OBJECTIVE: To understand how primary care physicians manage patients with dysphonia and the barriers they face when evaluating patients for voice problems.
STUDY DESIGN: Cross-sectional survey.
SETTING: Primary care community.
SUBJECTS AND METHODS: A total of 933 internal and family medicine physicians were randomly selected from a database of physicians in a referral basin of a tertiary care medical center and mailed a questionnaire. Questions concerned physician comfort level in recognizing an abnormal voice, their view of the quality of life impact of dysphonia, frequency of evaluating p

OBJECTIVE: This guideline provides evidence-based recommendations on managing hoarseness (dysphonia), defined as a disorder characterized by altered vocal quality, pitch, loudness, or vocal effort that impairs communication or reduces voice-related quality of life (QOL). Hoarseness affects nearly one-third of the population at some point in their lives. This guideline applies to all age groups evaluated in a setting where hoarseness would be identified or managed. It is intended for all clinicians who are likely to diagnose and manage patients with hoarseness.
PURPOSE: The primary purpose of

Voice production is a complex process that involves more than one system, yet most causes of dysphonia are attributed to disturbances in the laryngeal structures and little attention is paid to extralaryngeal factors. Persistent dysphonia after general anesthesia is a challenge to both anesthesiologists and otolaryngologists. The etiology is often multivariable and necessitates a team approach for proper diagnosis. Laryngeal symptoms are subdivided into phonatory disturbances and airway related complaints. When they become persistent for more than 72 hours or are coupled with airway symptoms s

OBJECTIVES: This study was performed to determine whether and how unilateral vocal fold paralysis (UVFP) affects the production and perception of voiced stop consonants as compared with unvoiced stops, and to analyze the phonetic effects of UVFP on the voicing feature.
METHODS: Phonetic constructs pronounced by 7 male patients with UVFP and 5 normal male subjects were recorded. The 432 speech tokens consisted of intervocalic, prevocalic, and postvocalic stop consonants (/p/, /t/, /k/, /b/, /d/, /g/) in the vowel contexts /a/ and /i/. Perceptual consonant identification testing was performed w

Botulinum toxin is the accepted method of managing SD and has given most patients with adductor SD the ability to develop satisfactory vocal function. Results have been less satisfactory in abductor SD. Management is accomplished best with a team approach. The physiatrist with experience in EMG and botulinum toxin usage can be a prominent member of this team.

It is essential for laryngologists, speech and language pathologists, and others caring for professional voice users to be familiar with the potential vocal effects of any substance ingested by a professional voice patient. This article reviews pharmacologic agents that may have laryngeal side effects. Essentially, any ingested substance is suspect as a cause for voice dysfunction. It is also necessary for laryngologists to educate voice professionals about the consequences of drugs on the voice and the potential problems associated with properly prescribed medication.

Children's voice disorders may be functional or organic in cause and a thorough evaluation by the otolaryngologist and speech-language pathologist is necessary to obtain the proper diagnosis, to understand the underlying etiologic factors, and to prepare a plan of treatment that is appropriate for the child. Because voice disorders may be present with developmental communication problems, treatment must include all factors that affect growth and development of the child.

Benign laryngeal disorders result in dysphonia because of effects on glottic closure and the vibratory characteristics of the true vocal fold. Treatment is initially directed at reversing medical conditions and patterns of abuse with surgery reserved for unresolving lesions resulting in troublesome dysphonia. Benign lesions that require surgery are excised as precisely as possible sparing overlying mucosa and the underlying vocal ligament. Vocal fold scarring is currently best treated by augmentation procedures, and atrophy may be compensated for by medialization thyroplasty or by adding bulk

Clinical, neurological, endoscopic, psychological findings, questionnaire data on vegetative sphere, diaphragm x-ray, articulation test and Viene test system evidence obtained on 25 patients with phonic spasm confirm organic neurological nature of spastic dysphonia as focal muscular dystonia. This condition can be accompanied with tremor, rotatory, winking and writers' spasms, oromandibular dystonia. As indicated by positive treatment outcomes, combined treatment of phonic spasm with GABA-ergic drugs of clonazepam (antelepsin) and baclofen, orthophonic voice correction, physiotherapy is pathog

In order to estimate the significance of the psychogenic factor in the etiology of functional voice disorders on the basis of relevant psychological dimensions we compared patients with functional dysphonia, aphonia and a normal control group by means of an empirical-psychological test procedure. Psychogenic conditions proved to be a major factor in the etiology of aphonia, whereas such an interrelation turned out only partially in patients with functional dysphonia. The conclusion can be that psychogenic aspects are not necessarily of exceeding importance in the etiology of functional dysphon

Options in the treatment of spasmodic dysphonia include surgical interruption of the recurrent laryngeal nerve, anterior laryngoplasty, voice therapy, and Botulinum toxin injection. Although none of these treatments is ideal, Botulinum toxin injection appears to have the greatest potential to benefit the greatest number of patients.

One thousand consecutive patients seen in a multidisciplinary voice clinic are reviewed. The incidence and relative severity of diagnostic features in each of five assessment areas are calculated and compared. The five areas include: 1. history, 2. laryngoscopic examination, 3. perceptual-acoustic assessment, 4. voice related musculoskeletal, and 5. psychological evaluations. Feature prevalence is presented for patients we have classified as having muscular tension dysphonia, which can be primary, or associated with vocal nodules, chronic laryngitis, or polypoidal degeneration; or as having a

Spastic dysphonia is a disorder of phonation which is characterized by a strained, creaking, and choked vocal attack, a tense and squeezed voice sound. Spastic dysphonia in a functional voice disorder can be classified into two types from the viewpoint of activities of the extrinsic and intrinsic laryngeal muscle groups. A functional voice disorder pertaining to abnormal activities of the intrinsic and extrinsic laryngeal muscles results in spastic dysphonia. The adductor spastic dysphonia may be due to abnormal actions of the intrinsic laryngeal muscles as such is relieved by sectioning of th

Recently, two types of spastic dysphonia, adductor and abductor, have been proposed. The literature suggests that patients may be categorized as having one form of the disorder or the other. The present study presents case history and spectrographic data on a patient who evidences vocal characteristics of both disorder types. It is suggested that spastic dysphonia is a disorder involving a harshness-breathiness continuum and is not amenable to strict, binary categorization.
