Cross-cultural adaptation and psychometric validation of the Kannada version of the Extended Effort Assessment Scale (EEAS-K) in adults with hearing loss.

INTRODUCTION: The Extended Effort Assessment Scale (EEAS) is a recently developed questionnaire for measuring listening effort, but a validated version for Kannada speakers is lacking. This study aimed to translate, culturally adapt, and psychometrically validate the EEAS for the Kannada-speaking adults with hearing loss. METHOD: The EEAS was translated into Kannada (EEAS-K) following rigorous forward and back translation, expert committee review and cognitive debriefing. The final EEAS-K was then field-tested with 100 native Kannada-speaking adults with hearing loss (predominantly non-hearin

What Can We Learn about Auditory Processing from Adult Hearing Questionnaires?

BACKGROUND: Questionnaires addressing auditory disability may identify and quantify specific symptoms in adult patients with listening difficulties. PURPOSE: (1) To assess validity of the Speech, Spatial, and Qualities of Hearing Scale (SSQ), the (Modified) Amsterdam Inventory for Auditory Disability (mAIAD), and the Hyperacusis Questionnaire (HYP) in adult patients experiencing listening difficulties in the presence of a normal audiogram. (2) To examine which individual questionnaire items give the worse scores in clinical participants with an auditory processing disorder (APD). RESEARCH DE

A Novel Artificial-Intelligence-Based Reverberation-Reduction Algorithm for Cochlear Implants Enhances Speech Intelligibility and User Experience.

OBJECTIVES: Cochlear implantation is the standard of care for severe-to-profound hearing loss in the Netherlands. Cochlear implants (CIs) generally perform well in quiet conditions, but speech understanding in an environment with reverberations remains difficult. This study aimed to minimize the amount of reverberation present in a speech signal by using novel artificial-intelligence-based algorithms created for use in CIs. DESIGN: A prospective crossover study was performed, which included 15 CI users, with each participant being their own control. Two versions of the algorithm were tested:

Investigating the Neural Correlates of the Attention Training Technique Using a Novel fMRI Paradigm for Measuring Attentional Control.

Attentional control (AC) plays a causal role in various mental disorders and, within the metacognitive model, contributes to maladaptive repetitive cognitive processes such as rumination and worry. The Attention Training Technique (ATT), an auditory psychotherapeutic intervention, improves AC and is associated with the efficiency of large-scale fronto-parietal control networks (FPN). This study investigates the neural correlates of ATT by applying a newly tailored fMRI paradigm, focusing on FPN engagement and its relationship with AC. We adapted ATT to examine neural responses during ATT compa

Objective and Subjective Benefit of Direct-to-Consumer Hearing Devices in Middle-Aged Adults.

PURPOSE: The purpose of this project was to assess subjective and objective benefit provided by several direct-to-consumer hearing devices for middle-aged adults. The primary goal of this study was to determine the extent to which this type of device can yield benefit when users are listening in a range of acoustic conditions, rather than to compare benefit among devices. METHOD: Participants (M age = 58 years, n = 40) completed a speech perception task with and without one of four direct-to-consumer devices. Stimuli were presented with three types of maskers (steady-state noise, modulated no

Effects of hearing loss on heart rate variability and skin conductance measured during sentence recognition in noise.

OBJECTIVE: The purpose of the study was to determine the effects of hearing loss and noise on (1) two autonomic nervous system measures associated with stress (skin conductance and heart rate variability) and on (2) subjective ratings of workload/stress. The authors hypothesized that hearing loss would increase psychophysiological and subjective reactivity to noise during speech recognition tasks. Both psychophysiological and subjective indicators of workload/stress were expected to increase with a reduction in signal-to-noise ratio. DESIGN: Sentence recognition in the presence of babble nois

Text Captioning Buffers Against the Effects of Background Noise and Hearing Loss on Memory for Speech.

