Stuck in Despair: Cognitive Behavioral Therapy Informed Communication for Serious Illness Care.

Patients with serious medical illnesses may respond to the challenges of their illness in unique ways, shaped by personal, relational, and contextual factors. For many patients, depression and anxiety are common and may feature avoidance or withdrawal, negative expectations for outcomes, repetitive negative thinking (rumination and/or worry), sad mood, and/or irritability. These symptoms often arise from a complex and bidirectional interplay between a patient's emotions, thoughts, and behaviors. This dynamic system provides clinicians with multiple, clinically actionable entry points to foster

From stereotyped recognition to situated meaning: university students' retrospective accounts of perceived schema revision in learning Jiaxing revolutionary heritage.

This qualitative interview study examines how university students retrospectively narrate perceived schema revision in learning Jiaxing revolutionary heritage, a politically and culturally saturated local heritage context associated with South Lake and the Red Boat. Schema revision was used as a sensitizing analytical lens rather than as a direct measure of cognitive change. Thirty Mandarin interview transcripts from students with varied learning experiences were analyzed through a hybrid deductive-inductive codebook thematic analysis conducted by three coders. Participants commonly recalled e

Anxiety sensitivity moderates the relationship of changes in physiological arousal with flight anxiety during in vivo exposure therapy.

Physiological sensations and discomfort constitute the major symptoms reported by aviophobics. Anxiety sensitivity (AS) seems to moderate the relationship between self-reported somatic sensations and flight anxiety, and AS has been identified as a vulnerability factor for flight phobia. In this study we examined whether AS moderates the effects of somatic sensations and autonomic nervous system reactivity on flight anxiety induced by real flight. In fifty aviophobics participating in Cognitive Behaviour Group Therapy (CBGT), flight anxiety, somatic sensations and autonomic nervous system react

Effectiveness of an empirically supported treatment for social phobia in the field.

This study examined the effectiveness of individual exposure combined with cognitive restructuring for social phobia in a clinical setting as well as the influence of sample restriction criteria on the effect size. Participants were 217 unselected patients with a primary diagnosis of social phobia who were treated by 57 therapists in four outpatient clinics of the Christoph-Dornier-Foundation of Clinical Psychology in Germany. Results 6 weeks after the end of therapy showed highly significant reductions in social phobic fears and avoidance as well as in general anxiety and symptoms of depressi

Neurometabolic and vascular dysfunction as an early diagnostic for Alzheimer's disease and related dementias.

INTRODUCTION: Recent studies suggest that the brain undergoes anatomical and functional restructuring, resulting in neurometabolic and vascular dysregulation (MVD) prior to amyloid beta accumulation, which begins at an early age and leads to the onset of Alzheimer's disease (AD). METHODS: Using a retrospective clinical population (n = 403) from the Alzheimer's Disease Neuroimaging Initiative, cerebral perfusion and metabolism changes across 59 brain regions were evaluated from clinical studies. Results were verified by transcriptomic signatures and clinical cognitive assessments. RESULTS: Ou

Cognitive-behavioural assessment and treatment of maladaptive help-seeking behaviour in a patient with schizophrenia.

OBJECTIVE: Maladaptive help-seeking behaviour in psychiatric patients is a significant problem for public mental health services, yet it is not addressed in the mainstream literature. We present a report on the successful treatment of a person with schizophrenia who displayed this common dilemma for patients and clinicians. CLINICAL PICTURE: A 31-year-old man with borderline intellectual functioning had a 10-year history of schizophrenia marked by negative features. He frequently presented in crisis to public mental health services, the local hospital, and his general practitioner; this resul

The Beneficial Effects of Combined Exercise and Polyphenols in Alzheimer's Disease.

Regular exercise enhances life quality, lowers the risk of cognitive damage, and slows the advancement of Alzheimer's disease (AD). Natural compounds rich in polyphenols have garnered attention as a non-pharmacological means of treating and preventing AD. The primary component of wine, grape seeds, and nuts is polyphenols. Research suggests that polyphenols slow down the rate of neurodegeneration in AD and lessen learning impairment. Furthermore, polyphenols lessen brain impairments related to cognition. Additionally, polyphenols can specifically restructure amyloid-β (Aβ) structures and solub

The Eyes as a Mirror of the Unconscious: What Pupil Dilation Reveals About Unconscious Analytic Thought in Insight Problem Solving.

Recent studies suggest forms of unconscious processing in classical insight problem solving. In particular, the unconscious analytic thought (UAT) approach speculates that the creative act of restructuring such very demanding problems implies a form of high-level unconscious thought, mainly active during incubation. The present study aims to investigate whether pupillary dilation, typically associated with conscious cognitive effort, can also represent an indicator of unconscious cognitive effort (UAT). In the experimental condition, participants attempted to solve the problem and, during the

[Behavioral response of older women in a stochastic environment].

