Significances of spirometry and impulse oscillometry for detecting small airway disorders assessed with endobronchial optical coherence tomography in COPD.

BACKGROUND: Spirometry confers limited value for identifying small-airway disorders (SADs) in early-stage COPD, which can be detected with impulse oscillometry (IOS) and endobronchial optical coherence tomography (EB-OCT). Whether IOS is useful for reflecting small-airway morphological abnormalities in COPD remains unclear. OBJECTIVES: To compare the diagnostic value of spirometry and IOS for identifying SADs in heavy-smokers and COPD based on the objective assessment with EB-OCT. METHODS: We recruited 59 COPD patients (stage I, n=17; stage II, n=18; stage III-IV, n=24), 26 heavy-smokers and

Arteriolar Elasticity Measurement in the Fingertip Based on Photoplethysmographic Volume-Oscillometry: A New Approach to the Assessment of Vasomotor Functions in the Microvasculature.

PURPOSE: Dysfunction of vasomotor reactions due to arteriolar smooth muscle causes serious adverse events, such as loss of hemodynamic coherence. This in turn can increase risks of cardiovascular-related diseases. A noninvasive and quantitative evaluation of microvascular disorder is therefore very important for early diagnosis and treatment. This paper describes a new approach to the assessment of vasomotor functions using the arteriolar elasticity measurement technique in the fingertip. METHODS: A recently developed device, modified to detect a photoplethysmogram with green light (gPPG) in

A comparison of respiratory oscillometry and spirometry in idiopathic pulmonary fibrosis: performance time, symptom burden and test-retest reliability.

STUDY QUESTION: In large multinational patient surveys, spirometry (which requires repeated, reproducible maximal efforts) can be associated with cough, breathlessness and tiredness, particularly in those with idiopathic pulmonary fibrosis (IPF). Oscillometry is an effort-independent test of airways resistance and reactance. We hypothesised that oscillometry would take less time to perform and would be associated with reduced symptom burden than spirometry. PATIENTS AND METHODS: Spirometry and oscillometry were performed in 66 participants with IPF and repeated 2 weeks later. We compared time

Oscillometry and pulmonary MRI measurements of ventilation heterogeneity in obstructive lung disease: relationship to quality of life and disease control.

Ventilation heterogeneity is a hallmark finding in obstructive lung disease and may be evaluated using a variety of methods, including multiple-breath gas washout and pulmonary imaging. Such methods provide an opportunity to better understand the relationships between structural and functional abnormalities in the lungs, and their relationships with important clinical outcomes. We measured ventilation heterogeneity and respiratory impedance in 100 subjects [50 patients with asthma, 22 ex-smokers, and 28 patients with chronic obstructive pulmonary disease (COPD)] using oscillometry and hyperpol

Small airway dysfunction by impulse oscillometry in asthmatic patients with normal forced expiratory volume in the 1st second values.

Small airways are relevant to the pathophysiology of asthma. We investigated whether in asthmatic patients with normal forced expiratory volume in the 1st second (FEV(1)) values, impulse oscillometry system (IOS), as a measure of small airway function, contributed additional information to spirometry either at baseline or after bronchodilator, and whether it was related to the disease control. The fall in resistance from 5 to 20 Hz (R5-R20) and reactance at 5 Hz (X5) by IOS and spirometry measures of small airway function (forced expiratory flow at 25-75% [FEF(25-75)] and forced vital capacity

Expiratory phase lung mechanics in late-onset Pompe disease: a multicenter study using oscillometry to identify specific breathing abnormalities.

INTRODUCTION: In late-onset Pompe disease (LOPD), muscle weakness causes restrictive lung impairment. In murine models, glycogen deposition in lung parenchyma suggests additional causes for restriction that remain poorly understood. We investigated whether oscillometry detects respiratory abnormalities not identified by spirometry and evaluated changes after air-stacking manoeuvres. METHODS: We prospectively evaluated 16 adults with LOPD from three Italian centres. Alongside spirometry, oscillometry (at 5, 11, and 19 Hz) was performed to assess resistance (R) and reactance (X) across the brea

Adding Oscillometry and Fractional Exhaled Nitric Oxide to Guidelines Potentially Enhances Identification of Exacerbation Risk Factors in Asthmatic Children.

BACKGROUND: A possible explanation for the continued risk of asthma exacerbations while utilizing current asthma guideline-based management is the failure to consider small airway dysfunction (SAD) and eosinophilic airway inflammation in assuming asthma-controlled status. OBJECTIVE: To construct a practical algorithm that potentially identifies additional risks of not well-controlled (NWC) asthma and exacerbations beyond guideline criteria with oscillometry and fractional exhaled nitric oxide (FENO) determination. METHODS: This was a retrospective, posthoc analysis of 183 children, ages 4-18

Consistency Between Oscillometry and Spirometry in Patients with Suspected or Confirmed COPD or Asthma: Results from a Multi-Center Study in China.

UNLABELLED: Spirometry is the standard for assessing airflow obstruction but can be challenging and/or time-consuming for patients. Oscillometry offers an effort-independent alternative. This study assessed the consistency between oscillometry and spirometry in Chinese patients with suspected or confirmed chronic obstructive pulmonary disease (COPD) or asthma. This prospective study enrolled patients from 46 Chinese hospitals. All underwent oscillometry with the Ambulatory Lung Diagnosis System followed by spirometry, both before and after salbutamol. The primary outcome was prebronchodilator

Impulse oscillometry defined small airway dysfunction in asthmatic patients with normal spirometry: Prevalence, clinical associations, and impact on asthma control.

BACKGROUND: The small-airway dysfunction (SAD), detected with impulse oscillometry (IOS) methods, has been recently better characterized in patients with asthma. However, little is known about SAD in asthmatic patients with normal spirometry (NS). OBJECTIVE: In this study, we aimed to investigate, in an unselected sample of 321 patients with physician-diagnosed asthma and NS, prevalence, clinical characterization, and impact on asthma control of IOS-defined SAD. As a secondary objective of the study, we focused on comparing the difference between IOS- and spirometry-defined SAD. METHODS: Con

Pulmonary function and physical fitness in obese children and adolescents: a comparison of impulse oscillometry, spirometry, and 6-min walk test.

UNLABELLED: Obesity-associated alterations in pulmonary function have been inconsistently identified by spirometry in pediatric populations. Impulse oscillometry (IOS) offers sensitive detection of peripheral airway resistance, yet data remain limited. Moreover, the relationship between obesity indices and pulmonary function has not been comprehensively evaluated. This study aimed to compare pulmonary function and physical fitness between obese and normal-weight children and adolescents using spirometry, IOS, and the 6-min walk test (6-MWT), and assessed obesity indices as predictors of pulmon

Impulse oscillometry for the detection of small airway dysfunction in patients with chronic respiratory symptoms, preserved ratio impaired spirometry and COPD.

BACKGROUND: Persistent chronic airway inflammation and progressive airflow limitation are typical features of chronic obstructive pulmonary disease (COPD). Emerging evidence indicates that small airway dysfunction (SAD) plays a critical role in driving the sustained pathological progression of COPD. Preserved ratio impaired spirometry (PRISm) represents a spirometric pattern characterized by a reduced forced expiratory volume in 1 second (FEV₁) despite a preserved ratio. Current evidence inadequately elucidates the pathophysiological role of SAD and its intricate interplay with PRISm and COPD
