Anesthetically blocked optic nerve conduction in congenital stationary night blindness.

It has been hypothesized that the negative configuration of the electroretinogram (ERG) in congenital stationary night blindness (CSNB) may change to a positive configuration following loss of the inhibitory function of the optic nerve centrifugal fibers. In order to study this hypothesis, the ERGs before and after blocking optic nerve conduction by retrobulbar anesthesia were recorded in a patient with CSNB, who had retinal breaks and was being treated with cryopexy under retrobulbar anesthesia. After complete blockade of the optic nerve conduction induced by retrobulbar anesthesia, the ERG s

Congenital stationary night blindness with myopia: a clinico-pathologic study.

The left eye of a 77 year old male patient was enucleated because of absolute glaucoma. This eye showed subnormal electroretinogram (ERG) and oscillatory potentials preoperatively, but the other eye showed Schubert-Bornschein type ERG and monophasic dark adaptation curve. Light and electron microscopic studies of the left eye showed a normal arrangement of discs of rod outer segments, normal synaptic ends of the photoreceptors, and complete loss of ganglion cells. From the subnormal ERG in the left eye we assumed it was originally Schubert-Bornschein type ERG but inverted to subnormal type ERG

The photopic electroretinogram in congenital stationary night blindness with myopia.

Previous studies have reported that subjects affected with congenital stationary night blindness and myopia demonstrated some photopic (cone) abnormalities in their electroretinogram (ERG). By comparing the photopic ERG elicited with a threshold and a suprathreshold stimulus it was found that, at threshold, no significant differences were noted both in the peak time and in the amplitude of ERGs evoked from CSNB and normal subjects. However, a more powerful stimulus (16 times the threshold) yields a significant difference in the ERGs recorded from the two groups. ERGs recorded from CSNB patient

Risk factors for maternal night blindness in rural South India.

PURPOSE: This study aimed to identify risk factors associated with maternal night blindness in rural South India. METHODS: At delivery, women enrolled in a population-based trial of newborn vitamin A supplementation were asked whether they were night blind at any time during the pregnancy. Multivariate logistic regression was used to identify socioeconomic, demographic, and pregnancy-related factors associated with maternal night blindness. RESULTS: Women reported night blindness in 687 (5.2%) of 13,171 pregnancies. In a multivariate model, having a concrete roof (Odds Ratio (OR): 0.60, 95%

[Oguchi disease or stationary congenital night blindness: a case report].

INTRODUCTION: Oguchi disease, originally described in Japanese people, is a rare form of stationary night blindness in patients with normal acuity. OBSERVATION: We report the case of an 8-year-old girl who presented with an abnormal terrified behavior in the dark. Thorough questioning revealed hemeralopia. Her clinical examination (visual acuity, Goldmann visual field, and color vision) were normal. The fundus examination showed golden-brown color, grayish, almost greenish yellow discoloration in the peripheral area with no osteoclast. This abnormality disappeared after prolonged dark adaptat

Incomplete congenital stationary night blindness associated with symmetrical retinal atrophy.

PURPOSE: To describe a Japanese patient with incomplete congenital stationary night blindness (iCSNB) with atypical retinal atrophy and kinetic visual field defects. METHODS: An ophthalmologic examination was performed, and the CACNA1F gene was analyzed by direct genomic sequencing. RESULTS: The patient had a hemizygous Arg913stop mutation in CACNA1F and had electroretinographic changes that were typical of iCSNB. The fundus had atrophic retinal lesions around the inferior vascular arcades OU, and Goldmann kinetic perimetry showed relative scotomas in the corresponding areas. CONCLUSIONS: A

Multifocal ERG findings in complete type congenital stationary night blindness.

PURPOSE: To study the multifocal electroretinogram (mfERG) in patients with the complete type of congenital stationary night blindness (cCSNB), which is thought to be due to a defect in neurotransmission from the photoreceptors to the ON-bipolar cells. METHODS: mfERGs were recorded with the VERIS recording system from four patients with cCSNB, none of whom had nystagmus. The stimulus array consisted of 61 hexagons, and the total recording time was approximately 4 minutes. The amplitudes and implicit times of the first- and second-order kernels of the local responses were compared with those f

Night blindness in Sorsby's fundus dystrophy reversed by vitamin A.