The article presents the results of studies of the behavioral response in 63 elderly women. With the help of a computer complex for psychophysiological studies KPFK-99 "Binatest" we estimated figures of restructuring strategies of decision making in a stochastic environment. The leading psychophysiological mechanisms of behavioral response of older women in a stochastic environment are presented as a high-speed component of decision-making process, and search activity as a dynamic component of motivational orientation dominates over the informational component of the decision-making on the tem

Grey glasses: sadness in young women.

Sadness as a developmental phenomenon in young women was discovered in a study designed to identify developmental transitions and the relationship of these transitions to mental health. Sadness is conceptualized as a pervasive feeling of disillusionment and unhappiness that influences the meaning of life events and decision-making. Field research and interpretive analysis techniques were used to observe, describe and analyse the life experiences of 25 volunteer participants. For most women in the study, sadness was found to be time-limited and perhaps had beneficial effects. Some women, howeve

Cognitive Behavioral Therapy for Social Anxiety Disorder: Predictors of Treatment Outcome in a Quasi-Naturalistic Setting.

We examined the outcomes of individual cognitive behavioral therapy (CBT) for social anxiety disorder (SAD) in a sample of 93 adults seeking treatment in a university outpatient clinic specializing in CBT for SAD. Treatment followed the structure of a manual, but number of sessions varied according to client needs. After approximately 20 weeks of therapy, patients' social anxiety had decreased and their quality of life had increased. Patients with more severe SAD or comorbid major depressive disorder (MDD) at pretreatment demonstrated higher levels of social anxiety averaged across pre- and po

Longitudinal case-studies of developmental dyslexia in Norwegian.

This study examined retrospectively the preschool cognitive and linguistic profiles and emergent literacy skills in four Norwegian dyslexic children. The aim was to identify prognostic indicators that were associated with the reading impairments observed in an earlier study of these children. In comparison to a control group of at-risk children who were normal readers at age 10, three of the four dyslexic children exhibited either stagnation or a decline in speech accuracy in the presence of a vocabulary growth spurt at age 2-3 years. Skills in phonological awareness seemed to vary inconsisten

Family therapy versus individual therapy for adolescent females with anorexia nervosa.

Behavioral family systems therapy (BFST) was compared with ego-oriented individual therapy (EOIT) in a controlled, random-assignment investigation involving 22 young adolescents with anorexia nervosa. Each adolescent and her parents received approximately 16 months of outpatient therapy along with a common medical and dietary regimen. BFST emphasized parental control over eating and weight gain, coupled with cognitive restructuring and problem-solving communication training. EOIT emphasized building ego strength, adolescent autonomy, and insight into the emotional blocks to eating. BFST produc

Negative Cognitions After the Death of a Close Person: Time-Varying and Time-Invariant Components and Their Associations with Prolonged Grief.

The loss of a significant other may shatter positive or neutral cognitions about the self, life, and the future, confirm preexisting negative cognitions in these areas, or lead to threatening misinterpretations about one's own grieving. Negative cognitions are a risk factor for the development of prolonged grief disorder (PGD). Using latent trait-state occasion (TSO) modeling, the aim of this study was to examine the longitudinal structure of negative loss-related cognitions over time, and to examine the associations between time-invariant (TI; trait) and time-variant (TV; occasional) componen

Palmtop computer-assisted group therapy for social phobia.

This article describes the application of group computer-assisted therapy for social phobia. The computer program includes a diary function for ongoing self-monitoring of anxiety as well as guidance on the practice of relaxation, cognitive restructuring, and self-control desensitization. Although the program was originally designed to treat individuals with generalized anxiety disorder (GAD), it was hypothesized that the program also would be effective for individuals with social phobia; therefore, it was implemented in a group of individuals with a primary diagnosis of GAD or social phobia. Emotion regulation related neural predictors of cognitive behavioral therapy response in social anxiety disorder.

Social anxiety disorder (SAD) is characterized by aberrant prefrontal activity during reappraisal, an adaptive cognitive approach aimed at downregulating the automatic response evoked by a negative event. Cognitive behavioral therapy (CBT) is first-line psychotherapy for SAD, however, many remain symptomatic after treatment indicating baseline individual differences in neurofunctional activity may factor into CBT outcome. An emotion regulation strategy practiced in CBT is cognitive restructuring, a proxy for reappraisal. Therefore, neural response during reappraisal may serve as a brain-based
