Similar Midterm Clinical Outcomes and Aortic Remodeling in Bifurcated and Straight Aortic Stent Grafts Used to Treat Isolated Abdominal Aortic Dissection.

PURPOSE: The optimal treatment for isolated abdominal aortic dissection (IAAD) is currently unknown. We compared the effects of straight and bifurcated aortic stent grafts on postoperative aortic remodeling in patients with IAAD. MATERIALS AND METHODS: From February 2012 to December 2019, 57 patients with IAAD were treated using endovascular methods, including either a bifurcated or a straight aortic stent graft. The clinical features, risk factors, computed tomography angiograms, midterm follow-up results, and aortic remodeling of these patients were reviewed and analyzed. RESULTS: In total

Two-stage surgery to repair a dissecting abdominal aortic aneurysm in a severely obese patient: Open bifurcated graft replacement after laparoscopic sleeve gastrectomy.

With the increasing prevalence of severe obesity worldwide, surgical treatment for severely obese patients is becoming more popular. Bariatric surgery has occasionally been performed as a precursor to major operations for serious diseases to make these difficult surgeries safer for severely obese patients. We present the case of a severely obese patient with a dissected abdominal aortic aneurysm and left iliac artery aneurysm. Initially, we performed bariatric surgery on this patient to reduce perioperative risk and then subsequently performed bifurcated graft replacement. A 54-year-old man pr

Repair of complicated type B dissection with an isolated left vertebral artery using the stented elephant trunk technique.

OBJECTIVES: The presence of an isolated left vertebral artery (ILVA) remains a challenging issue for thoracic endovascular aortic repair (TEVAR) of type B dissection if the proximal landing zones are inadequate. We retrospectively reviewed our experience of the surgical management of complicated type B dissection with an ILVA using the stented elephant trunk technique. METHODS: Between February 2009 and May 2013, 7 patients with complicated type B dissection (acute = 2 and chronic = 5) underwent the stented elephant trunk procedure under hypothermic cardiopulmonary bypass with selective anteg

Outcomes of fenestrated frozen elephant trunk technique in 150 patients with acute type A aortic dissection.

OBJECTIVE: The fenestrated frozen elephant trunk (FET) technique provides proximalization of distal anastomosis and antegrade blood flow into supra-aortic vessels through fenestration in a FET. We investigated the outcomes of the fenestrated FET technique in acute type A aortic dissection. METHODS: We evaluated 150 patients who underwent arch repair using the fenestrated FET technique for acute type A aortic dissection between July 2014 and January 2023. FET was deployed under hypothermic circulatory arrest and manually fenestrated under direct vision on the supra-aortic vessel aspect. Fenest

Clinical characteristics and outcomes in Takotsubo syndrome vs. spontaneous coronary artery dissection: a systematic review and meta-analysis.

BACKGROUND: Although a large body of literature describes Takotsubo syndrome (TTS) and spontaneous coronary artery dissection (SCAD) as having overlapping clinical features and benign outcome measures, they differ significantly in their pathophysiological mechanisms, demo-graphic profile, and natural history. Herein, we sought to investigate differences in clinical profile and outcomes between patients of these two conditions. METHODS: Following PRISMA guidelines, we compared TTS and SCAD in adult patients regarding epidemiological, clinical, and prognostic features. A systematic search of Pu

Black blood MRI in suspected large artery primary angiitis of the central nervous system.

OBJECTIVES: Single case reports suggest that black blood MRI (T1-weighted fat and blood suppressed sequences with and without contrast injection; BB-MRI) may visualize intracranial vessel wall contrast enhancement (CE) in primary angiitis of the central nervous system (PACNS). In this single-center observational pilot study we prospectively investigated the value of BB-MRI in the diagnosis of large artery PACNS. METHODS: Patients with suspected large artery PACNS received a standardized diagnostic program including BB-MRI. Vessel wall CE was graded (grade 0-2) by two experienced readers blind

What is the best thoracic endovascular aortic repair landing zone for Type B aortic dissection?

Dissection morphology is important when considering landing zones for thoracic endovascular aortic repair. Landing proximally in a healthy aorta, free of dissection and intramural hematoma, is generally recommended. For the most common location of the proximal entry tear near the left subclavian artery, this generally requires landing in zone 2. However, zone 2 landing is associated with higher rates of stroke and need for left subclavian revascularization. Distal landing zone 5 is associated with much higher rates of spinal cord ischemia, without significant benefit over zone 4 or bare metal

Florida elephant trunk hybrid aortic repair (FETHAR): a novel frozen elephant trunk technique with complete debranching sparing endograft modification.

Treatment of aortic arch pathology is rapidly evolving. Current techniques utilize a combination of open and endovascular approaches. Existing techniques such as B-SAFER require endograft modification for complete arch debranching which may predispose the graft to failure or endoleak. We developed the FETHAR, a novel method for hybrid aortic repair and demonstrate its use in a patient who presented with an acute dissection with contained rupture. Following repair, we demonstrate excellent seal without endoleak on the post-operative CT scan. Ultimately, this repair allows for complete arch debr

Successful extra-anatomical recanalization of occluded superficial femoral arteries using the Outback device--a report of 2 cases.

INTRODUCTION: We report 2 cases where the Outback catheter facilitated extra-anatomical bypass after vessel perforation during attempted subintimal vessel dissection. REPORT: Attempted subintimal angioplasty of the superficial femoral artery (SFA) resulted in vessel perforation in 2 patients with chronic SFA occlusion and limb ischemia. Due to the lack of other endovascular or surgical options, the Outback catheter was used to reenter the patent lumen distal to the perforation. A stent graft was then deployed from proximal to the perforation to beyond the reentry point with successful outcome

Lateral Node Dissection in Rectal Cancer in the Era of Minimally Invasive Surgery: A Step-by-Step Description for the Surgeon Unacquainted with This Complex Procedure with the Use of the Laparoscopic Approach.

INTRODUCTION: Lateral node dissection in rectal cancer has been routinely performed in Eastern countries. Technical and anatomical challenges and potential significant postoperative morbidity associated with the procedure have prevented its implementation into clinical practice in Western countries. However, the minimally invasive approach may offer the opportunity of performing this complex procedure with precise anatomical dissection and minimal intraoperative blood loss. In this setting, proper training and standardization of technical steps is highly warranted for surgeons not fully acquai

Utility of the variable flip angle 3D fast-spin echo (isoFSE) sequence on 3T MR for diagnosing vertebrobasilar artery dissection.

We aimed to investigate the utility of the isoFSE sequence, one of the variable flip angle 3D fast-spin echo sequences, on 3T-MR for displaying vessel walls and diagnosing vertebrobasilar artery dissection (VAD). We retrospectively evaluated 12 initial and 28 follow-up images from 12 patients diagnosed with either intracranial VAD or carotid artery dissection. The image quality for displaying the vessel wall was scored using a five-point scale (1 poor, 5 excellent) on initial T1-weighted isoFSE images for each region of the arteries. The intracranial artery dissection findings assessed at time
