Cost of respiratory syncytial virus hospitalisation in Brazilian infants: a micro-costing study from the perspective of a Brazilian tertiary public hospital, 2020-2023.

BACKGROUND: Respiratory Syncytial Virus (RSV) is the leading cause of hospitalisation for respiratory diseases among young children. Costing studies are essential for planning prevention strategies. Additional costing studies in middle-income countries (MIC) are still needed to understand the impact of hospitalisations given the high economic burden of RSV in these countries. We aimed to identify and quantify the resources required and associated costs of paediatric RSV hospitalisations at a tertiary hospital in a MIC. METHODS: A retrospective micro-costing study of RSV-related hospitalisatio

Barriers and Opportunities for Sustainable Hand Hygiene Interventions in Rural Liberian Hospitals.

Hand hygiene is central to hospital infection control. During the 2014-2016 West Africa Ebola virus disease epidemic in Liberia, gaps in hand hygiene infrastructure and health worker training contributed to hospital-based Ebola transmission. Hand hygiene interventions were undertaken post-Ebola, but many improvements were not sustainable. This study characterizes barriers to, and facilitators of, hand hygiene in rural Liberian hospitals and evaluates readiness for sustainable, locally derived interventions to improve hand hygiene. Research enumerators collected data at all hospitals in Bong an

Health economics and resource management: a model for hospital efficiency.

As health care spending continues to climb, government and industry, as the two major purchasers of health care services, are intensifying their scrutiny over health care delivery in an attempt to reduce their health care burden. The first round of utilization controls and reimbursement restrictions focused on necessity of admission and efficiency of care, causing a profound effect on hospital-based services. Declining occupancy rates, reduced inpatient reimbursements, and mounting contractual losses have pushed many hospitals to the point of financial disaster. The second round of controls ha

Budget Impact Analysis of Minimally Invasive versus Open Transforaminal Lumbar Interbody Fusion for Lumbar Degenerative Disease: A European Hospital Perspective.

PURPOSE: When traditional therapies fail to provide relief from debilitating lower back pain, surgeries such as transforaminal lumbar interbody fusion (TLIF) may be required. This budget impact analysis (BIA) compared minimally-invasive (MI)-TLIF versus open (O)-TLIF for single-level fusion from an Italian hospital perspective. METHODS: The BIA compared costs of 100 MI-TLIF and 100 O-TLIF procedures from an Italian hospital perspective over a one-year time horizon. The base case included costs for length of hospital stay (LOS), blood loss, and sterilizing surgical trays. The scenario analysis

What is Happening in the Ambulatory Space? Past Challenges and Future Directions.

Despite reimbursement pressures and scrutiny of procedural appropriateness, the demand for peripheral vascular ambulatory services remains strong. Improvements in minimally invasive technologies, coupled with a supportive regulatory environment and considerable preference for ambulatory services among purchasers, patients, and providers, have resulted in the rapid proliferation of ambulatory facilities in a number of markets. Emerging ecosystem dynamics, notably the rapid growth of Medicare Advantage and the growing presence of private equity and venture capital within healthcare, will likely

Reducing urinary tract infections in catheterized patients.

To compare the effectiveness of two drainage systems in controlling urinary tract infections (UTIs), 65 elderly home care patients with indwelling urinary catheters participated in a retrospective intervention study. The patients first used a Foley drainable bag (DB) system, followed by a nondrainable one (NDB). Both systems used a Foley catheter. Data were obtained from physicians, nurses, caregivers, and patients regarding the number of UTIs and hospitalizations that occurred when using each system. Using the DB, 65 patients had 1,395 UTIs, 27 of which required hospitalization. Using the NDB

Relationship between drug formulary and frequently used cephalosporins, macrolides and quinolones in Japanese hospitals.

In Japan, hospitals' pharmaceutical affairs committees freely select the drugs to be purchased depending on the regulations of each hospital. This system poses a risk of the absence of essential drugs or an excess of similar drugs, and may promote inappropriate use of third-generation cephalosporins (3GCs) and quinolones. Against this backdrop, we researched availability of antibacterial agents in Japanese hospitals. We conducted a questionnaire-based study in eastern Shizuoka Prefecture, Japan. Questionnaires were sent to 33 hospitals that had established an interactive regional partnership o

Bacterial cellulase from the intestinal tract of the sugarcane borer.

Cellulolytic enzymes have wide use in several industrial segments (e.g. biofuels, pulp and paper, food, and cosmetics). However, one of the challenges is their large-scale production with high specific activity to eliminate the dependence of the purchase of enzymatic cocktails produced by commercial parties. The aims of this study were (1) isolation, selection, and partial characterization of bacterial cellulases present in the intestinal tract of the sugarcane borer and (2) to identify cellulase-producing bacteria by analyzing the 16S rDNA gene. Cellulase production and purification assays re
