THE BIOLOGIC BASIS OF TRANSGENDER IDENTITY: 2D:4D FINGER LENGTH RATIOS IMPLICATE A ROLE FOR PRENATAL ANDROGEN ACTIVITY.

OBJECTIVE: The biologic basis for gender identity is unknown. Research has shown that the ratio of the length of the second and fourth digits (2D:4D) in mammals is influenced by biologic sex in utero, but data on 2D:4D ratios in transgender individuals are scarce and contradictory. We investigated a possible association between 2D:4D ratio and gender identity in our transgender clinic population in Albany, New York. METHODS: We prospectively recruited 118 transgender subjects undergoing hormonal therapy (50 female to male [FTM] and 68 male to female [MTF]) for finger length measurement. The c

Neurocognitive screening of lead-exposed andean adolescents and young adults.

This study was designed to assess the utility of two psychometric tests with putative minimal cultural bias for use in field screening of lead (Pb)-exposed Ecuadorian Andean workers. Specifically, the study evaluated the effectiveness in Pb-exposed adolescents and young adults of a nonverbal reasoning test standardized for younger children, and compared the findings with performance on a test of auditory memory. The Raven Coloured Progressive Matrices (RCPM) was used as a test of nonverbal intelligence, and the Digit Span subtest of the Wechsler IV intelligence scale was used to assess auditor

Pre-injury level of anxiety is associated with the rate of digit replant failure: A prospective cohort study.

BACKGROUND: Previous studies have demonstrated that age, smoking, Tamai's level of amputation, causes of injury and ischemia time were associated with the success rate of digit replantation. The primary objective of this study is to investigate whether mental status including anxiety and depression is associated with the rate of digit replant failure. METHODS: This study included 134 digits from 102 patients who received digital replantation after complete amputation from 1 September 2013 to 1 September 2015. The Zung self-rating anxiety scale (SAS) and the Zung self-rating depression scale (

Cognitive heuristics in the operating room: the role of 'left-digit bias' in ophthalmic surgical selection.

INTRODUCTION: Left-digit bias is a cognitive heuristic in which the leftmost digit of a number disproportionately influences judgment. This study evaluates the impact of perceived patient age on ophthalmic surgical decision-making. METHODS: Using a multicentre federated electronic health record network, patients aged 40-85 years with a routine health visit, followed by at least 1 documented ophthalmic examination and a diagnosis code specific to the surgical outcome under study, were followed for 1 year. Patients were grouped into consecutive 2-year age cohorts and matched 1:1 on demographic

Development of an Amyloidosis Risk Score for Positive Tenosynovial Biopsy at Carpal Tunnel Release.

PURPOSE: Tenosynovial biopsy at the time of carpal tunnel release affords a unique opportunity for the early diagnosis of amyloidosis. We aimed to build a risk score to stratify patients likely to have undiagnosed amyloidosis. METHODS: We conducted a prospective cohort study of 320 adult patients with carpal tunnel syndrome undergoing carpal tunnel release. Patients underwent tenosynovial biopsy at the time of surgery. The primary outcome measure was a positive biopsy for amyloid deposition as assessed by Congo red and sulfated Alcian blue staining. We generated a risk score by assigning poin

Neurocognitive performance in relapsing-remitting multiple sclerosis patients is associated with metabolic abnormalities of the thalamus but not the hippocampus- GABA-edited 1H MRS study.

OBJECTIVES: Multiple sclerosis (MS) is an inflammatory demyelinating disease that may cause physical disabling as well as cognitive dysfunction. The presented study investigated how the neuropsychological status depends on the thalamus and hippocampus's metabolic processes, using γ-aminobutyric acid-edited magnetic resonance spectroscopy (GABA-edited 1H MRS) in patients with early MS, and how the results differ from healthy volunteers. METHODS: We recruited 36 relapsing-remitting (RRMS) MS patients and 22 controls (CON). In addition to common 1H MRS metabolites, such as N-acetyl-aspartate (tN

Fundamental ratios and logarithmic periodicity in human limb bones.

Fundamental mathematical relationships are widespread in biology yet there is little information on this topic with regard to human limb bone lengths and none related to human limb bone volumes. Forty-six sets of ipsilateral upper and lower limb long bones and third digit short bones were imaged by computed tomography. Maximum bone lengths were measured manually and individual bone volumes calculated from computed tomography images using a stereologic method. Length ratios of femur : tibia and humerus : ulna were remarkably similar (1.21 and 1.22, respectively) and varied little (<7%) between

Slowed conduction velocity of the median sensory nerve across the carpal tunnel in normal adults.

