Superior sensitivity of motor over somatosensory evoked potentials in the diagnosis of cervical spondylotic myelopathy.

Myelopathy secondary to cervical spondylosis is often a difficult clinical diagnosis. Furthermore, with the introduction of magnetic resonance imaging (MRI) an increasing number of patients are identified with spondylotic cervical spinal cord compression. We analyzed the value of functional assessment of the spinal cord by motor and sensory evoked potentials (MEP and SEP) in the detection of myelopathy, with special emphasis on the correlation of clinical and electrophysiological findings. Fifty-one patients with at least some degree of spinal cord compression because of cervical spondylosis,

The use of motor evoked potentials in the diagnosis of psychogenic quadriparesis. A case study.

We present a case illustrating the usefulness of motor evoked potentials (MEPs) in differentiating psychogenic from organic postoperative paralysis. Discussed is a 12-year-old girl who underwent surgery for the repair of a recurrent syringomyelia. On the 6 day after a proximal revision of her syringoperitoneal shunt she returned to the hospital with deep quadriparesis, bowel and bladder incontinence, and complaining of severe headache. An MRI scan showed the syrinx to be collapsed, and removal of the shunt had no impact on her clinical symptoms. Repeat somatosensory evoked potentials (SEPs) sh

Post-tetanic transcranial motor evoked potentials augment the amplitude of compound muscle action potentials recorded from innervated and non-innervated muscles.

BACKGROUND CONTEXT: Transcranial electrical stimulation used to produce motor evoked potentials (TES-MEPs) and subsequent compound muscle action potential (CMAP) recording is widely used to monitor motor function during surgery when there is risk of damaging the spinal cord. Nonetheless, some muscles do not produce CMAP amplitudes sufficient for intraoperative monitoring. PURPOSE: This study aimed to investigate the utility of tetanic stimulation at single and multiple peripheral nerve sites for augmenting CMAP amplitudes recorded from innervated and non-innervated muscles. Anesthetic technique for awake artery malformation clipping with motor evoked potential and somatosensory evoked potential: A case report.

BACKGROUND: Awake craniotomy has been widely used for tumor resection, epilepsy surgery, deep brain stimulation, and carotid endarterectomy. The report on awake artery malformation clipping is rare, especially for anesthesia management. CASE SUMMARY: A 62-year-old female diagnosed with malformation of anterior cerebral artery at the right side. We clipped the artery malformation with intraoperative neuromonitoring (IONM) in awake craniotomy. Spontaneous respiration was maintained throughout the procedure by nasopharyngeal airway during the surgery successfully. CONCLUSION: The technique of m

Intraoperative neuromonitoring with direct cortical stimulation motor-evoked potentials in supratentorial glioma surgeries with preoperative moderate-to-severe motor weakness.

PURPOSE: Intraoperative neuromonitoring (IONM) is essential for safe brain tumor surgeries. Little is known about how to assess damaged motor function. To preserve damaged motor function in these patients, we evaluated the results of direct cortical stimulation motor-evoked potential (D-MEP) monitoring in glioma surgeries performed in and near the primary motor cortex (PMC) and cortico-spinal tract (CST). METHODS: IONM using D-MEPs in patients with preoperative moderate-to-severe motor weakness was performed in 16 cases (14 patients). Before the tumor resection, a single six-contact subdural

Monitoring of facial muscle motor evoked potentials during microvascular decompression for hemifacial spasm: evidence of changes in motor neuron excitability.

OBJECT: Hemifacial spasm (HFS) is thought to be due to a hyperactive facial motor nucleus consequent to chronic neurovascular contact. The lateral spread (LS) response is presumed to reflect changes in facial motor neuron excitability. Facial muscle motor evoked potentials (MEPs) use the same efferent pathway as LS, therefore the authors speculated that these potentials should reflect differences consistent with changes at the facial motor nucleus level. METHODS: Monitoring of LS and bilateral facial MEP was performed in 10 consecutive patients undergoing MVD for HFS. Ipsilateral facial MEPs

Retrospective Waveform Analysis of Transcranial Motor Evoked Potentials (MEP) to Identify Early Predictors of Impending Motor Deficits in Spinal Surgeries.

PURPOSE: Although there are guidelines analyzing transcranial motor evoked potentials (MEP) waveform criteria, they vary widely and are not applied universally during intraoperative neurophysiologic monitoring (IONM). The objective is to generate hypotheses to identify early and reliable MEP waveform characteristics prior to complete loss of MEP to predict impending motor spinal cord injuries during spinal surgeries. The ultimate goal is to enhance real-time feedback to prevent injury or detect reversible spinal cord damage. METHODS: Fifteen true positive cases of persistent intraoperative ME

Reliability of intraoperative neurophysiological monitoring using motor evoked potentials during resection of metastases in motor-eloquent brain regions: clinical article.

OBJECT: Resection of gliomas in or adjacent to the motor system is widely performed using intraoperative neuromonitoring (IOM). For resection of cerebral metastases in motor-eloquent regions, however, data are sparse and IOM in such cases is not yet widely described. Since recent studies have shown that cerebral metastases infiltrate surrounding brain tissue, this study was undertaken to assess the value and influence of IOM during resection of supratentorial metastases in motor-eloquent regions. METHODS: Between 2006 and 2011, the authors resected 206 consecutive supratentorial metastases, i

Safety of Transcranial Motor-Evoked Potential Linked Quadripolar Montage.

