PURPOSE: This interdisciplinary perspective aims to identify key limitations in current Human-in-the-Loop Optimization (HILO) approaches for lower-limb exoskeletons (LLEs) and to propose a shift toward more human-centred optimization.
MATERIALS AND METHODS: Drawing on literature from wearable robotics, rehabilitation science, motor learning, and clinical practice, we critically discuss three main limitations of current HILO approaches: narrow optimization scope, limited consideration of individual capabilities, and disconnection from personalized training.
RESULTS: We argue that human-center

BACKGROUND: The disconnect between episodic in-clinic assessments and patients goals to improve activity in daily life presents a challenge in rehabilitation. Wearable sensors are promising for addressing this disconnect but are not part of routine practice. The purpose of this study was to develop and test a digital health solution that integrated wearable sensor technology into outpatient rehabilitation.
METHODS: Using an iterative user-centered design approach, a digital solution was built that integrated data from a wrist sensor into the electronic health record (EHR). The feasibility, us

As the digital landscape continues to evolve, access to communication, literacy, and technology have become essential for citizenship, community inclusion, and social and emotional well-being. This paper reflects my personal experience on these topics, exploring the challenges faced by people who lack access to robust language-based expressive augmentative and alternative communication (AAC) tools. Drawing on my background as an educator, I discuss the implications of failing to provide such access, highlighting its effect on literacy skills and social inclusion. I also argue that many AAC tec

BACKGROUND: Globally, mental health (MH) problems increasingly affect young people, contributing significantly to disability and disease. In low- and middle-income countries, such as Colombia, barriers to accessing care exacerbate the treatment gap. In addition, the lack of widespread digital interventions further deepens the digital health divide between the Global North and Global South, limiting equitable access to innovative MH solutions.
OBJECTIVE: This study aims to co-design and develop an MH platform using participatory design methodologies and conduct a 15-month naturalistic observat

Sport with a disability has progressed from undertaking physical activity for recreation to one of a high performance environment at competitions such as the Paralympic Games. There is currently limited information and guidance to help inform stakeholders as to how to develop the high performance technology of elite athletes who possess limb absence. In this manuscript, a conceptual framework for high performance prosthetic limb creation is presented. This utilises a synthesis between contemporary product design theory and a review of existing case studies. This proposed framework is then appl

BACKGROUND: Evidence-based HIV interventions often fail to reach anticipated impact due to insufficient utilization in real-world health systems. Human-centered design (HCD) represents a novel approach in tailoring innovations to fit end-users, narrowing the gap between efficacious interventions and impact at scale.
METHODS: We combined a narrative literature review of HCD in HIV programs with our experience using HCD to redesign an intervention promoting patient-centered care (PCC) practices among health care workers (HCW) in Zambia. We summarize the use and results of HCD in the global HIV

BACKGROUND: The ongoing adoption and use of digital interventions offer promising opportunities to meet the growing demand for mental health support. The effectiveness, implementation, and usage of these interventions depend on how well they are designed and evaluated. However, given the emerging nature of design research in this area, there is still no clear consensus on the specific principles and guidelines for developing digital mental health interventions (DMHIs). There seems to be a lack of clarity regarding the best practices for designing and evaluating these tools.
OBJECTIVE: We aime

Health consumers (i.e., citizens), increasingly review their personal health information on patient portals. However, health information, especially graphs, can be difficult to interpret and use. To investigate reporting attributes that confused participants, what they misinterpreted, and their follow-up intentions, we examined participants' (N = 24) think-aloud responses to a laboratory system graph depicting lab values over time. Overall, 18 participants expressed confusion at least once and 12 misinterpreted one or more aspects of the graph. Common areas of confusion included axis labelling

Models used for the design and service delivery of Augmentative and Alternative Communication (AAC) systems are limited. There are no standardized protocols for gathering user requirements beyond clinical/diagnostic information relating to AAC access needs (i.e., physical and cognitive capabilities). Nonetheless, information on the social, cultural, and psychological aspects of technology orientation and use are important to understanding how an AAC system will complement the user's lifestyle, personal goals, values, and activities. Persona development is a user-centered design method that cre

INTRODUCTION: Infertility is a chronic bio-psycho-social condition associated with substantial psychological distress, particularly among women. High rates of anxiety, depression, reduced quality of life, low self-efficacy, and impaired resilience have been widely reported, while access to psychological support remains limited due to financial, social, and stigma-related barriers. Web-based mental health interventions offer a promising, accessible, and cost-effective approach to address these unmet needs. This study aims to develop and evaluate the effectiveness of a user-centered, web-based p
