Percutaneous radial access for peripheral transluminal angioplasty.

OBJECTIVE: The radial approach is currently gaining popularity in the setting of coronary percutaneous transluminal angioplasty (PTA) because it decreases the incidence of vascular complications. This study reports our initial results with radial access for peripheral PTA. METHODS: Between November 2011 and January 2014, we performed peripheral PTA in 526 patients. PTA was performed through left radial access in 24 ambulatory patients (4.6%) presenting with TransAtlantic Inter-Society Consensus A or B lesions on preoperative computed tomography angiography. Materials included a 110-cm-long in

Endovascular techniques in limb salvage: infrapopliteal angioplasty.

Critical limb ischemia (CLI) results from inadequate blood flow to supply and sustain the metabolic needs of resting muscle and tissue. Infragenicular atherosclerosis is the most common cause of CLI, and it is more likely to develop when multilevel or diffuse arterial disease coincides with compromised run-off to the foot. Reports of good technical and clinical outcomes have advanced the endovascular treatment options, which have gained a growing acceptance as the primary therapeutic strategy for CLI, especially in patients with significant risk factors for open surgical bypass. In fact, endov

Embolic capture angioplasty in peripheral artery interventions.

PURPOSE: To report a single-center experience with the Proteus embolic capture angioplasty balloon catheter for the treatment of symptomatic lower extremity peripheral artery disease. METHODS: Between June 2010 and January 2012, 36 lower extremity lesions were treated in 30 patients (29 men; mean age 65±9 years) with symptomatic peripheral artery disease. Nearly half of the patients had diabetes mellitus. The mean lesion length was 140 mm, and three quarters of the lesions were considered complex owing to severe calcification, chronic total occlusion (≥3 months), in-stent stenosis, and/or len

Cerebral intolerance during flow arrested carotid angioplasty.

INTRODUCTION: The use of flow arrest as a means of providing cerebral protection during carotid angioplasty offers the advantages of improved efficiency of debris removal and the ability to provide protection under unfavorable (tortuous) anatomic circumstances. However, in contrast to the filtration methods of cerebral protection, this modality requires complete interruption of antegrade carotid artery flow during balloon angioplasty and stent deployment. METHODS: We report our experience with 9 patients undergoing carotid angioplasty with the Mo.Ma device, which utilizes common and external

Subintimal angioplasty for femoro-popliteal occlusive disease.

There has been a longstanding debate about the roles of surgical bypass graft, percutaneous transluminal angioplasty, subintimal angioplasty, and conservative management for femoro-popliteal occlusive disease. Subintimal angioplasty was first described in 1987 as a method of performing an endovascular arterial bypass. The subintimal space at the start of the occlusion is entered with a catheter and a wire loop is used to cross the occlusion and reenter the vessel lumen distally. In patients with critical limb ischemia, there is high quality evidence demonstrating that the limb salvage rate and

Preventing restenosis after angioplasty: a multistage approach.

Arterial reconstruction procedures, including balloon angioplasty, stenting and coronary artery bypass, are used to restore blood flow in atherosclerotic arteries. Restenosis of these arteries has remained a major limitation of the application of these procedures, especially in the case of balloon angioplasty. Post-angioplasty restenosis results from two major processes: neointimal formation and constrictive remodelling. Neointimal formation is initiated by arterial injury with a resultant loss of contractile phenotype in tunica media, leading to VSMC [vascular SM (smooth muscle) cell] migrati

[Management of renal complications during primary angioplasty].

Primary angioplasty represents the best strategy for myocardial reperfusion in ST-elevation acute myocardial infarction. However, it is associated with a high risk of developing contrast-induced nephropathy, a renal complication coupled with markedly increased morbidity and mortality rates. This paper summarizes, on the current evidence available, the clinical and prognostic relevance of renal insufficiency and contrast-induced nephropathy in acute myocardial infarction, and emphasizes new possible preventive strategies for kidney protection in this clinical setting.

Intracranial percutaneous transluminal angioplasty for arteriosclerotic stenosis.

