Access to contraception in pharmacies during the COVID-19 pandemic.

BACKGROUND: As a result of the coronavirus disease 2019 (COVID-19) pandemic, shifts in traditional contraception access points have presented new challenges, leading people to seek alternative sources of contraception care, including pharmacies. Pharmacists in one-fourth of U.S. states are able to prescribe hormonal contraception, a model known as pharmacy access. Pharmacy access became available in California in 2016 and in Colorado in 2017. OBJECTIVE: To characterize how access to contraception products and services in pharmacies changed during the COVID-19 pandemic, including pharmacist pr

The community pharmacy eras tour: …Are we ready for it? Why our next era requires leaning into community-based pharmacy post-graduate training programs.

Community-based practice is an area of pharmacy that has accelerated its innovation significantly in the last decade. Pandemics, policies, and the needs of our communities have challenged this innovation. Post-graduate community-based pharmacy training programs (CPTPs) allow for community-based pharmacies to continue the work they are doing while developing new initiatives with trainees who are driven by learning, implementation, and creativity to create deliverables and complete projects during their training program. CPTPs are needed to fuel and support our profession if we want to continue

Qualitative exploration of the barriers and facilitators to community pharmacy PrEP delivery for UK pharmacists and underserved community members using the COM-B model of behaviour change.

OBJECTIVES: Expanding delivery of oral pre-exposure prophylaxis (PrEP) to community pharmacies could improve access, aligning well with the UK government's goals to eliminate new HIV acquisitions by 2030. Using the Capability, Opportunity, Motivation, Behaviour (COM-B) model for behaviour change, the aim of this research was to explore the barriers and facilitators of community pharmacy PrEP delivery, for pharmacists and community members. METHODS: Community members at elevated risk of acquiring HIV and community pharmacists were recruited to participate in semi-structured interviews. Intervi

Evaluation of a Collaborative Practice Agreement Intervention to Improve Access to HIV Prevention Services in Southeastern US Pharmacies: Protocol for a Hybrid Type 1 Effectiveness-Implementation Study.

BACKGROUND: The United States (US) Southeast experiences a disproportionate burden of HIV, compounded by limited access to pre-exposure prophylaxis (PrEP) services due to systemic barriers, such as stigma, medical mistrust, and restrictive pharmacy policies. Collaborative practice agreements (CPAs) between pharmacists and clinicians represent a promising strategy to improve PrEP accessibility through pharmacy-based services. OBJECTIVE: The Collaborative Agreement-based PrEP Using Pharmacies (CAP-UP) study aims to evaluate the development, implementation, and effectiveness of a CPA template an

Research Priorities for Expansion of Opioid Use Disorder Treatment in the Community Pharmacy.

In the last decade, the U.S. opioid overdose crisis has magnified, particularly since the introduction of synthetic opioids, including fentanyl. Despite the benefits of medications for opioid use disorder (MOUD), only about a fifth of people with opioid use disorder (OUD) in the U.S. receive MOUD. The ubiquity of pharmacists, along with their extensive education and training, represents great potential for expansion of MOUD services, particularly in community pharmacies. The National Institute on Drug Abuse's National Drug Abuse Treatment Clinical Trials Network (NIDA CTN) convened a working g

Community pharmacists' response to complaints of gastroesophageal reflux: A simulated patient study in the Northern United Arab Emirates.

INTRODUCTION: Patients frequently use gastric acid-reducing agents (ARAs) to treat symptoms affecting the gastrointestinal tract. Thus, the risk for drug-drug interactions (DDI) is a serious concern. This potentially makes the community pharmacist (CP) act as a primary intervention by providing the appropriate counseling and dispensing practice. OBJECTIVE: To evaluate CPs' counseling and dispensing practices regarding complaints of Gastroesophageal Reflux Disease (GERD), including recommending an appropriate course of action to prevent possible DDIs. MATERIALS AND METHODS: A simulated patien

Exploring community pharmacists' clinical decision-making using think aloud and protocol analysis.

