Artificial intelligence model outperformed experienced clinicians in differentiating the aetiology of pneumonia on chest computed tomography: a retrospective study.

BACKGROUND: Rapid and precise aetiological diagnosis is crucial for managing pneumonia. We aimed to develop and validate deep learning (DL) models for differentiating ten pneumonia aetiologies on chest computed tomography images. METHODS: We enrolled 1,091 pneumonia patients with 1 of 10 definite aetiological diagnoses between October 1st, 2015 and June 30th, 2022 in this retrospective study. We trained and validated two DL models: a classic 3D-DenseNet model (DenseNet) and a novel large vision model (LVM). The models were tested on a data from 183 nonoverlapping patients for external dataset

The role of respiratory management of Pompe disease.

Respiratory failure is an unavoidable event in the natural history of some neuromuscular diseases, while appearing very infrequently in others. In some cases, such as Pompe disease, respiratory failure progresses more rapidly than motor impairment, sometimes being the onset event. Home mechanical ventilation improves survival and quality of life of these patients, with a reduction in healthcare costs. Therefore, pulmonologists must improve their skills in order to play a more relevant role in the care of these patients. The aim of this statement is to provide pulmonologists with some simple in

The Evolving Role of Medical Thoracoscopy for the Management of Malignant Pleural Effusion.

MT is a minimally invasive endoscopic procedure which is a well-established tool for the management of pleural malignancies, which commonly cause pleural effusions. MT allows for pulmonologists to perform diagnostic and therapeutic maneuvers at the same time with high diagnostic sensitivity and can also shorten the hospitalization duration. MT, which is video-assisted, is performed by pulmonologists, and is not the same procedure as surgical thoracoscopy or video-assisted thoracoscopy surgery (VATS). To perform MT, pulmonologists use non-disposable rigid or semi-rigid telescopes in the endosco

Malignant Central Airway Obstruction: What's New?

Malignant central airway obstruction (MCAO) is a debilitating and life-limiting complication that occurs in an unfortunately large number of individuals with advanced intrathoracic cancer. Although the management of MCAO is multimodal and interdisciplinary, the task of providing patients with prompt palliation falls increasingly on the shoulders of interventional pulmonologists. While a variety of tools and techniques are available for the management of malignant obstructive lesions, advancements and evolution in this therapeutic venue have been somewhat sluggish and limited when compared with

Issues in the management of chest malignancies.

Opinions can vary as to the best method of treatment for specific types of chest malignancies. If the pulmonologist is to make the primary treatment decision regarding a complicated chest malignancy, he or she must be aware of both the surgical and nonsurgical treatment options. This awareness includes knowledge of applicable surgical procedures, complications, and expected results. Chest malignancies in this category include clinically advanced lung cancer, superior sulcus tumors, chest wall invasion by lung cancer, and malignant mesothelioma. The pulmonologist should also be familiar with th

Prevalence of obesity hypoventilation syndrome in ambulatory obese patients attending pathology laboratories.

BACKGROUND AND OBJECTIVE: The prevalence of obesity hypoventilation syndrome (OHS) in the unselected obese is unknown. Our objectives were: (i) to determine the prevalence of OHS in ambulatory obese patients not previously referred to a pulmonologist for suspicion of sleep breathing disorders and (ii) to assess whether venous bicarbonate concentration [HCO3-v ] can be used to detect OHS. METHODS: In this prospective multicentric study, we measured [HCO3-v ] in consenting obese patients attending pathology analysis laboratories. Patients with [HCO3-v ] ≥ 27 mmol/L were referred to a pulmonolog

Neuromuscular Disorders in Pediatric Respiratory Disease.

Respiratory sequelae are a frequent cause of morbidity and mortality in children with NMD. Impaired cough strength and resulting airway clearance as well as sleep disordered breathing are the two main categories of respiratory sequelae. Routine clinical evaluation and diagnostic testing by pulmonologists is an important pillar of the multidisciplinary care required for children with NMD. Regular surveillance for respiratory disease and timely implementation of treatment including pulmonary clearance techniques as well as ventilation can prevent respiratory related morbidity including hospital

Establishing a Multidisciplinary Team-Based Pleural Service in the Era of Financial Austerity: The Role of the Thoracic Surgeon.

