Comparison of dose distribution with and without reflecting heterogeneous boron distribution using 18F-BPA positron emission tomography in boron neutron capture therapy.

This study investigated the dosimetric impact of implementing heterogeneous boron distribution into dose evaluations on tumors in BNCT. The study involved 27 patients who underwent 18F-BPA PET/CT scans. Dose evaluations were performed using various methods reflecting boron concentrations on CT images. The conventional dose evaluations, which reflected uniform boron concentration in blood of 25 ppm and a constant tumor-to-blood (T/B) ratio of 3.5, were compared with individual dose evaluations, which reflected the blood boron concentration and T/B ratio calculated from 18F-BPA in each patient.

Rational surface modification of gadolinium borate nanoparticles enhancing colloidal stability in physiological media for potential neutron capture therapy and magnetic resonance imaging.

Colloidal stability of nanomaterials in physiological media is an indispensable property for their biomedical applications. However, gadolinium borate (GdBO3) nanoparticles that hold promise as a theranostic agent for neutron capture therapy (NCT) and magnetic resonance imaging (MRI) of cancer tend to precipitate in phosphate buffered saline (PBS) owing to formation of insoluble gadolinium phosphate. To address this issue, in this work 10B-enriched GdBO3 nanoparticles were prepared and coated with mesoporous silica (mSiO2) of ~ 40 nm in thickness and subsequently grafted with hydrophilic polyg

Ln3TCAS2-polyethyleneimine supramolecular nanogels: a platform for neutron capture therapy and complementary magnetic resonance imaging (Ln = lanthanide, TCAS = thiacalix[4]arene-p-tetrasulfonate).

Supramolecular nanogels with high drug-loading capacity were prepared via electrostatic crosslinking of cationic polyethyleneimine (PEI) with hepta-anionic lanthanide complexes, Ln3TCAS2 (Ln = Tb, Gd; TCAS = thiacalix[4]arene-p-tetrasulfonate), which act as both structural crosslinkers and theranostic agents. The optimized nanogels exhibited a hydrodynamic diameter of 163 nm (suitable for endocytotic uptake), a highly positive ζ-potential (+50 mV), and an exceptionally high loading capacity of 45%. Notably, the nanogels demonstrated robust colloidal stability with a negligible complex leakage

Development of a practical and high-speed deep learning-based dose calculation model in boron neutron capture therapy for head and neck cancer.

BACKGROUND: In boron neutron capture therapy (BNCT), Monte Carlo (MC) dose calculations are commonly employed because of the complicated neutron reactions. However, MC dose calculations are generally time-consuming. Recently, deep learning (DL)-based dose prediction/calculation has attracted increasing attention; however, the applications of DL models in BNCT are limited and have not been investigated extensively. In addition, there are no practical DL models that can be employed in BNCT clinical practice. PURPOSE: We propose a practical DL model for head and neck cancers using a commercial t

Boron neutron capture therapy for recurrent pediatric sialoblastoma: the first clinical experience and dosimetric considerations for skeletal safety with 4-year follow-up.

Sialoblastoma is a rare, aggressive pediatric salivary gland tumor often refractory to conventional therapies, which carry significant risks of growth impairment in children. We report the 52-month follow-up of a 6-year-old girl with recurrent sialoblastoma treated with two sessions of boron neutron capture therapy (BNCT). She achieved a 23-month complete response after the first session and a sustained partial response following the second. Dosimetric analysis using a conservative α/β ratio of 2.0 Gy for late-responding tissues showed that the equivalent dose in 2-Gy fractions to the mandible

Fluorine-18 labeling PEGylated 6-boronotryptophan for PET scanning of mice for assessing the pharmacokinetics for boron neutron capture therapy of brain tumors.

Two tryptophan compound classes 5- and 6-borono PEGylated boronotryptophan derivatives have been prepared for assessing their aqueous solubility as formulation of injections for boron neutron capture therapy (BNCT). The PEGylation has improved their aqueous solubility thereby increasing their test concentration in 1 mM without suffering from toxicity. In-vitro uptake assay of PEGylated 5- and 6-boronotryptophan showed that the B-10 concentration can reach 15-50 ppm in U87 cell whereas the uptake in LN229 cell varies. Shorter PEG compound 6-boronotryptophanPEG200[18F] was obtained in 1.7 % radi

Boron Neutron Capture Therapy (BNCT) for Cutaneous Malignant Melanoma Using 10B-p-Boronophenylalanine (BPA) with Special Reference to the Radiobiological Basis and Clinical Results.