BACKGROUND: Patients with intracranial arteriosclerotic disease have significant morbidity and mortality rates, and some are unresponsive to medical treatment and have unacceptable surgical risks. Percutaneous transluminal angioplasty of the intracranial vessels is a possible alternative to surgery. OBJECTIVES: To present our experience with percutaneous transluminal angioplasty and to summarize our data. PATIENTS AND METHODS: Sixteen patients underwent intracranial percutaneous transluminal angioplasty for high-grade arteriosclerotic stenosis based on strict inclusion and exclusion criteria

Percutaneous transluminal angioplasty for intracranial arteriosclerotic lesions.

We report 17 cases of intracranial arterial stenosis treated by percutaneous transluminal angioplasty (PTA), including 9 on the intracranial internal carotid (ICA), 4 on the middle cerebral (MCA), and 4 on vertebrobasilar artery (VBA) system. All patients had ischaemic brain symptoms and stenoses of more than 60% (calculated angiographically). We treated four patients by PTA for residual stenoses after thrombolysis for acute occlusion. We used PTA balloon catheters 2.0-3.5 mm in diameter for all procedures. As a rule, the balloon was inflated for 1 min at 6 atm. All arteries were successfully

Hemodialysis access graft stenosis: percutaneous transluminal angioplasty.

Maintenance of dialysis access continues to plague care of the patient with ESRD. Because of the poor outcomes from surgical revision, there is increasing interest in balloon angioplasty as a technique for dilating stenoses in the functioning, but compromised graft. Forty patients treated with percutaneous transluminal angioplasty (PTA), without subsequent intervention, for graft dysfunction were retrospectively studied. Time to thrombosis of the graft was noted. Patency was determined using the Kaplan-Meier technique. Patency following PTA at 1, 6, and 12 months was 76, 27, and 10%. Patency f

Improvement in cerebral hemodynamics after carotid angioplasty.

BACKGROUND AND PURPOSE: Carotid percutaneous transluminal angioplasty ( PTA) may offer an alternative treatment to carotid endarterectomy. However, in contrast to carotid endarterectomy, which has been shown to normalize impaired cerebral hemodynamics, the effects of carotid PTA are unknown. Therefore, we prospectively studied the effect of carotid PTA on both perioperative and postoperative cerebral hemodynamics. METHODS: Eleven patients undergoing carotid PTA for symptomatic carotid artery stenosis were prospectively studied. Transcranial Doppler recordings from the ipsilateral middle cereb

Bacteremia associated with percutaneous transluminal coronary angioplasty.

Bacteremia after diagnostic cardiac catheterization is uncommon, but bacteremia after percutaneous transluminal coronary angioplasty (PTCA) has not been studied prospectively. Unlike diagnostic cardiac catheterization, PTCA involves the use of an indwelling arterial sheath after completion of the procedure, which is connected to a pressurized heparin solution, both of which increase the risk of local infection and/or bacteremia. During a 16-week period, we prospectively evaluated patients undergoing 164 PTCA procedures in order to determine the frequency of bacteremia and the significance of f

Percutaneous transluminal angioplasty: delayed response in neurofibromatosis.

Percutaneous transluminal angioplasty (PTA) was eventually successful in a patient with renal artery stenosis due to neurofibromatosis. Although the immediate postangioplasty appearance showed little improvement, the residual stenosis has completely resolved at 5 months. Delayed response to PTA has been previously documented in other types of vascular disease. Its occurrence in renal artery stenosis due to neurofibromatosis emphasizes the importance of long-term follow-up and may be a factor in the poor short-term results that some have reported for PTA in patients with this condition.

[Percutaneous transluminal angioplasty in subclavian artery stenosis].

The technique of percutaneous transluminal angioplasty (PTA) that was introduced initially by Dotter and Judkins in 1964 has been applied to various arterial stenoses and some occlusive lesions and has advanced dramatically with the development of the Grüntzig catheter in the treatment of peripheral vascular disease. However, the application to occlusive disease in brachiocephalic arteries is still considered to be disputable and has been made only in selective cases, especially in Japan. We report here a case with stenosis of the left subclavian artery successfully treated with PTA. The patie