OBJECTIVE: Everyday speech understanding frequently occurs in perceptually demanding environments, for example, due to background noise and normal age-related hearing loss. The resulting degraded speech signals increase listening effort, which gives rise to negative downstream effects on subsequent memory and comprehension, even when speech is intelligible. In two experiments, we explored whether the presentation of realistic assistive text captioned speech offsets the negative effects of background noise and hearing impairment on multiple measures of speech memory. DESIGN: In Experiment 1, y

Comparing the Effects of Sensory Tricks on Voice Symptoms in Patients With Laryngeal Dystonia and Essential Vocal Tremor.

PURPOSE: This pilot study systematically compared voice symptomatology across varied sensory trick conditions in those with laryngeal dystonia (LD), those with essential vocal tremor (EVT), and vocally normal controls (NCs). Sensory tricks are considered signature characteristics of dystonia and were hypothesized to reduce voice symptoms in those with LD compared to EVT and NC groups. METHOD: Five participants from each group (LD, EVT, and NC) completed speech recordings under control and sensory trick conditions (delayed auditory feedback [DAF], vibrotactile stimulation [VTS], and nasoendosc

Sex-specific differences in self-reported speech, spatial, and qualities of hearing abilities.

OBJECTIVE: To investigate sex-specific effects in self-reported auditory abilities using an adapted version of the Speech, Spatial and Qualities (SSQ) questionnaire. DESIGN AND STUDY SAMPLE: Three mixed-model analyses of variance were performed, one for each questionnaire section, using rationalised arcsine unit-transformed scores. Fifty-one females and 39 males with normal or near-normal hearing. RESULTS: Females gave significantly higher (better) scores for: (i) four speech questions, indicating less difficulty following two targets or a conversation when many people are talking, and conve

Intelligibility and detectability of speech measured diotically and dichotically in groups of listeners with, at most, "slight" hearing loss.

The purpose of this investigation was to determine if a group of listeners having thresholds at 4 kHz exceeding 7.5 dB HL, and no more than "slight" hearing loss, would exhibit degradations in performance when "target" stimuli were masked tokens of speech. Intelligibility thresholds and detection thresholds were measured separately for speech masked by flat-spectrum noise or speech-shaped noise. Both NoSo and NoSπ configurations were employed. Consistent with findings of earlier investigations, when maskers and speech tokens were broadband, NoSo and NoSπ detection thresholds were substantially

Coordination between posture and phonation in vocal effort behavior.

BACKGROUND: Postural correlates of vocal effort are rarely described in the literature, while they are extensively dealt with in speech therapy. OBJECTIVES: This study aims at determining whether body movement is a side effect of vocal effort or an integral part of communication effort behavior. The answer to this question is mainly based on correlations between posture and phonation. METHOD: Twenty healthy subjects participated in this study. They had to communicate with a listener under 3 conditions requiring different levels of vocal effort. RESULTS: The vocal parameters increased and co

Emotional profiles across physical activity: exploring interoception, emotional regulation and attitudes toward physical effort.

UNLABELLED: While the health impacts of physical activity are well-established, the contribution of internal regulatory processes to these behaviors remains unclear. This study adopts an integrative perspective to examine how interoceptive sensibility and emotion regulation jointly differentiate physical activity levels and predict attitudes toward effort. METHOD: 1073 adults (557 males, mean age = 35.4) completed the Beck Depression Inventory, State-Trait Anxiety Inventory, Cognitive Emotion Regulation Questionnaire, Multidimensional Assessment of Interoceptive Awareness, and Physical Effort

The impact of face masks on metacognition in sign language is mediated by proficiency.

Face masks can impact processing a narrative in sign language, affecting several metacognitive dimensions of understanding (i.e., perceived effort, confidence and feeling of understanding). However, to what extent this is modulated by sign language proficiency remains an open question. Thirty deaf adults were administered a narrative in Italian Sign Language (LIS), signed in three different visual conditions: no mask, transparent mask, opaque mask. In addition, they completed a Sentence Reproduction Task in LIS (LIS-SRT) to measure their sign language proficiency. Results showed that some of t

Disfluencies reduce the effect of uh … word surprisal during narrative comprehension.

