Safety and Efficacy of Re-irradiation With Carbon-ion Radiotherapy for Pelvic Recurrence of Rectal Cancer After Preoperative Chemoradiotherapy: A Retrospective Analysis.

BACKGROUND/AIM: Previous evaluation of the safety and clinical efficacy of re-irradiation for pelvic recurrence of rectal cancer after preoperative chemoradiotherapy (PCRT) and rectal surgery is insufficient. We evaluated the safety and efficacy of re-irradiation with carbon-ion radiotherapy (C-ion RT) for pelvic recurrence of rectal cancer after PCRT. PATIENTS AND METHODS: We reviewed the medical records of patients treated with C-ion RT between August 2011 and December 2021 and analyzed the data of seven consecutive patients. The probabilities of overall survival (OS), local control (LC), a

Second infield re-irradiation with a resulting cumulative equivalent dose (EQD2max ) of >180 Gy for patients with recurrent head and neck cancer.

BACKGROUND: For locally recurrent head and neck cancer, re-irradiation with modern techniques like stereotactic body radiotherapy is a promising, possibly curative alternative to palliative systemic therapy. METHODS: We report on 1 patient with nasopharyngeal carcinoma (NPC) and 1 patient with cutaneous squamous cell carcinoma (SCC). Both patients received full dose primary treatment (>66 Gy, EQD2), full dose re-irradiation (>50 Gy, EQD2), and a second course of re-irradiation via robotic radiosurgery (CyberKnife) for a second local recurrence. RESULTS: Both treatments resulted in adequate t

Re-irradiation of adenoid cystic carcinoma: analysis and evaluation of outcome in 52 consecutive patients treated with raster-scanned carbon ion therapy.

BACKGROUND: Treatment of local relapse in adenoid cystic carcinoma (ACC) following prior radiation remains a challenge: without the possibility of surgical salvage patients face the choice between palliative chemotherapy and re-irradiation. Chemotherapy yields response rates around 30% and application of tumouricidal doses is difficult due to proximity of critical structures. Carbon ion therapy (C12) is a promising method to minimize side-effects and maximize re-treatment dose in this indication. We describe our initial results for re-irradiation in heavily pre-treated ACC patients. METHODS:

Re-irradiation in clinical practice: Results of an international patterns of care survey within the framework of the ESTRO-EORTC E2-RADIatE platform.

BACKGROUND: Re-irradiation is an increasingly utilized treatment for recurrent, metastatic or new malignancies after previous radiotherapy. It is unclear how re-irradiation is applied in clinical practice. We aimed to investigate the patterns of care of re-irradiation internationally. MATERIAL/METHODS: A cross-sectional survey conducted between March and September 2022. The survey was structured into six sections, each corresponding to a specific anatomical region. Participants were instructed to complete the sections of their clinical expertise. A total of 15 multiple-choice questions were i

Case Report: Innovative radiotherapy strategies in advanced colon cancer with oligometastasis: a case of online adaptive radiotherapy and lattice stereotactic re-irradiation.

Conventional radiotherapy demonstrates limited efficacy in advanced colon cancer with bulky radio-resistant metastatic lesions. A case report detailing the outcomes and toxicity profile of a 71-year-old female with progressive colon cancer and pulmonary oligometastasis treated using an individualized radiotherapy approach is presented. The patient, with an ECOG performance status of 3, presented severe symptoms including partial intestinal obstruction, intractable cough, and hemoptysis following multiple lines of systemic therapy and prior intensity-modulated radiotherapy (IMRT) to lung metast

Pattern of care for re-irradiation in locally recurrent rectal cancer: a national survey on behalf of the AIRO gastrointestinal tumors study group.

PURPOSE: Radical resection (R0) represents the best curative treatment for local recurrence (LR) rectal cancer. Re-irradiation (re-RT) can increase the rate of R0 resection. Currently, there is a lack of guidelines on Re-RT for LR rectal cancer. The Italian Association of Radiation and clinical oncology for gastrointestinal tumors (AIRO-GI) study group released a national survey to investigate the current clinical practice of external beam radiation therapy in these patients. MATERIAL AND METHODS: In February 2021, the survey was designed and distributed to members of the GI working group. Th

Radiomic signatures reveal multiscale intratumor heterogeneity associated with tissue tolerance and survival in re-irradiated nasopharyngeal carcinoma: a multicenter study.

BACKGROUND: Post-radiation nasopharyngeal necrosis (PRNN) is a severe adverse event following re-radiotherapy for patients with locally recurrent nasopharyngeal carcinoma (LRNPC) and associated with decreased survival. Biological heterogeneity in recurrent tumors contributes to the different risks of PRNN. Radiomics can be used to mine high-throughput non-invasive image features to predict clinical outcomes and capture underlying biological functions. We aimed to develop a radiogenomic signature for the pre-treatment prediction of PRNN to guide re-radiotherapy in patients with LRNPC. METHODS:

Marrow-ablative chemotherapy and re-irradiation are associated with improved overall survival in patients with relapsed central nervous system nongerminomatous germ cell tumors.

PURPOSE: Relapsed/refractory central nervous system nongerminomatous germ cell tumors (NGGCTs) have a poor prognosis. We explored prognostic factors that may guide the management of relapsed/refractory NGGCTs. METHODS: We performed a multinational retrospective study of relapsed/refractory NGGCTs. Data were summarized using standard descriptive statistics. Overall survival (OS) was estimated using the Kaplan-Meier method. Prognostic factors were evaluated with univariable and multivariable Cox proportional hazards models, reported as hazard ratios (HR) with 95% confidence intervals (CI). RESUL

Re-irradiation for intra-thoracic tumours and extra-thoracic breast cancer: dose accumulation, evaluation of efficacy and toxicity based on a literature review.

The improvement seen in the diagnostic procedures and treatment of thoracic tumours means that patients have an increased chance of longer overall survival. Nevertheless, we can still find those who have had a recurrence or developed a secondary cancer in the previously treated area. These patients require retreatment including re-irradiation. We have reviewed the published data on thoracic re-irradiation, which shows that some specific healthy tissues can tolerate a significant dose of irradiation and these patients benefit from aggressive treatment; however, there is a risk of damage to norm

Combined re-radiation therapy and regional or capacitive hyperthermia as a part of multimodal treatment for locoregional breast cancer recurrences in previously irradiated areas.

PURPOSE: Hyperthermia (HT) enhances the effects of radiotherapy (RT) and has shown benefit in recurrent breast cancer. Most existing studies focus on conventional superficial HT, whereas the efficacy of regional hyperthermia (RHT) and capacitive hyperthermia (CHT) in previously irradiated areas remains underexplored. METHODS: Patients with locoregional breast cancer recurrences previously treated with RT were included in this retrospective analysis. Re-irradiation was delivered in combination with RHT or CHT and was integrated into an individualized multimodal treatment concept. HT was applie
