BACKGROUND: Spinal epidermoid cysts are very rare tumors, especially in the thoracic spine; they represent 0.8% of all spinal epidermoids. In adult cases, they are often associated with surgical procedures such as lumbar puncture. We report a rare case of spinal epidermoid cyst in the thoracic spine of an elderly patient who had never undergone lumber puncture, thoracic spinal surgery, or trauma.
CASE DESCRIPTION: A 78-year old woman presented with a 1-month history of rapidly progressive impairment of sensation in both the lower limbs, with gait disturbance. She had no history of spinal surg

August Bier, the father of spinal anesthesia, suffered and reported the first lumbar puncture (LP) headache. On August 24, 1898 his assistant, a Dr. Hildebrandt, attempted to administer a spinal anesthetic to Dr. Bier; it was never completed because the syringe did not fit the already implanted spinal needle. Bier himself suggested that continued leakage of cerebrospinal fluid (CSF) through the dural puncture site was the cause of headache, a theory that has been embraced by the medical community; however, the mechanism is probably more complex. Nearly 50 years ago, J. Lawrence Pool, using an

BACKGROUND AND OBJECTIVES: Intracranial subdural hematoma is a rare complication following spinal anesthesia. The diagnosis is usually difficult because initial symptoms are the same of post-dural puncture headache. The objective was to report a case of early diagnosed subdural hematoma after spinal anesthesia performed with a fine-gauge needle and single puncture.
CASE REPORT: 48-year old female patient, ASA I, undergoing spinal anesthesia for surgery to correct urinary incontinence. The spinal anesthesia was performed with 27G Quincke needle and single puncture. There was no unexpected even

OBJECTIVE: The objective of this study was to compare 24-gauge Sprotte and 25-gauge Quincke needles with respect to post dural puncture headache and backache.
METHODS: Three hundred ASA Physical Status I or II patients scheduled for minor orthopedic or urologic operations under spinal anesthesia were chosen for this randomized, prospective study at a university hospital and a city hospital. Anesthetic technique, intravenous fluids, and postoperative pain therapy were standardized. Patients were randomly divided into three equal groups. Spinal anesthesia was performed with either a 24-gauge Sp

The lumbar puncture is considered as a safe routine procedure in widespread clinical use for nearly a century. To the best of our knowledge, intracranial and intraspinal subarachnoid hemorrhage has never been reported as a complication after a lumbar puncture. We presented a case of a 76-year-old woman who fell in a deep coma after a lumbar puncture with diffuse subarachnoid hemorrhage and acute obstructive hydrocephalus on computed tomographic scans. Magnetic resonance imaging studies of the whole spine showed the hematoma spread along the spinal cord upward to the intracranial subarachnoid s

Spinal muscular atrophy is a genetic motor neuron disease that leads to progressive muscular atrophy and muscle weakness. In December 2016, the Food and Drug Administration, and in June 2017, the European Medicines Agency approved the antisense oligonucleotide nusinersen for treatment of spinal muscular atrophy. Nusinersen has to be repeatedly administered intrathecally. Due to the clinical features of SMA, the application of the ASO by lumbar puncture can be challenging in symptomatic patients considering the frequently observed scoliosis, previous spine fusion surgeries, joint contractures,

BACKGROUND: Lumbar puncture is challenging for patients with scoliosis. Previous ultrasound-assisted techniques for lumbar puncture used the angle of the probe as the needle trajectory; however, reproducing the angle is difficult and increases the number of needle manipulations. In response, we developed a technique that eliminated both the craniocaudal and lateromedial angulation of the needle trajectory to overall improve this technique. We assessed the feasibility and safety of this method in patients with scoliosis and identify factors related to difficult lumbar puncture.

Lumbar puncture (LP) is a crucial procedure in the diagnosis and management of multiple neurologic diseases, and the procedure can be safely performed in most patients. However, dangerous complications may rarely occur, including herniation and death. There remains a paucity of high-quality data identifying patients at increased risk of life-threatening complications from LP, even in the post-neuroimaging era. Therefore, we performed a comprehensive review of the literature to summarize the known physiologic changes and associated complications after LP, especially in the presence of central n

BACKGROUND AND PURPOSE: The C1-2 intrathecal puncture is routinely performed when lumbar puncture is not feasible. Usage has steadily decreased in part because of the perceived high risk of injury to the cervical cord. Up to this point, vague fluoroscopic guidelines have been used, creating uncertainty about the actual needle location relative to the spinal cord. We present a novel osseous landmark to aid in C1-2 intrathecal puncture, corresponding to the posterior spinal cord margin on lateral fluoroscopic views. This landmark, which we have termed the "flare point," represents the triangular

A 62-year-old male presented with urinary incontinence, gait disturbance and dementia for 6 months. Neurological examination revealed severe paraparesis (1/5), sensory disturbance below Th10, neurogenic bladder and absence of patellar and achilles tendon reflexes. CT scan showed mild brain atrophy and symmetric ventriculomegaly with periventricular lucency. Magnetic resonance imaging (MRI) showed a linear flow void lesion on the dorsal surface on the back of his swollen lower spinal cord. Myelography showed a filling defect and flow disturbance of contrast medium in lower thoracic levels, sugg

