This article reviews how the Nordic countries of Denmark, Finland, Norway, and Sweden enforce their legislation pertaining to driving under the influence of alcohol and/or other impairing drugs. The evidence necessary for a successful prosecution of traffic offenders has undergone radical changes over the past 50 years. The once widely used clinical tests of impairment are no longer a major element of the prosecution case and a physician is more seldom required to examine apprehended drivers and document any clinical signs and symptoms of alcohol and/or drug influence. These clinical tests hav

OBJECTIVE: There is a substantial body of evidence that the recidivism of impaired-driving offenders is reduced while an ignition interlock device (IID) is on their vehicles. This study examines changes in driving behaviors and drinking behaviors used by DWI offenders to manage driving with the IID.
METHODS: A total of 166 IID participants who completed two surveys covering the period from arrest to IID installation (T1) and during IID use (T2) were examined. Four domains were covered: demographics, driving environments and transportation needs, reported driving activity, and reported drinkin

INTRODUCTION: The on-the-road highway driving test is generally regarded as a gold standard for assessing drug-induced driving impairment. The primary outcome measure is the standard deviation of lateral position (SDLP), a measure of road tracking error or "weaving". The test has been calibrated for incremental doses of alcohol almost 30 years ago in order to define the impact of drug-induced impairment in terms of blood alcohol concentration (BAC) equivalents. Drug-induced changes in SDLP exceeding 2.4 cm have been evaluated as clinically relevant ever since. The present analysis was conducte

OBJECTIVE: This study aims to explore the role of peer and parental influences on adolescent driving behaviors, particularly concerning distracted and alcohol-impaired driving, in light of the significant number of road accidents and fatalities involving young drivers.
METHODS: A cross-sectional study was conducted using data from the IMPACT program. Adolescents aged 14 to 19 in the Richmond area were recruited from local high schools through convenience sampling. Parental consent was obtained via media release forms provided by Virginia Commonwealth University (VCU), with students having the

BACKGROUND: Driving under the influence of cannabis is associated with impaired cognitive and psychomotor performance and an increased risk of traffic accidents. Reliable real-time in-vehicle systems for detecting cannabis-related driving impairment are currently lacking; but hold great potential for improving road safety.
METHODS: This protocol describes the REVELIO test-track pilot study: a randomised, controlled, open-label, interventional, single-centre trial. The study assesses the feasibility and methodological requirements for developing and evaluating a multimodal in-vehicle detection

Stimulant use while driving is a high-risk factor for collisions and fatalities. In recent years, several strategies to curtail impaired driving were employed on highways, including on-site oral fluid testing. This study evaluated four roadside oral fluid testing devices (AquilaScan®, Dräger DrugTest®, Druglizer®, and DrugWipe®) for the detection of amphetamine, methamphetamine, and cocaine in oral fluid from Brazilian drivers. Overall, 8985 screening tests were conducted, and LC-MS-MS analysis was performed on 46% of the oral fluid samples. Screening and confirmatory test results were compare

OBJECTIVE: Preventing Cannabis-impaired driving involves understanding how users assess risk, monitor their use, and plan to get home safely. While extant research has shown substantial heterogeneity in patterns of cannabis use among different user groups, far less research has examined self-regulation among users. The current study aims to identify sub-groups of individuals who used or have used cannabis based on how they perceive risks, monitor their impairment, and plan to avoid driving under the influence of cannabis (DUIC) to examine how the different profiles relate to DUIC outcomes.

Southeast Asia plays a vital role in the global alcohol trade, with rapid economic growth and increasing intra-regional travel contributing to shifts in alcohol consumption. This descriptive review of publicly available data sources aims to assess the extent to which alcohol control policies in Southeast Asia align with the World Health Organization (WHO)'s SAFER framework. Data on alcohol consumption, gross domestic product (GDP) per capita, religious demographics, and life expectancy were obtained from recent publicly available sources, including the World Bank Group and the WHO. Policy data

OBJECTIVE: Driving while intoxicated (DWI) remains a preventable source of morbidity and mortality in the United States. Ignition interlock devices (IIDs) are used to prevent DWI offenders from driving while intoxicated during a mandated installation period and are effective during that time. Once IIDs are removed, DWI rates are similar to levels for offenders who had no IID. This study tested the efficacy of a smartphone app (B-SMART) for DWI offenders with an IID and concerned family members (CFMs), with the goal of reducing IID lockout events and alcohol consumption.
METHOD: Four B-SMART a

The ratio of blood-alcohol concentration (BAC) to breath-alcohol concentration (BrAC), which is commonly referred to as the blood/breath ratio (BBR), is an important concept in forensic science and legal medicine. For example, the BBR serves as the calibration factor used when a breath-alcohol test result is converted into the coexisting BAC for clinical, research, and forensic purposes. Furthermore, when legislative bodies established statutory BrAC limits for driving, they divided the statutory BAC limit by an assumed population average BBR, hence BrAC = BAC/BBR. However, jurisdictions opted