PURPOSE: The linked quadripolar montage for transcranial electric motor-evoked potentials (qTceMEP) consists of four stimulation electrodes: two linked anodes and two linked cathodes. This montage has grown in popularity because it can result in a larger compound muscle action potential amplitude compared with the conventional bipolar montage. Despite the increasing number of centers adopting qTceMEP, no studies have investigated patient safety to date. The aim of this study is to evaluate the safety profile of qTceMEP. METHODS: A total of 3,806 spine surgeries performed at a single instituti

A Comparative Effectiveness Study of Bipolar and Linked Quadripolar Techniques for Eliciting Transcranial Motor Evoked Potentials.

INTRODUCTION: Linked Quadripolar Stimulation (LQP) is a recent adaptation of transcranial electric motor evoked potential stimulation, with proposed advantages over traditional Bipolar (BP) stimulation. This study aims to comprehensively compare BP and LQP stimulation to validate the efficacy of LQP. METHODS: BP and LQP stimulation were performed on 30 patients undergoing anterior cervical discectomy and fusion. A comprehensive assessment involved conducting four trials for each technique on each patient. An Accelerometer placed over the right masseter region recorded movement. A Mann-Whitney

Cervicothoracic trans-spinal magnetic stimulation: effect of coil position and orientation on spinal motor evoked potentials.

Stroke is a leading cause of long-term adult disability, and over 80% of survivors have persistent upper limb motor impairment. Spinal electrical stimulation has shown therapeutic potential after neurological disorders. Post-stroke applications have largely relied on invasive implants that limit clinical adoption, and non-invasive high intensity electrical stimulation may be intolerable for those with residual sensory function. Trans-spinal magnetic stimulation offers a non-invasive alternative capable of modulating residual sensorimotor pathways. Fundamental parameters, such as coil position

The Effects of Depth of Anesthesia on Muscle-Recorded Motor Evoked Potentials: A Prospective Observational Study.

BACKGROUND: Intraoperative neurophysiological monitoring is used to assess neurological function during surgeries placing the spinal cord at risk. Transcranial electrical stimulation muscle motor evoked potentials (Tc-mMEPs) are used to monitor motors tracts, but their interpretation is complicated by the large temporal variability which can result in false-positive warnings. Although the choice of anesthetic drug regimen and drug dose are often claimed to be factors causing this variability, the relationship between depth of anesthesia, quantified by processed electroencephalogram (pEEG) para

[Transcranial motor evoked potentials in resection of supratentorial brain tumors adjacent to corticospinal tract in children].

BACKGROUND: Transcranial motor evoked potentials (TMEPs) are one of the methods allowing functional preservation of motor pathways in resection of tumors adjacent to corticospinal tract. Studies of this method in pediatric neurosurgery note some difficulties in data interpretation, but most demonstrate high efficacy even in younger children. OBJECTIVE: To determine the role of TMEPs in surgical treatment of children with supratentorial tumors adjacent to corticospinal tract. MATERIAL AND METHODS: There were 82 patients with intracerebral tumors who underwent 95 surgeries between January 2016

Motor evoked potentials in standing and recumbent calves induced by magnetic stimulation at the foramen magnum.

The aims of this study were to determine reference values for magnetic motor evoked potentials (mMEPs) in calves and the influence of position during examination (standing or lateral recumbency). Reference values were determined using 41 healthy Holstein Friesian bull calves aged 1-10 months; standing and lateral recumbency were examined in 11 calves. Maximal magnetic stimulation was performed at the level of the foramen magnum with a magnetic field of 4 T at the coil surface. In standing position, distinct, reproducible mMEPs were obtained in all calves. Onset latency (LAT) (mean ± standard d

Repetitive common peroneal nerve stimulation increases ankle dorsiflexor motor evoked potentials in incomplete spinal cord lesions.

Plasticity of corticospinal tract (CST) activity likely plays a key role in motor function recovery after central nervous system (CNS) lesions. In non-injured adults, 30 min of repetitive common peroneal nerve stimulation (rCPnS) increases CST excitability by 40-50% and the effect persists for at least 30 min. The present study evaluated with transcranial magnetic stimulation (TMS) the changes in CST excitability after 30 min of rCPnS in people with foot drop due to incomplete SCI. Suprathreshold rCPnS (25 Hz, alternating 1 s on 1 s off stimulation cycle) was given for two 15-min periods, whil

Usefulness of magnetic motor evoked potentials in the surgical treatment of hemiplegic patients with intractable epilepsy.

Five hemiplegic patients with intractable epilepsy were studied with transcranial magnetic stimulation (TMS) before and after various surgical treatments. These patients had unilateral widespread cerebral lesions acquired at various times, including congenital, infantile and childhood injury. Motor evoked potentials (MEPs) of the abductor pollicis brevis (APB) muscles were simultaneously recorded on both sides following TMS of the motor cortex in the respective hemisphere using a figure-8 or circular coil. In all patients with congenital disease, the abolition of motor function in the affected
