The role of chronogenetics and of gemellology in the prevention of genetic morbidity.

The prevention of hereditary diseases after birth is essentially based on the detection of these diseases at their preclinical level. This may be made possible through a careful pedigree examination and possible identification of a "genealogical alarm", as well as a detailed clinical examination of the subject and possible identification of preclinical signs of the pathogenesis, i.e., of a "clinical alarm". The respective roles of chronogenetics and of gemellology in this approach are reviewed.

High false alarm rates on a vigilance task may indicate recreational drug use.

Neuropsychologists need more sensitive methods to detect and measure recreational drug use in both research and clinical settings. In a study comparing the sensitivity of information processing tasks and neuropsychological instruments to detect early HIV-related cognitive decrements, 18 of 129 subjects tested positive for recreational drugs. Sixteen of these 18 subjects had elevated false alarm rates on one of the information processing tasks, the vigilance task. Another 45 subjects who tested negative for recreational drugs also had elevated false alarm rates. Neuropsychological measures of p

Subdomain Investigation of Executive Functioning in Emerging Adults With and Without ADHD Symptoms.

ADHD often persists from childhood into adulthood yet understanding of the neuropsychological profile of ADHD in emerging adults (18-24 years) is limited and inconsistent. Both diagnosed individuals and those with subthreshold symptoms often struggle with executive functioning. This study examined executive function subdomains in emerging adults with ADHD symptoms. Participants included 71 emerging adults (45 women, 24 men, 2 other) who completed computerized tasks assessing working memory, response inhibition, set shifting, and sustained attention; and questionnaires of ADHD symptoms and cogn

Technical considerations for evaluating clinical prediction indices: a case study for predicting code blue events with MEWS.

Objective. There have been many efforts to develop tools predictive of health deterioration in hospitalized patients, but comprehensive evaluation of their predictive ability is often lacking to guide implementation in clinical practice. In this work, we propose new techniques and metrics for evaluating the performance of predictive alert algorithms and illustrate the advantage of capturing the timeliness and the clinical burden of alerts through the example of the modified early warning score (MEWS) applied to the prediction of in-hospital code blue events.Approach. Different implementations o

[About safety parameters for patient-controlled analgesia (PCA) devices].

During the course of preparation of an opioid prescription, the nurse in charge became aware that the patient-controlled analgesia (PCA) syringe driver did not permit programming for the delivery as required: a maximum bolus number (Bmax) was indicated but only a maximum cumulative dose (Dcmax) could be programmed. The prescription dose criteria were consistent with the guidelines of the French societies of palliative care, anesthesiology, and reanimation (Société française d'accompagnement et de soins palliatifs [Sfap] and Société française d'anesthésie réanimation [Sfar]). A Dcmax dose simul

Clinical impact of upper endoscopy in the management of patients with gastroesophageal reflux disease.

OBJECTIVE: Esophago-gastro-duodenoscopy (EGD) is frequently performed in patients with heartburn. However, the benefit of an EGD is unclear. The objective is to determine the clinical impact of performing an EGD in the management of GERD. METHODS: Patients with alarm symptoms or persistent heartburn from primary care and gastroenterology clinics were invited to participate. Patients with known reflux complications or an EGD procedure within 3 yr were excluded. Endpoints for EGD-improving management were prospectively defined: change in medical therapy, dilating esophageal stricture, identifyi

Validation of the Rome III criteria and alarm symptoms for recurrent abdominal pain in children.

OBJECTIVES: Rome criteria were formulated to define functional gastrointestinal disorders (Rome III criteria, 2006) excluding organic diagnoses when alarm symptoms were absent. The aims of the study were to validate the Rome III criteria as to their capacity to differentiate between organic and functional abdominal pain and to assess the role of alarm symptoms in this differentiation. METHODS: During 2 years all of the patients (ages 4-16 years) presenting with recurrent abdominal pain (Apley criteria) and referred to secondary care were included. Clinical diagnoses were based on protocolized

The effect of arousal on auditory threat detection and the relationship to auditory hallucinations.

BACKGROUND AND OBJECTIVES: A recent model of a subtype of auditory verbal hallucinations (AVHs) has proposed such experiences may result from increased arousal altering auditory threat perception. METHODS: This study considered this theory using undergraduate students who undertook a new experimental paradigm, the Auditory Threat Discrimination Task (ATDT). This examined the effects of arousal on auditory threat perception (Study 1), and its relation to hallucination-proneness (Study 2). RESULTS: Study 1 (n = 66) found evidence that the high, as compared to low-arousal condition, was associa

Cross-Cultural Adaptation and Validation of the Japanese Charité Alarm Fatigue Questionnaire (CAFQa) Among ICU Nurses and Physicians: A Multicentre Study.

Alarm fatigue threatens patient safety in intensive care units (ICUs), yet no validated Japanese instrument exists. We cross-culturally adapted the Charité Alarm Fatigue Questionnaire (CAFQa) into Japanese following COSMIN guidelines and evaluated it in 129 clinicians (103 nurses, 26 physicians) at five hospitals, with 102 retested. Confirmatory factor analysis supported the two-factor structure (CFI = 0.922; RMSEA = 0.041; SRMR = 0.076), but the inter-factor correlation was near zero (r = 0.05), suggesting that Alarm Stress and Alarm Coping behave as largely independent dimensions. A novel ECG detector performance metric and its relationship with missing and false heart rate limit alarms.

PURPOSE: Performance of ECG beat detectors is traditionally assessed on long intervals (e.g.: 30min), but only incorrect detections within a short interval (e.g.: 10s) may cause incorrect (i.e., missed+false) heart rate limit alarms (tachycardia and bradycardia). We propose a novel performance metric based on distribution of incorrect beat detection over a short interval and assess its relationship with incorrect heart rate limit alarm rates. BASIC PROCEDURES: Six ECG beat detectors were assessed using performance metrics over long interval (sensitivity and positive predictive value over 30mi

The Impact of Reduced Pulse Oximetry Use on Alarm Frequency.

BACKGROUND: Concerns about alarm fatigue prompted The Joint Commission to issue a Sentinel Event Alert urging hospitals to minimize alarms. We previously conducted a quality improvement project on a single unit that reduced time on continuous pulse oximetry, a common source of physiologic monitor alarms, for patients with wheezing (ie, asthma and bronchiolitis, wheezing-associated respiratory infections). OBJECTIVE: To study the impact of our improvement work on overall physiologic monitor alarm frequency for these patients. METHODS: This was a retrospective cohort study at a freestanding ch

Use of Retrospective Continuous Glucose Monitoring Data Is Underrated and Underused.

Access to real-time glucose data in real time allows users to make informed decisions about their insulin dosages, appropriate carbohydrate intake, exercise, and other health behaviors. Programmable alarms and alerts warn users about current and/or impending acute glycemic events. However, the value and utility of retrospective data analysis for clinical decision-making have gone mostly unrecognized. This article presents a series of patient case examples that illustrate how our use of retrospective data enables us to identify and effectively address patterns of problematic glycemia.

Melodic algorithms for pulse oximetry to allow audible discrimination of abnormal systolic blood pressures.

An anesthesiologist must remain vigilant of the patient's clinical status, incorporating many independent physiological measurements. Oxygen saturation and heart rate are represented by continuous audible tones generated by the pulse oximeter, a mandated monitoring device. Other important clinical parameters--notably blood pressure--lack any audible representation beyond arbitrarily-configured threshold alarms. Attempts to introduce further continuous audible tones have apparently foundered; the complexity and interaction of these tones have exceeded the ability of clinicians to interpret them
