Effects of tolterodine, trospium chloride, and oxybutynin on the central nervous system.

Antimuscarinic compounds are increasingly used to treat the symptoms of overactive bladder; however, their use is often restricted by peripheral adverse effects (AEs). On the other hand, data regarding their influence on the central nervous system (CNS) are limited. This randomized, single-blind, parallel-group quantitative-topographical EEG (qEEG) study of clinical phase I investigates the potential CNS adverse effects of the three antimuscarinic drugs--tolterodine, oxybutynin, and trospium chloride--in comparison to placebo. Overall, 4 x 16 (total 64) young, healthy male volunteers were incl

[High-dose trospium chloride in therapy of detrusor hyperreflexia].

The efficacy and tolerance of a high-dose treatment with trospium chloride (20 mg twice daily) were investigated in pilot studies carried out in three trial centres and involving a total of 29 patients suffering from reflex bladder due to transverse lesions of the spinal cord with paraplegia. In all three centres the trial procedure was the same. Urodynamic measurements (maximum bladder capacity, bladder compliance, maximum detrusor pressure during micturition, urinary flow and residual urine) were taken both before and after treatment with trospium chloride for a period of at least 2 weeks. I

Single-photon emission computed tomography 123I-ioflupane imaging in CSF1R mutation carriers.

AIM OF THE STUDY: CSF1R-related disorder (CSF1R-RD) is a rare, autosomal dominant microgliopathy characterized by leukoencephalopathy. While extrapyramidal symptoms are common, the underlying integrity of the presynaptic dopaminergic system in these patients remains poorly understood. The aim of our study was to analyze dopamine transporter availability using single-photon emission computed tomography (SPECT) 123I-ioflupane in patients with CSF1R mutations to differentiate the pathophysiology of their parkinsonism. MATERIAL AND METHODS: We conducted a retrospective review of 112 electronic me

The Impact of Xanomeline and Trospium Chloride on Cognitive Impairment in Acute Schizophrenia: Replication in Pooled Data From Two Phase 3 Trials.

OBJECTIVE: Xanomeline and trospium chloride (formerly known as KarXT), a novel M1/M4 muscarinic receptor agonist, demonstrated efficacy across phase 2 and 3 trials as monotherapy for the treatment of inpatients with acute schizophrenia on the Positive and Negative Syndrome Scale total score primary endpoint. In the phase 2 trial, xanomeline/trospium improved performance on a cognitive outcome measure in the subgroup of participants with clinically significant baseline cognitive impairment. The authors sought to confirm this finding using data from two phase 3 trials. METHODS: Data were pooled

DaT's Awesome: Hawai'i's First Dopamine Transporter (DaT) Nuclear Medicine Study.

Parkinsonian Syndromes are difficult to accurately diagnose and distinguish from other neurological processes such as essential tremor. Until now, physical exam and clinical presentation have been the gold standard for diagnosis (bradykinesia, tremor, rigidity, and postural instability).1 However, this leads to over- or under diagnosis and improper treatment due to variability in presentation and symptoms.1 A nuclear medicine study using I-123 Ioflupane (DaTSCAN) has been developed, which allows accurate differentiation of Parkinsonian Syndromes from other etiologies.1 This study is now widely

On the utility of ChatGPT in conducting a literature review on deep learning for dopamine transporter SPECT with [¹²³I]ioflupane.

AIM: This study evaluated ChatGPT (GPT-5.2) for drafting a review paper on deep learning in dopamine transporter (DAT)-SPECT with [¹²³I]ioflupane. METHODS: The review workflow consisted of 3 steps: (i) literature search, (ii) generation of structured summaries with 24 predefined fields for each publication, and (iii) drafting a review paper based on the structured summaries across all publications. A detailed prompt for ChatGPT was iteratively designed for each step with ChatGPT support. A manual literature search was independently performed by an expert in DAT-SPECT and deep learning. ChatGP

Transitioning from clozapine to Cobenfy (xanomeline-trospium chloride) in treatment resistant schizophrenia and clozapine induced OCD.