Sorsby's fundus dystrophy (SFD) is an autosomal dominant retinal degeneration caused by mutations in the tissue inhibitor of metalloproteinases-3 (TIMP3) gene. Mechanisms of the visual loss in SFD, however, remain unknown. In a SFD family with a novel TIMP3 point mutation, we tested a hypothesis that their night blindness is due to a chronic deprivation of vitamin A at the level of the photoreceptors caused by a thickened membrane barrier between the photoreceptor layer and its blood supply. Vitamin A at 50,000 IU/d was administered orally. Within a week, the night blindness disappeared in pat

Characteristic ERG-flicker anomaly in incomplete congenital stationary night blindness.

Ten patients with the incomplete type of congenital stationary night blindness (CSNB) were examined with a 30 Hz flicker electroretinogram (ERG). After 30 min of dark adaptation, 30 Hz flicker ERG was recorded continuously for 12-15 min under white background illumination. All patients showed an exaggerated increase of amplitude and a universal characteristic change of wave shape as the light adaptation progressed. Thirty normal subjects also showed increased amplitude during light adaptation, but the increase in amplitude was significantly less than in incomplete-type CSNB, and there was litt

Congenital stationary night blindness with negative electroretinogram. A new classification.

An analysis of 64 patients with congenital stationary night blindness showed that all had essentially normal fundi. Electroretinography (ERG) showed a normal a wave with extremely reduced b wave (negative type) when recorded with a single bright white stimulus in the dark. We classified these patients into two groups based on the evaluation of rod ERG and/or psychophysical dark adaptation measured with an 11 degree test target at 15 degrees in the upper part of the retina. One group (35 patients) lacked rod function (complete type). Nine patients could not be classified. No pedigree showed a c

Vitamin A treatment for night blindness in primary biliary cirrhosis.

Three patients with late stage primary biliary cirrhosis were found to have appreciable night blindness. Serum vitamin A concentrations were low in all three patients despite regular intramuscular supplementation in two. All patients responded dramatically to high dose oral supplementation, with full recovery of adaptation to dark and visual fields. Oral rather than intramuscular vitamin A supplementation seems appropriate in the prevention of ocular complications of vitamin A deficiency in biliary cirrhosis.

Night blindness, characteristic facies, and skeletal abnormalities in two brothers.

Two brothers are described with a similar physical appearance characterised by minor periorbital anomalies, malar flatness, a maxillary overbite, retrognathia, sloping shoulders, joint hyperextensibility, and minor radiological anomalies. In addition, they had a slowly progressing night blindness, myopia, and extinguished electroretinograms. The mother had mild expression of some of the physical anomalies and a decreased electroretinogram response to red light. We have been unable to find any report of similarly affected children. The possible modes of inheritance are discussed.

Changes in the DC electroretinogram in Briard dogs with hereditary congenital night blindness and partial day blindness.

Five Briard dogs, 7-12 months old, with congenital night blindness and severely reduced day vision (offspring of a sister and brother with congenital and supposedly stationary night blindness but with normal or nearly normal day vision) and three normal control dogs were studied by means of direct current (DC) electroretinography in order to analyse fast and slow retinal and pigment epithelial (RPE) potentials. No definite a- and b-waves were seen in the affected dogs in the dark-adapted state, which indicates severely impaired rod function. All affected dogs responded to 30 Hz flickering ligh

Clinical and genetic findings in TRPM1-related congenital stationary night blindness.

PURPOSE: Congenital stationary night blindness (CSNB) is a heterogeneous group of Mendelian retinal disorders that present in childhood. Biallelic variants altering the protein-coding region of the TRPM1 gene are one of the commonest causes of CSNB. Here, we report the clinical and genetic findings in 10 unrelated individuals with TRPM1-retinopathy. METHODS: Study subjects were recruited through a tertiary clinical ophthalmic genetic service at Manchester, UK. All participants underwent visual electrodiagnostic testing and panel-based genetic analysis. RESULTS: Study subjects had a median ag

Genetic Analysis of Consanguineous Pakistani Families with Congenital Stationary Night Blindness.

INTRODUCTION: Congenital stationary night blindness (CSNB) is a rare, largely nonprogressive, inherited retinal disorder that can be clinically classified on the basis of fundus and electroretinogram abnormalities. METHODS: We analyzed four large consanguineous families from the Southern Punjab region of Pakistan including multiple individuals affected with CSNB. Exome sequencing was performed in probands of all four families; Sanger sequencing was performed in additional members to test co-segregation of the variants identified. RESULTS: We identified two novel and likely pathogenic variant