BNCT is a radiotherapeutic method for cancer treatment that uses tumor-targeting 10B-compounds. BNCT for cutaneous melanoma using BPA, a phenylalanine derivative, was first initiated by Mishima et al. in 1987. This article reviews the radiobiological basis of melanoma control and damage to normal tissues as well as the results of clinical studies. Experimental studies showed that the compound biological effectiveness (CBE) values of the 10B (n, α)7Li reaction for melanoma control ranged from 2.5 to 3.3. The CBE values of the 10B (n, α)7Li reaction for skin damage ranged from 2.4 to 3.7 with mo

Automated radiosynthesis of 2-[18F]BPA for PET-based planning of boron neutron capture therapy (BNCT): rational precursor design, radiofluorination, and characterization of methodology.

BACKGROUND: Boron neutron capture therapy relies on the selective accumulation of boron-containing compounds in tumor tissue, making accurate quantification of boron distribution essential for effective treatment planning. The amino acid analog boronophenylalanine is widely used as a boron delivery agent, yet direct assessment of its biodistribution remains challenging. A fluorine-18 labeled analog, 2-fluoro-boronophenylalanine, offers the potential to visualize and quantify uptake through positron emission tomography. However, reported radiosynthetic methods often suffer from low radiochemica

Boron neutron capture therapy of brain tumors: functional and neuropathologic effects of blood-brain barrier disruption and intracarotid injection of sodium borocaptate and boronophenylalanine.

Sodium borocaptate (BSH) and boronophenylalanine (BPA) are two drugs that have been used clinically for boron neutron capture therapy (BNCT) of brain tumors. We previously have reported that hyperosmotic mannitol-induced disruption of the blood-brain barrier (BBB-D), followed by intracarotid (i.c.) administration of BPA or BSH, either individually or in combination, significantly enhanced tumor boron delivery and the efficacy of BNCT in F98 glioma bearing rats. The purpose of the present study was to determine the short-term neuropathologic consequences of this treatment and the long-term effe

Tissue Architecture Influences the Biological Effectiveness of Boron Neutron Capture Therapy in In Vitro/In Silico Three-Dimensional Self-Assembly Cell Models of Pancreatic Cancers.

Pancreatic cancer is a leading cause of cancer death, and boron neutron capture therapy (BNCT) is one of the promising radiotherapy techniques for patients with pancreatic cancer. In this study, we evaluated the biological effectiveness of BNCT at multicellular levels using in vitro and in silico models. To recapture the phenotypic characteristic of pancreatic tumors, we developed a cell self-assembly approach with human pancreatic cancer cells Panc-1 and BxPC-3 cocultured with MRC-5 fibroblasts. On substrate with physiological stiffness, tumor cells self-assembled into 3D spheroids, and the c

Cerebrospinal fluid dissemination of high-grade gliomas following boron neutron capture therapy occurs more frequently in the small cell subtype of IDH1R132H mutation-negative glioblastoma.

We have used boron neutron capture therapy (BNCT) to treat patients in Japan with newly diagnosed or recurrent high-grade gliomas and have observed a significant increase in median survival time following BNCT. Although cerebrospinal fluid dissemination (CSFD) is not usually seen with the current standard therapy of patients with glioblastoma (GBM), here we report that subarachnoid or intraventricular CSFD was the most frequent cause of death for a cohort of our patients with high-grade gliomas who had been treated with BNCT. The study population consisted of 87 patients with supratentorial hi

Investigation of the usability of cone-beam computed tomography images using digital radiography equipment for boron neutron capture therapy treatment planning in the sitting position.

In boron neutron capture therapy (BNCT), treatment planning images are acquired in the recumbent position. However, treatment is occasionally performed in the sitting position. For BNCT treatment planning, we investigated the usability of cone-beam computed tomography (CBCT) images using digital radiography equipment that allows imaging in the sitting position. The dose calculation results in both CBCT and fan beam CT were in good agreement. This method will eliminate the posture difference between planning and treatment.

Avidin-dendrimer-(1B4M-Gd)(254): a tumor-targeting therapeutic agent for gadolinium neutron capture therapy of intraperitoneal disseminated tumor which can be monitored by MRI.

Peritoneal carcinomatosis is a late stage in cancer progress, for which no effective therapeutic modality exists. Targeting therapeutic agents to disseminated lesions may be a promising modality for treating peritoneal carcinomatosis. Gadolinium ((157,155)Gd) is known to generate Auger and internal conversion electrons efficiently by irradiation with a neutron beam. Auger electrons from neutron-activated Gd(III) are strongly cytotoxic, but only when Gd(III) atoms have been internalized into the cells. In the present investigation, we have developed a quickly internalizing tumor-targeting syste