Post-dural puncture headache (PDPH) often occurs after lumbar puncture during spinal anesthesia, and younger patients tend to be more susceptible to this complication. In this case report, we present a needle-over-needle guidance (NNG) technique utilizing a larger gauge (20G) 3.5" regular spinal needle, referred to as the "Super Introducer," to guide a longer, smaller gauge (25G) 4.7" spinal needle toward the dura in the midline. The "Super Introducer," which is longer than the standard introducer needles found in spinal kits, allows for deeper engagement with the interspinous ligament and pro

Prolonged headache following dural puncture is an uncommon problem that may occur after a spinal tap, often as a complication of epidural anaesthesia. This problem has also been described after long-term epidural or spinal anaesthesia, myelography or spinal surgery. A case of prolonged postdural puncture headache following lumbar nerve root decompression is described in a healthy young man. No other cause could be found either clinically or with the aid of scanning by computerized tomography or magnetic resonance imaging techniques at the spinal level involved. The symptoms were successfully t

Spinal anesthesia is a common procedure performed in orthopedic surgery, and it is regarded as secure and safe. Although puncture-related complication of spinal anesthesia has a very low incidence, it would lead to dramatic neurological damage (tetra- or paraplegia). Early diagnosis and surgical decompression are mandatory to promote a better outcome. We present a case of acute spinal hematoma from T11 to L3, triggered by laborious anesthesia puncture after total knee arthroplasty. A prompt surgical decompression within few hours after diagnosis allowed rapid functional recovery and avoided pe

Three hours after spinal anaesthesia, a previously healthy 71-year-old man developed a lethal intracranial subarachnoid haemorrhage that was very likely due to rupture of a pre-existing aneurysm. Three weeks previously the patient had undergone an operation for which spinal anaesthesia had been administered uneventfully. The question is raised whether there was a causal relationship between the haemorrhage and the spinal anaesthesia; this is discussed in the light of similar cases previously published. Loss of cerebrospinal fluid (CSF) from the puncture site with a subsequent decrease in CSF p

A case of epidermoid tumor of the sacral area with S3 root irritation resulting in bladder dysfunction is presented and its possible relationship to spinal puncture is discussed. Precautions should be taken to prevent this complication by using a stylet instead of a plain needle. The recognition of this complication should always be in the differential diagnosis of a child with back pain, gait abnormality and bladder dysfunction with a history of spinal puncture.

At present, few studies on labor analgesia focus on preventing and managing neurological complications, and there is a lack of specific operational guidelines for clinical practice. This study aims to compare spinal analgesia-epidural analgesia (SA-EA) and combined spinal-epidural analgesia (CSEA) in reducing neurological complications during labor analgesia. SA-EA group: A standard spinal needle (0.5 × 113mm) was first inserted to perform a dural puncture, and 2mL of 0.1% ropivacaine + 1µg/mL sufentanil was injected intrathecally. After the spinal needle was withdrawn, a standard epidural nee

INTRODUCTION: Causes of pain due to spinal metastases have been insufficiently investigated. Tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) were the focus of this study. Both are known as proinflammatory cytokines associated with the pathophysiology of pain syndromes1 ). It is well known that cancer cells produce these cytokines, but whether osteoclasts produce them as well remains unclear. We hypothesize that osteoclasts produce these cytokines; in other words, pain from spinal metastasis is stronger than pain from the primary tumor.
METHODS: We made a rat spinal metastasis model o

Total spinal anesthesia (TSA) is a rare complication of lumbar epidural anesthesia through inadvertent spinal injection of local anesthetics following an undiagnosed dural breach or spinal placement of the catheter. TSA has rarely been reported in children. TSA occurred during epidural anesthesia in a 7-year-old child undergoing abdominal surgery. Recent previous lumbar punctures and intrathecal chemotherapy for Burkitt's lymphoma at the same level may have facilitated dural breach. Epidural anesthesia should not be attempted at the same intervertebral level as prior recent lumbar punctures.

Postdural puncture headache remains a major complication of subarachnoid anaesthesia that bothers the anaesthetist and the young mother. Identification of factors that may affect PDPH after caesarean section under subarachnoid anaesthesia could be critical to policy formulation in an emerging obstetric anaesthesia unit. All patients who had caesarean section under subarachnoid anaesthesia were evaluated in a cross-sectional manner. Only patients in whom 25G Quincke needles were employed for lumbar puncture were recruited for analysis. Patients who met criteria for postdural puncture headache w

This is a report of a case of a subdural haematoma with resulting paraplegia after attempted spinal anaesthesia. Epidural and subdural haematomas are rare complications after central neural blockade. The complication described here was the result of an unsuccessful attempt to puncture the spinal channel. The patient was a 72-year-old woman with a fracture of the left femoral neck, which it was intended to stabilize operatively. Findings that made lumbar spinal puncture difficult were severe overweight, and lordosis and scoliosis of the lumbar spine resulting from degenerative changes. Spinal a
