acid, frequently prescribed for neurological and psychiatric disorders, can cause hyperammonemia (HA). This retrospective study aimed to investigate the association among the basic characteristics, comorbidities, co-medications, and risk of HA in patients receiving valproic acid.
MATERIALS AND METHODS: We compared groups with and without HA using data collected from the medical records of adults undergoing valproic acid monitoring between January 1, 2019, and December 31, 2021. We conducted a multivariable logistic regression analysis to explore the risk factors for HA and

OBJECTIVES: Argininosuccinate synthase 1 (ASS1) is a key rate-limiting enzyme in the urea cycle and is highly conserved at the protein level. Its catalytic activity directly determines the efficiency of ammonia clearance. Hyperammonemia is a clinical syndrome caused by abnormally elevated blood ammonia levels and is characterized mainly by central nervous system dysfunction, with neurotoxicity and a high mortality rate. When liver function is impaired or key enzymes of the urea cycle are deficient, ammonia clearance is reduced, leading to ammonia accumulation and passage across the blood-brain

RATIONALE: Hyperammonemia in patients receiving hemodialysis is uncommon but poses a significant clinical challenge due to the effective clearance of ammonia by dialysis, which can obscure the underlying cause. Recognizing atypical etiologies is crucial for appropriate management.
PRESENTING CONCERNS OF THE PATIENT: A 59-year-old man being treated with hemodialysis presented with altered level of consciousness and recurrent hyperammonemia. Despite previous episodes of hyperammonemia, the etiology of his intermittently elevated ammonia remained unclear and was initially attributed to his kidne

BACKGROUND: Hyperammonemia can occur after acute overdose or chronic use of valproic acid (VPA). Although VPA-related hyperammonemic encephalopathy (VHE) is a rare complication of VPA therapy, early recognition of VHE and identifying its risk factors are important because VHE can lead to loss of consciousness and increased seizure frequency.
PURPOSE: The purpose of our study is to evaluate the risk factors of hyperammonemia in epilepsy patients during treatment with VPA therapy.
METHODS: We reviewed the medical records of 1084 adult patients with epilepsy and enrolled 116 patients with VPA t

Neuroendocrine neoplasms (NENs) are rare, usually slow-growing tumors, often presenting with extensive liver metastases. Hyperammonemia due to insufficient hepatic clearance has been described in NEN cases; however, no systematic evaluation of risk factors and outcomes of NEN-associated hyperammonemia exists so far. This case report and retrospective review of NEN patients developing hyperammonemia from the years 2000 to 2020 at the Erasmus Medical Center in Rotterdam, the Netherlands, aimed to describe these patients and determine prognostic factors to improve evaluation and treatment. Hyperammonemia of newborns should be treated promptly, and the outcome depends on the rapid elimination of excessive plasma ammonia. We encountered a case of transient hyperammonemia in an extremely low-birthweight infant whose plasma ammonia decreased sufficiently after continuous hemodialysis therapy. It seems that continuous hemodialysis therapy using the peripheral artery and umbilical vein is useful for hyperammonemia of extremely low-birthweight infants; however, there are several problems to consider due to the immaturity of these infants.

CONTEXT: Idiopathic hyperammonemia is characterized by elevated serum ammonia associated with neurological deterioration of no other obvious etiology associated with relatively normal liver function tests and normal amino-acid levels.
CASE REPORT: We report a case of a 32-year-old woman who presented with acute mental status changes with a pelvic abscess approximately a year following her total pancreatectomy and islet cell transplant surgery. Her ammonia level was elevated to 425 µg/dL. Traditional ammonia-reducing therapies were initiated, but proved ineffective. Metabolic, pharmacologic, m
