[Urinary incontinence].

Incontinence is a common age-dependent and increasing problem in women that may mainly present as stress incontinence, overactive bladder, mixed incontinence or other forms. A thorough history, gynaecological and neurological examination and urinalysis as initial step will lead to the diagnosis and treatment. If midstream urine is difficult to receive, a catheter urine will be easy to obtain. Further investigations as urodynamics, cystoscopy and ultrasound may be required. As initial step, stress incontinence should be treated with physiotherapy and pelvic floor exercises, if not successful wi

[Urethrovaginal fistulae resulting from sub-urethral slings for stress urinary incontinence treatment. A report of two cases and review of the literature].

Urethrovaginal fistula is an extremely rare condition after surgical treatment of stress urinary incontinence using a suburethral sling. We report two cases of urethrovaginal fistulae after surgical treatment of stress urinary incontinence using polypropylene monofilament material, that have been managed with clinical evaluation, removal of the sling and treatment of the fistula. It is a rare but severe and invalidating complication of "mini-invasive" techniques, not just for the functional results but also for the patient's social condition. The aim of this article is to describe clinical pre

Sling transection of urethra: a rare complication.

Common postoperative complications associated with suburethral sling procedures include voiding disorders and urinary retention, de novo development of detrusor instability, sling graft rejection and, rarely, erosion of the graft into the urethra. The authors present a case of a late postoperative complication of polytetrafluoroethylene graft erosion and partial transection of the urethra, with resultant acute urinary retention. A 50-year-old patient presented with acute urethral outflow obstruction due to sling graft erosion into the urethra nearly 2 years after she underwent a curative sling

[Surgical treatments for male stress urinary incontinence].

Stress urinary incontinence (SUI) in men is common, radical prostatectomy (RP) remaining its main cause. Despite improvements in surgical techniques, SUI rates range from 5 % to 48 %. Management begins with non-invasive and conservative methods. Surgical treatment includes implantation of an artificial urinary sphincter, a suburethral sling or periurethral balloons. This article summarizes the surgical treatments for male SUI.

The Placement of a Single-Incision Mini-Sling for the Treatment of Complete Intrinsic Sphincter Deficiency in an Adolescent Female: A Case Report.

INTRODUCTION: Intrinsic sphincter deficiency (ISD) is associated with a patient history of urethral injury or childbirth. Suburethral sling placement for ISD has been found to be beneficial in patients with this diagnosis. ISD in the pediatric population is rare and surgical management may prove difficult. ISD requires intensive counseling on available treatment options for this unique population. CASE PRESENTATION: This report is on the use of the single-incision mini-sling for complete ISD in an adolescent patient. The patient was a 15-year-old nulligravid female who was found to have idiop

Single-center retrospective study of the technique, safety, and 12-month efficacy of the MiniArc™ single-incision sling: a new minimally invasive procedure for treatment of female SUI.

This study was conducted to report on the technique, safety, and early efficacy of a single-incision mini-sling to treat female stress urinary incontinence (SUI). Women suffering from SUI were offered a single-incision approach to place a suburethral polypropylene mesh tape in a position similar to that of a transobturator sling without passage of needles through the groin. Retrospective data was collected on 61 patients that received the new MiniArc™ single-incision sling at a single center in the United States. Patient selection and concomitant procedures were determined by the senior author

Is modified in situ vaginal wall sling operation the treatment of choice for recurrent genuine stress incontinence?

PURPOSE: We evaluate objectively the results of a modified in situ vaginal wall sling operation for recurrent genuine stress incontinence and whether it is a substitute for the traditional sling procedure. MATERIALS AND METHODS: A total of 23 patients with urodynamically proved recurrent genuine stress urinary incontinence were recruited in this study. Patients were treated with a modified needle urethropexy technique using an island of in situ vaginal skin as a sling to support the bladder neck and urethra. Surgical outcome was evaluated subjectively and objectively at a median of 15 months.

A preliminary evaluation of in vivo response to a filament-wound macroporous collagen midurethral sling in an ovine model.

Stress urinary incontinence (SUI) impacts ~1/3 of women over age 50. Negative publicity around PP meshes used in pelvic prolapse repair drives the need for identifying alternative biomaterials for SUI repair. Our study evaluated in vivo response to collagen sling implanted in an ovine model. Electrocompacted collagen threads were filament wound as slings and crosslinked in genipin. Collagen slings were implanted suburethrally mimicking the transvaginal tape technique. Main study groups were: Collagen sling (n = 3, 6 months) and PP sling (n = 3, 6 months). Collagen sling was also tested at 3-we

Sling location in women with recurrent stress urinary incontinence following midurethral sling.

OBJECTIVE: Persistent or recurrent stress urinary incontinence (SUI) after a midurethral sling (MUS) may result from incorrect location of the sling relative to the midurethra. This study's objective was to evaluate the incidence of bladder neck (BN) or more proximal MUS in women undergoing reoperation for SUI after synthetic MUS. MATERIAL AND METHODS: A retrospective review was performed of patients referred and treated for isolated recurrent SUI after synthetic MUS (transobturator or retropubic approach). Patients undergoing sling excision for other indications (eg, outlet obstruction, urin

Application of the concept of 'therapeutic index' to surgery for stress incontinence.

The pharmacologic concept of therapeutic index can be applied to surgery for stress incontinence to obtain an objective prediction of surgical outcome. The rationale and mechanics of its application to surgery are discussed in this article. The conclusions support current clinical trends which favor discarding the anterior repair and needle suspension procedures as first-line surgical treatment of stress urinary incontinence in favor of the retropubic procedures. Suburethral sling procedures should be, as now, reserved for second-line surgical treatment of stress urinary incontinence.

Urinary Incontinence After Prostate Treatment: Special Considerations in Geriatrics.

The effects of aging are important in the workup and treatment of incontinence after prostate treatment. While conservative measures should be discussed, geriatric patients can also be offered surgery, as the available treatments are safe and effective in this group. Artificial urinary sphincter implantation is appropriate in those who can operate the device, though they should be followed to ensure they can continue to use the device over time. A suburethral sling can be safe and efficacious in those with milder incontinence and is not affected by mental or physical decline.

Tension-free vaginal tape, Burch, and slings: are there predictors for early postoperative voiding dysfunction?

This study compared the incidence, duration, and predictors of early postoperative voiding dysfunction (VD) after three continence operations. Charts of women undergoing tension-free vaginal tape (TVT), Burch, or suburethral sling were reviewed. Early postoperative VD was defined as urinary retention or postvoid residual (PVR) >200 ml at discharge. Analysis of variance, Chi-square, and non-parametric tests were used. Sixty-three TVT, 42 Burch and 33 slings were identified. Incidence of VD with TVT, sling, and Burch was 50, 24, and 15%, respectively ( P<0.001), and the mean number of days of po