OBJECTIVE: To examine the difference of using onset or peak latency in the segmental conduction study of the median nerve in normal adults without carpal tunnel syndrome. DESIGN: We investigated 60 hands of 30 young healthy adults (15 men and 15 women; mean age, 27.1 yrs; range, 18-36 yrs). We performed the mid-palm segmental conduction study using antidromic and orthodromic techniques in the median sensory nerves of the second and third digits and the onset and peak latencies of the proximal (wrist-palm) and distal (palm-digit) segments were measured. The distoproximal latency ratios were co

T1W/T2W Ratio to Assess Brain Microstructural Damage in Multiple Sclerosis: A Multicenter Study.

BACKGROUND AND PURPOSE: The T1-weighted/T2-weighted ratio, a proposed surrogate measure of myelin integrity/damage, has debated specificity in multiple sclerosis. We investigated compartment-specific T1-weighted/T2-weighted alterations across multiple sclerosis phenotypes and age at onset, and their associations with disability and cognition. MATERIALS AND METHODS: In this multicenter study, 918 multiple sclerosis and 272 healthy controls from an Italian neuroimaging initiative underwent standardized 3D T1- and T2-weighted MRI (3D or 2D dual-echo sequences depending on site), together with cl

A useful electrophysiologic parameter for diagnosis of carpal tunnel syndrome.

In 43 patients (50 hands) with clinical manifestations of mild-moderate carpal tunnel syndrome (CTS) and 36 healthy volunteers (40 hands), orthodromic sensory nerve conduction velocity (SNCV) was measured with surface electrodes in the median nerve between the third digit and palm and between the palm and wrist. These figures were used to calculate the ratio of distal to proximal conduction (distoproximal ratio). All 90 hands were also subjected to other nerve conduction studies used for diagnosis of CTS. All control hands presented distoproximal ratios < 1.0 reflecting higher conduction rates

Sport-Specific Neuromuscular Profiles in Male Youth Swimmers and Sailors: A Comparative Analysis.

Understanding the distinct biomechanical and neuromuscular demands of aquatic sports is key to developing targeted training strategies for youth athletes. This study compared neuromuscular profiles between male youth Thai swimmers and Optimist sailors by examining the Dynamic Strength Index (DSI) in upper and lower limbs. A secondary aim was to assess the relationships between the biological maturity, estimated via 2D:4D digit ratios and predicted peak height velocity (PHV) and neuromuscular performance. Forty male athletes (n = 20 per group; mean age: swimmers = 12.9 ± 0.7, sailors = 13.3 ± 0

A Bayesian argument against rigid cut-offs in electrodiagnosis of median neuropathy at the wrist.

BACKGROUND: Nerve conduction (NC) tests, using rigid cut-offs separating normal from abnormal test values, are commonly used to confirm median neuropathy at the wrist (MNW). The authors studied patients with clinically defined mild MNW and a normal median distal motor latency to determine 1) how much sensory or mixed NC test results increase (or decrease) the probability of MNW and 2) the NC test values required to confirm (or exclude) MNW for the range of pretest probabilities of MNW. METHODS: Palmar, digit 4 (D4), and digit 2 (D2) median NC tests were reviewed in 125 hands with mild carpal

Differences in the response to TNF inhibitors at distinct joint locations in patients with psoriatic arthritis: results from nine European registries.

BACKGROUND: Efficacy of tumour necrosis factor inhibitors (TNFi) for peripheral arthritis in patients with psoriatic arthritis (PsA) has been established in randomized clinical trials that have used improvement in summated joint counts as an outcome. Whether joints at different anatomical locations might respond differentially to TNFi remains unknown. The aim of the study was to investigate potential variations in the responsiveness to a first tumour necrosis factor inhibitor (TNFi) among joints at distinct locations in patients with psoriatic arthritis (PsA) treated in routine clinical care.

Thermal perception thresholds among young adults exposed to hand-transmitted vibration.

INTRODUCTION: Quantitative sensory testing assesses non-invasively the function of the sensory pathways from receptors to cortex. Studies of workers exposed to vibration support evidence that neuro-sensory hand-arm vibration syndrome also encompasses neuropathy of the small-diameter nerve fibres. OBJECTIVES: To assess the risk of disturbed thermal perception developing among young adults exposed to vibration and hand-intensive manual work. The aim also encompasses the study of alternative covariates in small-diameter nerve fibre neuropathy assessment. METHODS: This cross-sectional multi-cent

Associations between plasma clozapine/N-desmethylclozapine ratio, insulin resistance and cognitive performance in patients with co-morbid obesity and ultra-treatment resistant schizophrenia.

Clozapine (CLZ), the sole antipsychotic with superior efficacy for ultra-treatment resistant schizophrenia (TRS), is limited by adverse effects, including metabolic dysregulation. Clozapine's main metabolite, N-desmethylclozapine (NDMC), has potent 5-HT2C antagonist properties which may explain this metabolic dysfunction, thus the CLZ:NDMC ratio is of particular interest. High insulin resistance states could be associated with CYP1A2 induction and lower CLZ:NDMC ratios. Additionally, lower CLZ:NDMC ratios have been associated with better cognitive, but worse metabolic functioning. This study i