BACKGROUND: Clinical decision-making (CDM) is the dynamic process used to gather, interpret and evaluate data to select an evidence-based choice of action. As the role of the pharmacist becomes more patient-focused, effective CDM skills are increasingly vital to achieve patient outcomes centred on quality, safety and efficacy. OBJECTIVE: To examine how community pharmacists reason through a clinical problem and what factors assist or hinder decision-making. METHOD: Fifteen New Zealand registered community pharmacists in central Auckland were presented with a bacterial conjunctivitis case. Th

Exploring oral healthcare management across Australian community pharmacies using case vignettes.

OBJECTIVES: In Australia, more than 80% of pharmacists and pharmacy assistants are consulted for oral healthcare advice up to five times or more each week. This study assesses Australian community pharmacy staff's (pharmacists and pharmacy assistants) ability to recognize and manage common oral health presentations using case vignettes, and determine whether appropriate identification and management are associated with self-reported confidence. METHODS: Five oral healthcare case vignettes were developed based on previous literature by a multidisciplinary team of dental and pharmacy practition

Can private pharmacy providers offer comprehensive reproductive health services to users of emergency contraceptives? Evidence from Nairobi, Kenya.

OBJECTIVE: To evaluate the provision of reproductive health information and services to users of emergency contraceptives (ECs) by private pharmacists. METHODS: The study involved intervention (9) and control (8) pharmacies, with baseline and endline assessments of EC provision through the use of mystery clients. Intervention pharmacies received weekly updates on EC, fliers with three key messages on EC, and information, education, and communication materials. Logistic regression models are estimated to predict the provision of reproductive health services to EC clients. RESULTS: The differe

Ischemic stroke of unusual cause: clinical features, etiology and outcome.

BACKGROUND: Clozapine is the most effective antipsychotic for treatment-resistant schizophrenia, although serious adverse effects such as agranulocytosis and cardiomyopathy limit its use. In July 2015, Australian regulations changed to allow community-based prescribing and supply of clozapine for maintenance therapy. However, there is currently no information on the rate of clozapine services available in Australian community pharmacies, or the factors that influence a pharmacist's decision to provide, or not provide, a clozapine service, particularly from the perspective of those pharmacists

Acceptability, Appropriateness, and Feasibility of Administering Long-Acting Injectable Antipsychotics in Community Pharmacies.

OBJECTIVE: Long-acting injectable antipsychotics (LAIAs) are evidence-based treatments for schizophrenia with demonstrated improved adherence and health outcomes relative to oral antipsychotics. Despite their benefits, LAIAs remain underprescribed. Administering LAIAs in community pharmacies could improve uptake, but the feasibility of integrating new services in this setting is often disregarded. The authors aimed to evaluate the feasibility, acceptability, and appropriateness of administering LAIAs in community pharmacies in Washington State. METHODS: Community pharmacy staff and psychiatri

Beyond the Counter: The Community Pharmacist of the Future.

Fifteen years ago, a community pharmacist's role in the United States was dispensing medication. Five years ago, it expanded to include administering routine vaccines. Fifteen years from now, the authors posit that it will include minor acute illness evaluation and treatment, chronic disease prevention, and complex disease management, including medication prescribing and administration. This evolution, which just began recently in the United States but is well under way in many other countries, would help to relieve pressure on primary care, control costs, improve outcomes, and increase access

Phenylpropanolamine hydrochloride.

Phenylpropanolamine has been classified by the FDA advisory review panel as generally safe and effective for short-term weight control. The maximum daily dose for this indication is 75 mg. When used in recommended doses by patients without significant risk factors, serious side effects appear to occur infrequently. The pharmacist has an important role in preventing misuse and abuse of phenylpropanolamine through counseling the patient about its appropriate, short-term (not exceeding 12 weeks) use in a comprehensive weight-reduction program. Patients should be alerted to signs and symptoms of s
