Illicit drug overdose--prevalence and acute management.

BACKGROUND: Illicit drug use is common in Australia, with nearly 40% of Australians having tried an illicit substance in their lifetime. While cannabis is by far the most prevalent substance used, heroin and other opiates contribute to over 90% of deaths associated with illicit drug use. OBJECTIVE: This article describes the current epidemiology and harms related to illicit drug use, and outlines the principles in the management of acute overdose. DISCUSSION: The acute management of a suspected overdose involves the identification of the toxicants by taking a careful history accompanied by a

Medicaid And Methadone For Opioid Use Disorder: Expanded Coverage Increased Distribution In 10 States, 2019-24.

Methadone is a vital medication for opioid use disorder, yet it remains underused because of restrictive federal and state regulations. Before 2018, not all state Medicaid programs covered methadone treatment. This prompted passage of the 2018 Substance Use Disorder Prevention that Promotes Opioid Recovery and Treatment for Patients and Communities (SUPPORT) Act, mandating Medicaid coverage of methadone by 2020. This study evaluated the impact of expanded coverage on methadone distribution and opioid overdoses. Using controlled substance distribution data from the period 2016-24, as well as na

[Interest of take-home naloxone for opioid overdose].

Over the course of these last decades, we observed a change on opioid use with the marketing of opiate maintenance treatment, an increase of opioids used for pain management and recent concerns have arisen around the use of synthetic opioid. The World Health Organization (WHO) reports around 70,000 people opioid overdose death each year. In France, according to the DRAMES program (fatalities in relation with abuse of licit or illicit drugs) of the French addictovigilance network, most of deaths are related to opioids overdose (especially methadone, following by heroin, buprenorphine and opioid

[Guideline 'Medicinal care for drug addicts in penal institutions'].

In the Netherlands, the policy on care for prisoners who are addicted to opiates is still heterogeneous. The recent guidelines entitled 'Medicinal care for drug addicts in penal institutions' should contribute towards unambiguous and more evidence-based treatment for this group. In addition, it should improve and bring the care pathways within judicial institutions and mainstream healthcare more into line with one another. Each rational course of medicinal treatment will initially be continued in the penal institution. In penal institutions the help on offer is mainly focused on abstinence fro

Fentanyl Polysubstance Use Patterns and Their Associations With Hepatitis C Virus, Skin and Soft Tissue Infections, and Non-Fatal Overdose Among People Who Inject Drugs in New York City.

INTRODUCTION: Fentanyl's euphoric effects and short half-life may increase infectious disease transmission risks through frequent injecting and syringe sharing. We examined fentanyl polysubstance use (PSU) patterns and associations with hepatitis C virus (HCV), skin and soft tissue infections (SSTI), and non-fatal overdose among people who inject drugs (PWID) in New York City. METHODS: We recruited 495 PWID between October 2021 and July 2024. Participants were tested for HCV antibody and underwent urine toxicology screenings using the Premier Biotech 13-panel BioCup. RESULTS: Fentanyl was id

Hyperalgesia, allodynia, and eventual overdose: The problems with chronic opioid use.

Opioids are known to come with some relatively benign side effects, not including their addictive potential. This review will look at some of the side effects that occur when patients, especially chronic pain patients, take opioids chronically. These side effects include both hyperalgesia and allodynia caused by opioids. Overdose and factors, including hyperalgesia and allodynia, that make a patient more likely to overdose, is the other topic that will be covered. Not much research has been done with human subjects regarding treatment of opioid induced hyperalgesia or allodynia, but some of th

The Donovan Memorial Lecture: 'Fentanyl-Plus': A New Era of Fentanyl Polydrug Combinations in the North American Overdose Crisis.

The 2025 ACMT Ward and Ryan Donovan Memorial Fund lecture was presented by Daniel Ciccarone, MD, MPH from Department of Family and Community Medicine at the University of California San Francisco (UCSF). This article is an edited version of his keynote address during ACMT's 2025 Annual Scientific Meeting. During the course of his talk, Dr. Ciccarone discussed polysubstance use in the "Fentanyl-plus" era. The epidemiology and economics of the four waves of opioid overdose deaths were examined, and promising harm reduction strategies to assist in reducing overdose mortality were highlighted.

Intranasal overdose reversal formulations: a rapid review of available agents.

The opioid epidemic continues to pose a significant threat to public health. Naloxone, a potent opioid antagonist, has proven to be a crucial tool in reversing opioid overdoses. Efforts to increase access to overdose reversal agents, initially given by injection in hospitals and emergency environments, resulted in the development of intranasal (IN) forms of naloxone, and more recently, nalmefene. This rapid review examines the currently available IN overdose reversal agents in the United States, focusing on their dosing, efficacy, and prescription status. We conducted a comprehensive search of

Recent Innovations to Detect and Intervene to Prevent Opioid Overdose Deaths.