Disfluencies in speech frequently occur before the production of longer and more complex speech content. Listeners are thought to use the distribution of disfluencies in the comprehension of speech to inform their predictions. Here, we investigated whether the presence of disfluencies in speech affects word processing also in naturalistic listening conditions. Participants (n = 36) listened to the spoken recall of the events of a television series while undergoing fMRI. We modelled word processing effort using parametric modulations for word length, frequency, entropy, as well as surprisal and

Lidocaine block of the recurrent laryngeal nerve in adductor spasmodic dysphonia: a multidimensional assessment.

OBJECTIVES/HYPOTHESIS: Lidocaine block of the recurrent laryngeal nerve (RLN) has been reported as a procedure for surgical selection of patients with adductor spasmodic dysphonia (ADSD). However, its effects on phonation have not been rigorously assessed in a prospective fashion using strict entry criteria and multiple measures of phonatory function. This investigation assessed the phonatory effects of RLN lidocaine block in ADSD to explore its potential as a diagnostic tool. STUDY DESIGN: Single group, pre/postexperimental trial. METHODS: Twenty-one consecutive patients with suspected ADSD

Auditory-Perceptual Evaluation of Situationally-Bound Judgements of Listener Comfort for Postlaryngectomy Voice and Speech.

BACKGROUND: The influence of the listening environment in which communication occurs has been of longstanding interest related to communication disorders. It also has been posited that specific listening scenarios may differentially effect listener judgements of a speaker. In the case of those who have lost their larynx and normal voice due to cancer, a listener's assessment may carry a substantial penalty. This study was designed to determine if situationally-bound communication 'scenario' influences a listener's auditory-perceptual judgement of level of comfort for three modes of postlarynge

Spatial hearing disability after acoustic neuroma removal.

OBJECTIVES/HYPOTHESIS: Previous studies on hearing loss (HL) after acoustic neuroma removal concentrate mainly on pure-tone hearing results rather than hearing disability. Our objectives were to use the Speech, Spatial and Qualities of Hearing scale (SSQ), a comprehensively validated questionnaire, to characterize and quantify the auditory disabilities that patients experience with a profound unilateral HL after acoustic neuroma removal. STUDY DESIGN: Forty-four patients with profound unilateral HL after acoustic neuroma surgery completed the SSQ. Their findings were compared with those of a

Listening to Patient and Caregiver Voices to Anchor Global Cancer Control.

The experiences of those living with illness and those who care for them are central to disease control and prevention efforts. The history of public health is full of reminders of when consultations with patients and families helped develop meaningful interventions that were ultimately adopted by communities. Global cancer control, on the other hand, seems to increasingly take place in tertiary hospitals and centers where care is sought often at a late stage of cancer. This essay draws on personal experiences of the authors in working with cancer survivors in southern India and discusses how

Exploring boundary conditions of the listening comprehension-reading comprehension discrepancy index.

Conversations about the nature of dyslexia and how dyslexia impacts reading and listening comprehension get to the heart of classification and identification models of dyslexia. Recently, this conversation has been expanded to include efforts to estimate the prevalence of dyslexia in the population through the introduction of a discrepancy index of listening comprehension and reading comprehension. This discrepancy index was proposed to serve as a proxy for dyslexia when estimating its prevalence in the population. Individuals whose reading comprehension is considerably lower than their listen

The Intelligibility-Based Repeat-Recall Test: I. Bayesian-Guided Estimation of Multiple Speech Reception Thresholds.

OBJECTIVES: The intelligibility-based Repeat-Recall Test (i-RRT) seeks to compare secondary speech-in-noise (SiN) test outcomes, such as listening effort, against normative references measured at equal levels of SiN intelligibility. The present study evaluates the reliability, sensitivity, and accuracy of the easy speech reception threshold ("ezSRT") test, the first part of the i-RRT, a Bayesian-guided adaptive SiN test for quickly estimating a listeners' complete performance-intensity functions and deriving estimated SRTs. DESIGN: Single-blind mixed design. Twenty normal hearing and 19 heari