Medical thoracoscopy/pleuroscopy has become, after bronchoscopy, the second most commonly utilized endoscopic procedure in interventional pulmonology. Due to their common origin, medical thoracoscopy/pleuroscopy and video-assisted thoracic surgery (VATS) are quite similar procedures technically. In contrast to the prevailing attitude that it should predominantly be performed by interventional pulmonologists, we believe that, like all hybrid-in-nature techniques, it should be implemented as part of a combined specialist care service/team. Herewith, we describe our attempt to establish a multidi

Diagnosis and management of aspiration using fiberoptic endoscopic evaluation of swallowing in a Pediatric Pulmonology Unit.

BACKGROUND AND OBJECTIVES: Swallowing disorders lead to chronic lung aspiration. Early detection and treatment of aspiration in children with dysphagia are important to prevent lung damage. Diagnosis of aspiration, which may be silent, requires an instrumental study such as fiberoptic endoscopic evaluation of swallowing (FEES). Despite its usefulness, it is rarely practiced by pediatric pulmonologists. This study aimed to evaluate the feasibility and utility of FEES performed in the pediatric respiratory unit of a tertiary hospital, analyze the clinical characteristics, endoscopic findings and

[Screening for alpha1-antitrypsin deficiency using dried blood spot: Assessment of the first 20 months].

INTRODUCTION: Alpha1-antitrypsin deficiency is a predisposing factor for pulmonary disease and under-diagnosis is a significant problem. The results of a targeted screening in patients with respiratory symptoms possibly indicative of severe deficiency are reported here. METHODS: Data were collected from March 2016 to October 2017 on patients who had a capillary blood sample collected during a consultation with a pulmonologist and sent to the laboratory for processing to determine alpha1-antitrypsin concentration, phenotype and possibly genotype. RESULTS: In 20 months, 3728 test kits were req

The diagnosis of lung cancer in the era of interventional pulmonology.

Advances in bronchoscopic and other interventional pulmonology technologies have expanded the sampling procedures pulmonologist can use to diagnose lung cancer and accurately stage the mediastinum. Among the modalities available to the interventional pulmonologist are endobronchial ultrasound-guided transbronchial needles aspiration (EBUS-TBNA) and transoesophageal bronchoscopic ultrasound-guided fine-needle aspiration (EUS-B-FNA) for sampling peribronchial/perioesophageal central lesions and for mediastinal lymph node staging, as well as navigational bronchoscopy and radial probe endobronchia

Acquisition of endobronchial ultrasound bronchoscopy proficiency at a Canadian academic center.

BACKGROUND: Endobronchial ultrasound transbronchial needle aspiration (EBUS-TBNA) is a diagnostic technique for assessing intrathoracic lymph nodes and masses. EBUS-TBNA skill is most commonly acquired through training in interventional pulmonology (IP) or thoracic surgery, but there is no consensus on what constitutes appropriate training. Kingston Health Sciences Centre (KHSC) is a Canadian tertiary care academic institution without a formal IP training program that introduced EBUS in 2014. We seek to describe the attainment of EBUS skill by assessing diagnostic performance and to describe a

Assessment of cardiopulmonary risk in COPD patients: Expert opinion formalized using the Delphi method.

BACKGROUND: Cardiovascular (CV) comorbidities are frequent in patients with chronic obstructive pulmonary disease (COPD), increasing individual morbidity and mortality. This observation has given rise to the concept of cardiopulmonary risk. However, assessment and management of CV risk in COPD patients is highly variable in practice. Cardiologists' and pulmonologists' knowledge and understanding of cardiopulmonary risk and its assessment in COPD patients, along with the management of special situations, were analyzed using the Delphi consensus methodology. Using the Endoscope for Endobronchial Ultrasound in the Esophagus.

EUS-B is a procedure using the echoendobronchoscope in the esophagus and stomach. The procedure is a minimally invasive, safe, and feasible approach that pulmonologists can use to visualize and biopsy structures adjacent to the esophagus and stomach. EUS-B gives access to many structures of which some may also be reached by EBUS (mediastinal lymph nodes, lung or pleural tumors, pericardial fluid) while others cannot be reached such as retroperitoneal lymph nodes, ascites, and lesions in the liver, pancreas or left adrenal gland. The procedure is a pulmonologist- and patient- friendly version o