Opioid overdose deaths have become a rapidly growing global health crisis, with current estimates indicating over 125,000 deaths annually worldwide. This crisis has reached particularly high levels across North America, where the USA recorded over 100,000 opioid-related deaths in 2022, while the UK has seen mortality rates surpass those from road traffic accidents. Despite the scale and urgency of this public health emergency, opioid overdose remains an area of strangely 'undone science'. Five interconnected areas of research are examined, addressing critical gaps in our understanding and our

Neighborhood and Individual Disparities in Community-Based Naloxone Access for Opioid Overdose Prevention.

Improving access to naloxone for laypersons is a cornerstone of the US strategy to reduce opioid overdose deaths. This study evaluated change in distance to opioid overdose prevention programs (OOPPs) providing walk-in naloxone across two time points. We also explored individual and neighborhood disparities in distance to OOPPs, associations between 2020 OOPP locations and 2018 overdoses, and associations between OOPPs and neighborhood fatal overdose rates. Using fatal opioid overdose locations in 2018 (n = 1167) and 2020 (n = 2045) in New York City, we mapped OOPP locations and fatal overdose

A pilot study investigating cognitive impairment associated with opioid overdose.

BACKGROUND: In 2021, while overdose (OD) deaths were at the highest in recorded history, it is estimated that >80% of ODs do not result in a fatality. While several case studies have indicated that opioid-related ODs can result in cognitive impairment, the possible association has not yet been systematically investigated. METHODS: 78 participants with a history of OUD who reported experiencing an OD in the past year (n=35) or denied a lifetime history of OD (n=43) completed this study. Participants completed cognitive assessments including the Test of Premorbid Functioning (TOPF) and the NIH

All Hands on Deck: We Need Multiple Approaches To Uncover the Neuroscience behind the Opioid Overdose Crisis.

Opioid use disorder (OUD) affects millions of people throughout the United States, yet there are only three Food and Drug Administration-approved pharmacological treatments. Though these treatments have been shown to be effective, the number of overdose deaths continues to rise. The increase of fentanyl, fentanyl analogs, and adulterants in the illicit drug supply has further complicated treatment strategies. Preclinical researchers strive to model OUD to better understand this complicated disorder, and this research is a critical enabler for the development of novel treatments. As a result, t

Lessons Learned from Implementation of a Post-opioid Overdose Outreach Program in a Rural Massachusetts Community.

Post-overdose outreach programs can play a key role in reducing opioid overdose deaths and increasing access to healthcare services. The design and implementation of these programs, especially in rural communities, remains a gap in knowledge. We aimed to understand the lessons learned from the implementation experiences of the Community, Opportunity, Network, Navigation, Exploration, and Connection Team (CONNECT), a post-overdose outreach program based in a rural community in Massachusetts. We conducted semi-structured focus groups and interviews with 21 community partners after the first year

No obvious effect on mortality from a patient choice reform expanding access to opioid disorder treatment - results from a natural experiment of policy change in Sweden.

BACKGROUND: Opioid-related overdose deaths remain a common cause of death in many settings, and opioid maintenance treatment is evidence-based for the treatment of opioid use disorders. However, access to such treatment varies and is limited in many settings. METHODS: The present study examines the longitudinal effects of a regional patient choice reform which substantially increased availability to opioid maintenance treatment in one Swedish county, starting from 2014. A previous follow-up, limited in time, indicated a possible effect on mortality from this intervention, demonstrating a lowe

Mortality, incarceration and cost implications of fentanyl felonization laws: A modeling study.

BACKGROUND: Opioid overdose continues to be a major cause of death in the United States. One effort to control opioid use has been to implement policies that enhance criminalization of opioid possession. Laws to further criminalize possession of fentanyl have been enacted or are under consideration across the country, including at the national level. OBJECTIVE: Estimate the long-term effects on opioid death and incarceration resulting from increasingly strict fentanyl possession laws . DESIGN: We built a Markov simulation model to explore the potential outcomes of a 2022 Colorado law which m

Perspectives and recommendations of opioid overdose survivors experiencing unsheltered homelessness on housing, overdose, and substance use treatment in Boston, MA.

INTRODUCTION: Opioid overdose causes one in four deaths among people experiencing homelessness in Boston, MA. To reduce overdose risks, the experience and perspectives of people experiencing homelessness should be incorporated into housing, overdose prevention, and substance use treatment efforts. METHODS: In 2021, we conducted qualitative interviews with 59 opioid overdose survivors to inform equitable access to treatment services. In response to policy debate surrounding a public drug scene near a key recruitment site, we conducted a targeted thematic analysis of transcribed interview data