The aim of the study was to determine the components of measurement uncertainty in the concentration of alcohol in exhaled breath and to determine the state of sobriety at the time of incident. Based on the literature review and the authors' experience in providing opinions for law enforcement and the judiciary, the influence of various factors on the final interpretation of sobriety state is described on the basis of measurement uncertainty of breath analyzers, uncertainty of retrospective and prospective calculations, and uncertainty related to the conversion of alcohol concentrations detect

BACKGROUND & OBJECTIVES: This study reports the prevalence and concentrations of sedative-hypnotic drugs as exemplified by benzodiazepines (BZD) and zolpidem (Z-hypnotic) in blood samples from drivers involved in road traffic accidents (RTA) in the Padova region of Italy. Another aim of the study was to estimate the prevalence of these drugs with concentrations in blood above the therapeutic intervals and above specific per se limits.
METHODS: A total of 4066 blood samples collected from drivers involved in RTA were analysed for the presence of alcohol, drugs of abuse and medicinal drugs with

Rapid oral fluid testing (ROFT) devices have been extensively evaluated for their ability to detect common drugs of abuse; however, the performance of such devices on simultaneous screening for ketamine has been scarcely investigated. The present study evaluated three ROFT devices (DrugWipe® 6S, Ora-Check® and SalivaScreen®) on the detection of ketamine, opiates, methamphetamine, cannabis, cocaine and MDMA. A liquid chromatography tandem mass spectrometry (LCMS) assay was firstly established and validated for confirmation analysis of the six types of drugs and/or their metabolites. In the fiel

BACKGROUND AND AIMS: Several publications have suggested increasing cannabis potency over the last decade, which, together with lower amounts of cannabidiol (CBD), could contribute to an increase in adverse effects after cannabis smoking. Naturalistic studies on tetrahydrocannabinol (THC) and CBD in blood samples are, however, missing. This study aimed to investigate the relationship between THC- and CBD concentrations in blood samples among cannabis users, and to compare cannabinoid concentrations with the outcome of a clinical test of impairment (CTI) and between traffic accidents and non-ac

BACKGROUND: In some jurisdictions, persons who are convicted of driving under the influence of alcohol (DUI) are allowed to serve some portion of their prison sentence under home confinement as part of Intensive Supervision Programs (ISPs) which include pre-release psycho-education and close post-release supervision.
OBJECTIVES: Test the hypothesis that persons convicted of DUI offenses who have spent some portion of their sentence under home confinement, as compared to a historical comparison group, will exhibit a relatively low re-conviction rate.
METHODS: Using administrative data for 1,4

Repeatedly Crash-Involved Drivers (RCIDs) pose significant challenges to traffic safety, contributing disproportionately to crash occurrences and their severe consequences. While existing research has explored factors influencing crash involvement, the literature often neglects the influence of a driver's crash history and inter-crash intervals on their evolving crash risk. Additionally, many traditional models fail to address unobserved heterogeneity, limiting their ability to capture the complex interplay of factors contributing to repeated crash involvement. This study investigates the fact

This technical note reviews the plethora of concentration units used to report blood-alcohol concentration (BAC) and breath-alcohol concentrations (BrAC) for legal purposes in different countries. The choice of units sometimes causes confusion when scientific papers originating from a certain country might be introduced into evidence via expert testimony, such as when alcohol-related crimes are prosecuted. The concentration units are also important to consider when blood/breath ratios (BBRs) of alcohol are calculated and compared between countries. Statutory BAC limits for driving in most nati

OBJECTIVE: Very little is known about rural female impaired drivers despite disproportionate rates of impaired driving arrests and associated traffic fatalities in rural areas. The present study examined past-year impaired driving histories and impaired driving correlates in a sample of rural female drug-involved offenders.
METHODS: Female drug-involved offenders (N = 400) from 3 rural jails completed a confidential interview focused on substance use and related risk behaviors. After removing cases with missing data (n = 23), participants self-reporting past-year impaired driving (n = 254) we

INTRODUCTION: Crashes involving drinking drivers represent as much as one-third of all fatal crashes around the world. Progress has been made in reducing this toll through a series of interventions that attempt to discourage driving while intoxicated (DWI) and reoffending among drivers who have been convicted of DWI. However, these approaches cannot eliminate the problem. In-vehicle technologies are being developed, such as the Driver Alcohol Detection System for Safety-commonly referred to as DADSS-that have the potential to prevent alcohol-impaired drivers from driving their vehicles. DADSS

OBJECTIVE: Cannabis-impaired driving (CID) is an increasing public health concern, particularly among emerging adults. Despite growing prevalence, little is known about episode-level patterns of use and impairment before driving. This study examined detailed features of recent and heaviest CID episodes to better understand high-risk driving contexts.
METHOD: Participants were 149 emerging adult cannabis users (ages 18-29) reporting ≥3 instances of driving after cannabis use in the past 3 months. Event-level data were collected via retrospective self-report of the most recent and heaviest CID