BACKGROUND: Treatment-resistant schizophrenia (TRS) is defined as nonresponse to at least two adequate antipsychotic trials at therapeutic doses for six or more weeks. Currently, clozapine is the only antipsychotic approved for TRS by the FDA and a plethora of evidence indicates that it is the most efficacious antipsychotic for treatment of chronic and refractory cases of schizophrenia. However, its utilization is often limited by metabolic, hematologic, and anticholinergic side effects. Notably, clozapine-induced obsessive-compulsive symptoms (OCS) or obsessive-compulsive disorder (OCD) have

Association of Hemispheric Asymmetry of Dopamine-Transporter Binding with Risk of Parkinsonian Depression.

BACKGROUND: Depression is the most common psychiatric disorder diagnosed in patients with Parkinson's disease (PD). A direct role in PD depression for loss of dopaminergic terminals and dopamine-transporter (DAT) expression in the striatum is revealed by many studies. OBJECTIVES: The objective was to discern the relationship between DAT neuroimaging and risk of depression in PD. METHODS: One hundred and ninety-eight PD patients (101 with depression, 97 without depression) were evaluated using an extensive protocol from 2015 to 2023. DAT availability at striatal terminals was assessed with si

Diagnostic Performance of the Visual Reading of 123I-Ioflupane SPECT Images With or Without Quantification in Patients With Movement Disorders or Dementia.

Visual interpretation of 123I-ioflupane SPECT images has high diagnostic accuracy for differentiating parkinsonian syndromes (PS), from essential tremor and probable dementia with Lewy bodies (DLB) from Alzheimer disease. In this study, we investigated the impact on accuracy and reader confidence offered by the addition of image quantification in comparison with visual interpretation alone. Methods: We collected 304 123I-ioflupane images from 3 trials that included subjects with a clinical diagnosis of PS, non-PS (mainly essential tremor), probable DLB, and non-DLB (mainly Alzheimer disease).

Pharmacologic management of overactive bladder.

Overactive bladder (OAB) is a prevalent and costly condition that can affect any age group. Typical symptoms include urinary urgency, frequency, incontinence and nocturia. OAB occurs as a result of abnormal contractions of the bladder detrusor muscle caused by the stimulation of certain muscarinic receptors. Therefore, antimuscarinic agents have long been considered the mainstay of pharmacologic treatment for OAB. Currently, there are five such agents approved for the management of OAB in the United States: oxybutynin, tolterodine, trospium, solifenacin and darifenacin. This article summarizes

Treatment of the overactive bladder syndrome with muscarinic receptor antagonists: a matter of metabolites?

Antagonists of muscarinic acetylcholine receptors, such as darifenacin, oxybutynin, propiverine, solifenacin, tolterodine, and trospium, are the mainstay of the treatment of the overactive bladder syndrome. Fesoterodine is a newer drug awaiting regulatory approval. We briefly review the pharmacological activity of their metabolites and discuss how active metabolites may contribute to their efficacy and tolerability in vivo. Except for trospium, and perhaps solifenacin, all of the above drugs form active metabolites, and their presence and activity need to be taken into consideration when eluci

[Tuberculosis of the urinary bladder: diagnosis and treatment].

Tuberculosis of the urinary bladder (UB) is registered in 10-45.6% patients with urogenital tuberculosis. It is a severe incapacitating complication of kidney tuberculosis which significantly lowers quality of life. The results of the standard therapy of 76 patients with tuberculous cystitis are analysed. UB tuberculosis is diagnosed more often in women, outcomes of their treatment are worse. Twenty six patients with UB tuberculosis at the stage of overactive bladder received polychemotherapy and adjuvant trospium. After 3 months urination frequency decreased 4.3 times, on the average; UB volu

Utility of the combination of DAT SPECT and MIBG myocardial scintigraphy in differentiating dementia with Lewy bodies from Alzheimer's disease.

PURPOSE: (123)I-2β-Carbomethoxy-3β-(4-iodophenyl)-N-(3-fluoropropyl) nortropane ((123)I-FP-CIT) dopamine transporter single photon emission computed tomography (DAT SPECT) and (123)I-metaiodobenzylguanidine (MIBG) myocardial scintigraphy can be used to assist in the diagnosis of patients with dementia with Lewy bodies (DLB). We compared the diagnostic value of these two methods in differentiating DLB from Alzheimer's disease (AD). Furthermore, we evaluated whether a combination of DAT SPECT and MIBG myocardial scintigraphy would provide a more useful means of differentiating between DLB and AD
