Intravenous magnesium for pediatric sickle cell vaso-occlusive crisis: methodological issues of a randomized controlled trial.

Multiple recent Sickle Cell Disease studies have been terminated due to poor enrollment. We developed methods to overcome past barriers and utilized these to study the efficacy and safety of intravenous magnesium for vaso-occlusive crisis (VOC). We describe the methods of the Intravenous Magnesium in Sickle Vaso-occlusive Crisis (MAGiC) trial and discuss methods used to overcome past barriers. MAGiC was a multi-center randomized double-blind placebo-controlled trial of intravenous magnesium versus normal saline for treatment of VOC. The study was a collaboration between Pediatric Hematologists

What stops us from healing the healers: a survey of help-seeking behaviour, stigmatisation and depression within the medical profession.

BACKGROUND: Doctors are poor at help-seeking, particularly for mental ill health; attitudes of colleagues reflecting stigmatisation may be important factors influencing decisions to seek support. AIMS: This article focuses on doctors' attitudes to depression rather than mental illness in general. It seeks to determine the extent to which doctors perceive depression is stigmatised within the medical profession and whether the level of perceived stigma affects patterns of help-seeking behaviour. METHOD: : A postal survey was sent to 1488 General Practitioners and 152 psychiatrists in Devon and

Faculty encounters with uncivil nursing students: an overview.

The critical incident technique (CIT) was used to explore faculty experiences with uncivil nursing students. Twenty-one nursing faculty with various years of experience in teaching were interviewed to ascertain what they considered critical incidents of uncivil encounters with nursing students and what effect those encounters had on them. Thirty-six encounters were described by the faculty. Of the 36 encounters, 33 occurred with individual students and 3 occurred with groups of students. Twenty-three encounters occurred in the context of poor student performance requiring constructive criticis

Integration of Endodontic Services Into Preventive Oral Health Care: A Narrative Review.

BACKGROUND: The integration of endodontic services into preventive oral health care has become increasingly important as dental caries and pulpal pathologies remain major contributors to the global burden of oral disease. However, despite advances in preventive dentistry, gaps persist in the early identification and management of pulpal disease within primary care systems. Integrating endodontic services may enhance early diagnosis, timely intervention, and long-term tooth preservation. METHODS: A narrative review of published literature was conducted to examine current models, strategies, an

Optimization of Pharmacist Medication Management and Mortality in the Intensive Care Unit.

BACKGROUND: Medication-related morbidity due to inappropriate prescribing, delays in appropriate treatment, and adverse drug events contributes to ICU patient mortality. Comprehensive medication management (CMM) is a care standard provided by pharmacists in collaboration with the interprofessional team. Optimizing ICU pharmacist workload via the pharmacist-to-patient ratio while ensuring daily CMM may reduce mortality. RESEARCH QUESTION: Is ICU pharmacist staffing, measured by the pharmacist-to-patient ratio and absence of CMM, associated with in-hospital mortality among critically ill adults

Management of eating disorders for people with higher weight: clinical practice guideline.

INTRODUCTION: The prevalence of eating disorders is high in people with higher weight. However, despite this, eating disorders experienced by people with higher weight have been consistently under-recognised and under-treated, and there is little to guide clinicians in the management of eating disorders in this population. AIM: The aim of this guideline is to synthesise the current best practice approaches to the management of eating disorders in people with higher weight and make evidence-based clinical practice recommendations. METHODS: The National Eating Disorders Collaboration Steering

[Syncope and professional driving: the role of the occupational physician].

BACKGROUND: Road traffic injuries constitute one of the main causes of death and disability in Italy and in the European Union. Although much scientific evidence exists on the important role of complete loss of consciousness in the dynamics of motor vehicle accidents, at present there is no specific legislation in Italy. Occupational medicine should take particular care in this field, because every year a large number of accidents with fatal outcomes occur among truck drivers. Via health surveillance, the occupational physician can play an important role in the prevention of such events. OBJE

Palliative Social Workers' Contributions to Cultivating Prognostic Awareness: A Qualitative Study Using the Prognostic Awareness Impact Scale Framework.

BACKGROUND: Prognostic awareness is central to informed, value-concordant care for patients with serious illness. Although often conceptualized as cognitive understanding of a time-based prognosis, emerging research highlights prognostic awareness as a multidimensional process involving cognitive understanding, emotional coping, and adaptive response. Palliative social workers may be especially well positioned to support this process, yet their specific skills and strategies remain understudied. OBJECTIVE: To describe the strategies hospital-based specialty-level palliative social workers use
